Primary health care trading companies for sustainable development.

A programme of comprehensive primary health care in Zambia has been accompanied by the emergence of autonomous, non-profit trading and production companies that sell health-related goods to communities and health institutions and find markets for locally produced good.

Typing and molecular characterization of human papillomaviruses in genital warts from South African women.

Condyloma acuminata from 12 women were examined for the presence of human papillomavirus DNA. Six of the women had HPV 6a, two had HPV 11a, one had a new HPV 6 subtype, and one a new HPV 11 subtype. The new HPV 6 subtype could be distinguished from other HPV 6 subtypes on the basis of Pst I fragments as well as Hind II fragments. The new HPV 11 subtype had a different Hpa II restriction endonuclease pattern. Restriction maps of both new subtypes were constructed. Two of the biopsies did not contain detectable HPV DNA when probed with HPV 6, HPV 11, and HPV 16. Biopsies were taken from normal tissue 1 cm away from the condyloma in 11 of the patients. Only one of these normal tissue biopsies was positive for HPV by Southern blot hybridization.

A new approach to the management of malignant ascites; a permanently implanted abdominal drain.

A new approach to the problem of intractable malignant ascites in preterminal abdominal carcinoma is presented. Seventeen patients treated with a new implantable silastic drain are described. Symptomatic relief was excellent in all patients for as long as the drain was patent. Complications included abdominal wall cellulitis in two instances and one case of peritonitis, but did not preclude continued drain function in 15 of the 17 cases. In eight of the 17 cases organisms were cultured from the ascitic fluid while the catheter was in situ. Drainage of ascitic fluid in this way appears to be a practical management for intractable malignant ascites.

Typing of human papillomaviruses in cervical intraepithelial neoplasia grade 3 biopsies from Cape Town.

DNA from 98 cervical intraepithelial neoplasia grade 3 (CIN 3) biopsies was screened, using the Southern blotting hybridization technique, for human papillomavirus (HPV) types 6, 11, 16, 18, 31, 33, and 35. HPV 16 was detected in 16 biopsies (16%), HPV 33 in eight biopsies (8%), HPV 31 in two biopsies (2%), and HPV 18 in one biopsy (1%). One of the biopsies contained both HPV 31 and 18. Six biopsies (6%) contained an HPV type very similar but not identical to HPV 16, and 35 biopsies were positive for HPV, but the Pst 1 restriction fragments were distinct from any of the HPV types used as probes. HPV was not detected in 32% (32/98) of the biopsies screened.

A clinical appraisal of stage 1 mucinous carcinoma of the ovary.

Thirty patients with stage I mucinous carcinoma of the ovary are reviewed with special reference to possible prognostic factors. The role of adjuvant therapy is discussed. Adjuvant therapy is recommended only for patients with poorly differentiated tumours and with stage Ic disease.

Combined chemotherapy and radiotherapy in patients with advanced squamous carcinoma of the cervix (cis-platinum-bleomycin-vinblastine).

In a pilot study, nine patients with advanced carcinoma of the cervix were treated with combination chemotherapy including cis-platinum, vinblastine, and bleomycin repeated at 3-week intervals for three courses followed by radiotherapy. Only one patient showed a partial response (reduction of tumor size more than 50%). There were no complete responses. Five of the nine patients have died from the disease with a median survival time of 20 months. These findings suggest that this particular combination of chemotherapy with radiotherapy does not have a significant effect in patients with advanced stage carcinoma of the cervix.

Computerized calculation of essential drug requirements.

The implementation of an effective essential drug programme, although accepted by most WHO member states is still meeting serious problems. Apart from politics the reasons for this slow implementation should be sought in organizational difficulties. A methodology for the calculation of national drug requirements has been developed for Gabon based on standard treatments and estimated number of cases per disease (also known as the demand-morbidity method). Allocation of these drugs to the Gabonese health institutions is calculated based on data from the existing health information system such as days of hospitalization and number of patient visits. A rudimentary form of the demand-morbidity method has been used manually in the past but was extremely cumbersome. Modern microcomputers and standard software have now improved speed and flexibility of the method and have therefore made its implementation feasible even in developing countries. The advantages of the method are the improved availability of essential drugs at all levels of the health system and a decrease of the required drug budget (for Gabon it was calculated that a saving of 45% could be obtained). The method makes possible interaction between health planners and peripheral staff unimaginable a few years ago.

Ovarian Sertoli-Leydig cell tumour with raised serum alpha fetoprotein. A case report.

A case of ovarian Sertoli-Leydig cell tumour with a raised serum alpha fetoprotein is reported. The patient first presented at the age of 27 years with a history of 6 years' amenorrhoea followed by 3 months irregular vaginal bleeding. A ovarian tumour was found and excised and shown microscopically to be a spindle cell malignant tumour. The patient was treated with chemotherapy and had a complete response. Thirty months after first presentation there was a recurrence in the pelvis which microscopically showed the typical features of a Sertoli-Leydig cell tumour. Six months later a second recurrence had the microscopic appearance of a lipid cell tumour. A raised serum alpha fetoprotein was found at the time of the second recurrence and immunohistochemistry showed this protein in the Leydig and luteinized cells of the recurrent tumours but not in the spindle cells of the original ovarian neoplasm.

The role of neurosurgery in the treatment of cerebral metastases from choriocarcinoma: a report of two cases.

Cerebral metastases from choriocarcinoma are a poor prognostic indicator of outcome in both the World Health Organization and FIGO classification systems. However, with the increased experience with chemotherapy and radiotherapy the prognosis of this group of patients has improved substantially. Neurosurgery remains an option for selected patients. We present two patients who underwent craniotomy as part of their management of choriocarcinoma, and review the role of neurosurgery in the treatment of gestational trophoblastic disease.
