Studies on the fibrinolytic activity of the human female genital tract.

The success of a fibrin glue depends on the correct admixture of fibrinogen, thrombin, and the proteinase inhibitor aprotinin. Using a modification of Astrups method we measured the fibrinolytic activity of surgical specimens of human fallopian tube and uterus. Increasing concentrations of aprotinin were added until there was complete inhibition of fibrinolysis. The fallopian tube and the uterus had higher intrinsic fibrinolytic activity than peripheral nerve tissue. At concentrations of between 10 and 20 KIU of aprotinin, fibrinolysis was blocked in all tissues studied.

[Velamentous insertion--epidemiology and clinical aspects of the pathological insertion of the umbilical cord].

Among 9460 birth an incidence of 1.6% of velamentous insertion of the umbilical cord is recorded and the clinical relevance of this anomaly is examined. Apart from statistical confirmation of well-known connections between membranous insertion of the umbilical cord on the one hand, and malformations, multiple pregnancies and bleeding sub partu on the other hand, the present investigation is mainly concerned with premature births and/or placental insufficiency in the case of velamentous insertion of the umbilical cord, which is associated with "late premature births" in the 35th, 36th and 37th week of pregnancy as characteristic finding.

[Reanastomosis of the rabbit fallopian tube with fibrinogen concentrate].

In order to investigate the use of fibrin glue in reconstructive surgery of the oviduct, tubal reanastomosis was performed with homologous fibrinogen concentrate in 26 rabbits. The contralateral tube was clamped. The pregnancy rate was 25%. No adhesions were seen in the area of the anastomosis on examination of the abdomen 3 months after operation.

[The effect of calf blood dialysate on the excretion of estriol in placental insufficiency].

The efficacy of a dialysate of calf blood in the treatment of chronic placental insufficiency is assessed using the urinary oestriol elimination as parameter. Compared with a control group, those cases receiving 2 times 250 ml protein-free dialysate of calf blood (Solcoseryl) in saline solution over 9 days showed a significant increase in the daily quantity of urinary oestriol elimination already from the second day of treatment onwards. Other conventional procedures in the medical treatment of placental insufficiency are discussed. Some of these are known to have secondary effects or must be regarded as invasive.

Morphology of rabbit oviduct after reanastomosis with fibrin glue.

We examined the site of experimental reanastomosis with fibrin glue in rabbit oviducts 3 months after the operation with light and scanning electron microscopy. Local tissue reaction was minimal, no fibrin clots were seen and inflammatory reactions were almost absent. On scanning electron microscopy ciliated cells were reduced in number. Morphologic changes at the site of anastomosis after the use of fibrin glue appear comparable to the alterations seen after the use of microsutures.

[The significance of clinical and morphological parameters for the prognosis of granulosa cell tumors of the ovaries].

27 women with granulosa cell tumours of the ovaries were treated at the 2nd Gynaecological Department of the University of Vienna, Medical School, during 1962 to 1976. The principal symptoms were pain and irregular menstrual haemorrhages. Hysterectomy and bilateral salpingo-oophorectomy was performed in most cases. 8 patients were additionally irradiated. 25 women had lesions of stages I and II, whereas only 2 had stage III lesions. The overall survival rate was 90% after 5 years and 77% after 10 years. All 5 patients having polymorphic tumours with a high mitotic activity died of the disease. In 4 of these cases the tumour showed a sarcomatoid pattern. The high percentage of survival rate is partly due to the high percentage of stage I and II lesions and to the favourable prognosis of highly differentiated tumours. Granulosa cell tumours should always be treated via hysterectomy and bilateral salpingo-oophorectomy. Because of the poor prognosis of cases with polymorphic tumours and in cases with a high mitotic activity, simultaneous radiochemotherapy - as in ovarian cancer - may be considered.

[Can fetal kidney dilatation be physiologic?].

Sonography was used to detect and to measure the foetal calyceal system in more than 100 cases of pregnancies ranging from the 25th week to term. In 13.6% of the investigated cases physiological dilatations can be demonstrated. There is no connection with the volume of the foetal bladder or with the extension of the maternal calyceal system. The observed dilatations disappear spontaneously post partum. References to pathological dilatation of the foetal calyceal system are discussed.

[Role of endorphins in the function of the lower urinary tract].

In order to investigate the influence of the endorphins on lower urinary tract function we antagonized the endogenous opiates with naloxone. We performed urodynamic examination in 17 incontinent women, administered intravenously 0.4 mg naloxone, and repeated the urodynamic examination. Bladder volume and pressure were not influenced by naloxone; the maximum urethral pressure showed a slight and urethral closure pressure a significant rise after naloxone treatment. Contrary to other authors, our results do not support a role of endorphins in lower urinary tract function.

[Etiology of uterine prolapse].

From the factors known to promote descensus those which may best be objectified--among them overweight, hard physical labour, weakness of the connective tissue and number of births--are examined in a group of women with descensus and a control group with regard to their valence for the development of a descensus. Log-linear models are applied in the calculation of qualitative variables. According to that, overweight means a fourfold, hard physical labour a threefold, weakness of the connective tissue a 2,7-fold and birth of more than one child a 2,2-fold risk of descensus.

[Uterine prolapse after episiotomy].

The value of episiotomy in regard to the prevention of descensus vagina et uteri is examined in a statistically reliable number of patients (64 cases). Women who were delivered without episiotomy provided the control group (70 cases). In both groups factors promoting pelvic relaxation were taken into consideration. According to the results of this examination the importance of episiotomy as prophylaxis of pelvic relaxation does not justify any obligatory use.

[Bladder dysfunction in pregnancy (author's transl)].

105 primiparous and 125 multiparous women have been asked for complaints of incontinence during their pregnancies. Multiparae suffered more often from stress incontinence, nullips reported a higher rate of urgency. The reasons and the influence of episiotomy, operative deliveries (forceps and vacuum) and pelvic floor training on incidence and recurrency of urinary incontinence during pregnancy are discussed.

[Urodynamic evaluation of the effect of neural therapy in motor and sensory urgency].

We treated 10 patients with motor or sensory urgency by paraurethral injection of a local anesthetic. After clinical and urodynamic work-up we applied 2.5 ml of 1% lidocain solution with Kollidon to delay absorption (Xyloneural) transvaginally to the right and left of the urethro-vesical junction. Each treatment cycle consisted of 3 injections with 2-week intervals between injections. 2 weeks after the last injection a repeat urodynamic investigation was done. All patients were improved by therapy: after the 3rd injection 3/10 women were free of symptoms, 6/10 women reported marked improvement, and one woman slight improvement of the symptoms of urgency and frequency. The mean bladder volume at the first sensation to void increased significantly after neural therapy (p = 0.02); other urodynamic parameters were not affected by neural therapy. No side effects were observed. We conclude that the paraurethral injection of a local anaesthetic is a simple and effective treatment in motor and sensory urgency.

[Echographic investigation of the calyceal system in cases of pyelitis gravidarum].

Dilatation of the upper urinary tract can be found even in pregnancies of healthy women. The frequency of dilatations in patients with pyelitis gravidarum is higher. Comparing the symptoms -- costovertebral tenderness and significant bacteriuria -- with the nephrosonograms it can be demonstrated that urinary tract infections incline to reoccur in patients with dilated upper urinary tract despite adequate antibiotic therapy. Echographic examination of the calyceal system during pregnancy might therefore be useful in detecting risk patients who have to undergo intense urologic surveillance.

[Intravesical prostaglandin E2 and placebo in urinary retention after gynaecological surgery (author's transl)].

In a randomized single blind trial we studied the effectiveness of intravesical prostaglandin E2 in the treatment of postoperative urinary retention after vaginal hysterectomy with colposuspension. One group of patients (n = 8) received 1.5 mg prostaglandin E2 in 50 ml 0.9% NaCl on the 9th postoperative day (72 hours after removal of the Foley catheter) and 50 ml NaCl without prostaglandin E2 on the following day. In the second group (n = 12) NaCl was instilled on the first day of treatment and prostaglandin E2 on the following day. Four hours after the instillation of prostaglandin E2 5 out of 8 women were able to void spontaneously, while after the instillation of NaCl spontaneous micturition occurred in only 2 out of 12 women (p less than 0.05). In both groups the average time period until patients were free of residual urine was 4 days after the initiation of therapy. Compared to placebo intravesical prostaglandin E2 is significantly more effective in achieving spontaneous voiding after gynaecological surgery.
