Vena cava filter placement using a cutdown of the superficial epigastric vein.

Percutaneous placement of vena cava filters through the femoral vein has been associated with insertion site venous thrombosis. Reported incidence varies from 2% to 41%. In the majority of placements, sequential dilators are used to create the venotomy and subcutaneous tract. This technique disrupts all layers of the vein wall. The injured area may extend as far proximal as the dilator or sheath is placed. The authors present their experience with placement of vena cava filters using a cutdown of the superficial epigastric vein. During a 5-year period, 27 patients underwent placement of the LGM-Vena Tech vena cava filter via a femoral approach. A cutdown of the superficial epigastric vein was performed. The guidewire, dilator, and introducer sheath were inserted under direct fluoroscopic examination. After removal of the dilator, the LGM-Vena Tech filter was placed through the introducer. There were no wound infections and no clinical signs of insertion site venous thrombosis in the postoperative period. Insertion site venous thrombosis is a well-documented complication of percutaneous filter placement. Superficial epigastric vein cutdown is a reasonable alternative technique, which allows gentle atraumatic manipulation of the femoral vein. It is a simple, safe procedure that can be performed without any significant increase in operative time and no additional morbidity.

Effect of synthesis temperature and doping level on conductivity and structure of poly(3-methyl thiophene).

Poly(3-methyl thiophene) was synthesized by oxidative chemical polymerization technique using ferric chloride as the dopant in an inert atmosphere. Samples of different doping levels were prepared and analyzed by Fourier transform infrared (FTIR) spectroscopy, scanning electron microscopy, and direct current (DC) conductivity measurement at room temperature (300 K). Synthesis of the polymer was confirmed by FTIR studies. FTIR spectra showed a shift in the heterocyclic bands in the region of 700-1200 cm(-1) with a decrease in synthesis temperature. It was evident from the scanning electron micrographs that the surface structure of the polymer became denser with an increase in doping level. The measured DC conductivity increased initially up to the doping level of 0.8 M and then this increase tended to slow down. Samples having a doping level of 0.4 M were synthesized at 300, 280, and 270 K while maintaining the other synthesis parameters. The conductivity and yield were found to increase as the temperature of the polymerization decreased.

Direct current conductivity studies on poly(3-methyl thiophene).

The direct current (DC) conductivity of poly(3-methyl thiophene) was measured in the temperature range of 77-300 K. The observed DC conductivity data were analyzed in the light of Mott's variable range hopping model. Different Mott's parameters such as characteristic temperature (T0), average hopping distance (R), average hopping energy (W), and density of states at the Fermi level (N [E(F)]) were evaluated. By taking the inverse of the coefficient of exponential decay of the localized states involved in the hopping process as 0.5 nm, a realistic value of density of states at the Fermi level (N [E(F)]) was obtained that agrees well with the values reported earlier for other conjugated polymers.

Study of vibrational spectra of polyaniline doped with sulfuric acid and phosphoric acid.

Vibrational spectra of insulator emeraldine base (EB) form of polyaniline and electrical conductive sulfuric acid- and phosphoric acid-doped emeraldine salts (ES) were studied in the region of 4000-400 cm(-1) at ambient temperature by Fourier transform infrared spectroscopy. Infrared transmittance spectra of EB and ES were investigated to understand the bonding behavior of different organic and inorganic groups present in the polymeric chains and their structural variations on protonation by sulfate or phosphate ion inclusion in the polymer salt network. These studies revealed the para-coupling of deformed disubstituted benzenoid (B) and quinoid (Q) rings with ends capped predominantly by (B4Q1) units. The deformation of B and Q rings was confirmed by the appearance of many weak bands, very weak bands, and satellite structures in strong transmittance peaks of polymeric chain-constituting groups. Protonation takes place at the nitrogen sites of Q rings and forms semiquinone radical ions in ES. The vibrational bands pertaining to B rings, Q rings, B4Q1 units, semiquinone segment, sulfate ions, and phosphate ions were observed and assigned from these measurements. The shift in peak position of some bands with gain or loss in intensity and appearance of some new bands were observed in sulfuric acid- and phosphoric acid-doped ES spectra. These variations are attributed to the formation of new structural groups in ES on protonation and a change in crystalline field by sulfate and phosphate ion doping for crosslinking the polymeric chains.

Multiple emulsion-based systems carrying insulin: development and characterization.

An insulin delivery system based on liquid surfactant membranes has been developed. The formulation was based on a w/o/w emulsion where an organic membrane separated two aqueous phases and the internal aqueous phase contained insulin. Sesame and cotton seed oils were used as organic membranes. In order to facilitate the transportation of glucose across the organic membrane various additives such as calcium stearate, lecithin, cholesterol, hexamine, stearic acid and glyceryl tristearate were used. The additives were found to be successful carriers for the transportation of glucose to the internal aqueous phase. Similarly, viscosity enhancers, e.g. cetostearyl alcohol, in the organic phase enhanced the immobilization of insulin. Various parameters affecting the stability of the emulsions were established. The developed system was characterized for insulin activity and insulin efflux profile.

Liposomally encapsulated diclofenac for sonophoresis induced systemic delivery.

Liposomes containing diclofenac, an anti-inflammatory agent were incorporated into an ointment base for topical application. The drug loaded liposomes were characterized for various physico-chemical attributes and drug efflux profile in in vitro. The systemic availability of drug from liposomes following topical application was evaluated in rats. The effect of sonophoresis on the drug release profile in vitro and systemic availability in vivo was established. The application of liposomal diclofenac resulted in localization of the drug at the site of application with slow systemic availability; however, with the application of ultrasound pulsed drug systemic levels could be achieved.

Synchronous adenocarcinoma and MALT lymphoma of stomach.

We describe a patient in whom adenocarcinoma and lymphoma occurred simultaneously in the stomach. She presented with pain and lump in the epigastrium with history of hematemesis. Endoscopy revealed a growth involving the lesser curvature, and biopsy showed poorly differentiated carcinoma. Histological examination of the gastrectomy specimen showed synchronous diffuse adenocarcinoma with primary lymphoma of MALT type. The latter entity is known to be associated with Helicobacter pylori infection.

Evidence for a circadian oscillation in the gonadal response of the tropical weaver bird (Ploceus philippinus) to programmed photoperiod.

The effect of an asymmetrical skeleton photoperiod scheduleee was studied on the gonadal development in a tropical finch, the weaver bird (Ploceus philippinus). The schedul comprised a short nonstimulatory primary photoperiod of 6 hrs and a secondary much shorter lightperiod given as a 15-min light pulse at different times in the dark period. The light pulse 11 hrs after the basic period resulted in gonadal stimulation, while light pulse in contimuation with the basic period or 8, 14 or 21 hrs after the basicperiod was not stimulatory. The "photoinducible phase" was much more precisely outlined by shifting the birds from an 8-h to a 10-h pulse, and from an 11-h to 12-h pulse and was found to be very short, lasting about an hour falling between 11 and 12 hrs after the primary light period. The short photoinducible phase may be of an adaptive value since in the tropics the difference between the shortest and the longest daylength is also rather small (3 hrs and 15 min at Varanasi, 25degrees N). Clearly the weaver bird possesses a fine time-measuring device involving an endogenous circadian rhythm in photosensitivity. In nature, spermatogenesis in this bird also begins in March when the daylength exceeds 11 hrs (thus perhaps coinciding with the photoinducible phase). In rather small, photoperiod may not serve as a cue to trigger seasonal reproductive periodicity, it seems that photoperiod can act as a Zeitgeber for the initiation of spermatogenesis in the weaver bird at least.

Thyroid function in haemodialysed patients of Gassim, Saudi Arabia: TRH stimulation.

To study the hypophysical-pituitary-thyroid axis in Saudi patients on (RDT), 200 units of TRH were given intravenously predialysis to each of 21 such subjects. As expected (TSH) rose promptly within 30 minutes followed by a gentle decline at 60 minutes. A more gentle rise was also noted in T3 which continued for the 60 minutes of observation. Rather unexpectedly, T4 level was noted to show a reciprocal decline. To our knowledge, this pattern has not been defined in literature. In 12 patients (GH) was noted to rise promptly within 30 minutes followed by a gentle decline at 60 minutes. (LH) and (FSH) did not follow any definite pattern as observed for T3 and T4. Since the oral preparation of TRH is now available, can (TRH) be effectively used to treat renal dwarfism?

Hodgkin's disease: a retrospective analysis of 15 years experience at a large referral centre.

BACKGROUND: The prognosis of patients with Hodgkin's disease has improved significantly over the past two decades, primarily as a result of the development of curative combination chemotherapy. However, data on the presentation and results of treatment in Indian patients are scarce. We reviewed the clinical and pathological data and the treatment outcome of patients with Hodgkin's disease seen at our centre over the last 15 years. METHODS: The case records of 289 patients diagnosed to have Hodgkin's disease at our centre between 1977 and 1992 were reviewed. Their clinical characteristics, treatment outcome and prognostic factors for survival were analysed. RESULTS: The median age of the patients was 26 years with 30% being in the third decade. The male:female ratio was 4:1. The mixed cellularity histological subtype was the most common (68.5%), followed by nodular sclerosis (8.9%). 'B' symptoms were present in 73% of patients and 9.4% had bone marrow involvement. At the time of presentation, 70% of patients had advanced disease (stage III: 36%, stage IV: 34%). Complete response to treatment was obtained in 65.7% of patients while 17.3% had partial response. The predictors of decreased overall survival were age > 40 years, presence of 'B' symptoms, lymphocyte depletion histological subtype and stage IV disease. Disease-free survival was influenced by the presence of 'B' symptoms, bone marrow involvement and stage IV disease. CONCLUSION: Hodgkin's disease in Indian patients has a distinct clinico-pathological profile. Combination chemotherapy has high response rates even in patients with advanced disease at presentation.

Maternal mortality in Benghazi: a clinicoepidemiological study.

We conducted a clinicoepidemiological study of 14 maternal deaths out of 79,981 live births at Al-Jamahiriya Hospital, Benghazi between 1993 and 1997. The maternal mortality rate per 100,000 live births was 17.5. The reproductive profile of these women was: mean age 31.5 +/- 6.9 years, mean parity 4.5, mean birth interval 14.6 +/- 7.0 months, mean gestation 27.7 +/- 14.6 weeks and mean haemoglobin 9.3 +/- 2.1 g/dL. None of the women had prebooked their delivery, 50% had preconceptional medical or obstetric risk factors, around 70% were anaemic, almost all were admitted with serious medical conditions and > 50% required surgical intervention. The main underlying medical causes of death were: hypertensive disease of pregnancy (28.6%), haemorrhage (14.3%), pulmonary embolism (14.3%) and brain tumour (14.3%).

Hypothalamic stimulation of prolactin release in a freshwater catfish, Heteropneustes fossilis.

Hypothalamic extract has been found to contain certain factor(s) which triggers the release of prolactin from the pituitary gland. In assay recipients, hypothalamic extract injection was as effective as perphenazine-HCl treatment in triggering the release of prolactin from pituitary gland. Rise in prolactin concentration of serum and resultant decrease in pituitary prolactin level in response to hypothalamic extract administration suggests that hypothalamus in H. fossilis has prolactin releasing factor(s). This releasing effect of hypothalamus seems to be specific since cortical extract injections were ineffective in producing the similar response.

Clinical performance of intrauterine device TCu-380 A in Benghazi, Libyan Arab Jamahiriya.

The study examined the reproductive and health profile of Benghazi women using intrauterine contraceptive devices (IUDs) and evaluated one such device, the TCu-380 A. An historical longitudinal study was carried out using data from the Fertility Regulation Clinic, Keish Polyclinic, Benghazi. The subjects were 457 women registered for TCu-380 A insertion between 1995 and 1998, who had been under follow-up for at least 6 months. The majority were Libyan (87.8%), aged 20-29 years (63.4%), of parity 1-6 (67.1%), non-lactating (64.3%) and with normal delivery at last conception (95.1%). Half had a chronic disease. The cumulative 36-month follow-up of those using TCu-380 A revealed an effectiveness rate of 99.8%, a continuation rate of 96.1% and complications in 3.5%. TCu-380 A appears to be an effective, durable and safe IUD.

Cancer mortality in Benghazi, Libyan Arab Jamahiriya, 1991-96.

We present a descriptive study of 1221 cancer deaths among Libyans in Benghazi for the period 1991-96. The cancer mortality rates per 10(5) person-years at risk for males, females and both sexes were 39.8, 26.5 and 33.3 respectively. The age-standardized cancer death rate per 10(5) standard world population was 91.5, 60.0 and 76.5 respectively. The 10 most common cancer deaths by site (comprising 67.7% of the total), in descending order of frequency, were: trachea, bronchus and lung, blood (leukaemia), colon/rectum, other lymphatic and haemo-poietic tissue (lymphomas), stomach, breast, prostate, liver, bladder, and larynx. The results point to the necessity for conducting comprehensive prospective studies, initiating a cancer registry and establishing a national cancer control programme.
