Temporal gait monitoring: a new device.

A new device that provides continuous recording of gait and gait events is presented. It can be used with motor point electromyography to perform walking electromyography, an electrogoniometer to continuously record joint position, or other measuring devices to record events or activities related to phase of gait. The major advantages of this system as compared with available systems are ease of use, decreased patient encumbrance, potential for portability, potential for digitizing the signal for minicomputer compatibility, and greatly reduced expense.

Adult-onset hemiplegia: changes in gait after muscle-balancing procedures to correct the equinus deformity.

Fifty-four adult patients with acquired spastic equinus and equinovarus deformity were treated with lengthening of the Achilles tendon, lateral transfer of the anterior tibial tendon, and appropriate muscle releases. All patients had preoperative dynamic electromyography and electrogoniometry performed in order to assist in planning the surgical procedures and to provide a baseline assessment of the dynamic deformities. Preoperatively, the stance and double-support phases of gait were prolonged. Throughout the stance phase, the gait of these patients was characterized by equinus deformity of the ankle, decreased flexion of the knee (hyperextension in the most severely involved patients), and increased flexion of the hip (which also varied with the severity of the equinus deformity of the ankle and hyperextension of the knee). In all patients, the operation was performed at least one year after onset of the hemiplegia. Clinical follow-up at an average of thirty months (range, twenty-four to sixty-two months) showed that the equinus deformity was corrected in all patients and that 59 per cent of them were brace-free. Two patients had a superficial infection that healed uneventfully, and two had pull-out of the tendon that required reoperation. Postoperative analyses of gait, performed at least one year after surgery for twenty-seven of the patients, showed that the stance and double-support phases of gait (which had been prolonged before surgery) approached the findings in normal control subjects.(ABSTRACT TRUNCATED AT 250 WORDS)

Bactericidal effects of photoradiation therapy with hematoporphyrin derivative.

Hematoporphyrin derivative (Hpd) localizes selectively in malignant and rapidly metabolizing tissues and undergoes a cytotoxic reaction when exposed to light of a specific wavelength. Hpd has been studied extensively with regard to the diagnosis and treatment of tumors but not with regard to bactericidal activity. This investigation assessed the effect of light-activated Hpd on various microorganisms, on human polymorphonuclear leukocytes, and on the interactions of polymorphonuclear leukocytes and bacteria. Light-activated Hpd was 99.9% bactericidal against Staphylococcus aureus, Streptococcus faecalis, Bacteroides fragilis, Streptococcus M-G intermedius, Streptococcus mutans, Peptostreptococcus anaerobius, Peptococcus magnus, and Clostridium perfringens, but did not affect Escherichia coli, Pseudomonas aeruginosa, or Candida albicans. Photoactivation of Hpd rendered 98% of polymorphonuclear leukocytes inviable. Combinations of light-activated Hpd and polymorphonuclear leukocytes had a 4-log (99.99%) bactericidal effect on both intra- and extracellular S. aureus. The ability of Hpd to localize in inflammatory tissues may have therapeutic applications in the treatment of abscesses.

Modified moberg opponensplasty in acquired spastic hemiplegia.

Loss of the transverse arch of the hand and decreased thumb opposition has heretofore been an unreported deformity in the adult acquired spastic hemiplegic upper extremity. Based on Moberg's experience in quadriplegic patients, 11 patients underwent release of the annular ligament at the base of the thumb. This procedure allows the flexor pollicis longus tendon to bowstring and substitute for thumb opposition. All 11 had increased prehension capacity following the surgery. In carefully selected adult acquired spastic hemiplegic patients, release of the annular ligament at the base of the thumb can functionally restore the transverse arch of the hand, improve thumb opposition and improve prehensile capacity.

Qualitative and quantitative gait phase analysis by continuous monitoring of inter-ankle distance.

A new device is presented that is able to provide a temporal recording of gait as well as a continuous qualitative monitoring of gait events. It can be used with motor-point electromyography to perform walking electromyography, with an electrogoniometer to record joint position, or with other measuring devices to record events relative to phase of gait. The major advantages of this system as compared with available systems are ease of use, decreased patient encumbrance, potential for portability, greatly reduced expense, and the potential for digitizing the signal for computer compatibility. Preliminary data are comparable to available accepted norms.

Hematoporphyrin-derivative and photoradiation therapy of malignant tumors.

Kodak projectors with #2418 red Corning filters were used as a light source to treat cutaneous and subcutaneous malignancies in five patients who previously had been given hematoporphyrin derivative (HpD). An argon dye laser system was used to treat malignancies in patients who also were given the HpD. These tumors included 11 melanomas of the eye, three carcinomas of the esophagus, one melanoma of the esophagus, four carcinomas of the lung, three basal cell skin cancers, and one retropharyngeal metastatic oral cancer. Clinical results and technical problems of this therapy are discussed.

Management of childhood "hyperactivity" by primary care physicians.

A questionnaire assessing current clinical approach to the problem of childhood hyperactivity was mailed to 910 primary care physicians in the state of Washington. A response of 462 (50.8%) was obtained. Pediatricians assess and manage hyperactivity in a manner significantly different from that of family physicians or general practitioners. Age of physician also accounted for significant differences, although to a lesser degree than type of training. Few differences were determined by size of community. An overall high prevalence of the problem of hyperactivity was apparent. Combined use of stimulant medications, behavioral programs, and special diets was common.

High pressure liquid chromatographic determination of mono- and disaccharides in presweetened cereals: Collaborative study.

A collaborative study was conducted using a modified AOAC method (sugars in chocolate) for the determination of fructose, glucose, sucrose, and maltose in presweetened cereals by high pressure liquid chromatography (HPLC). Eight samples consisting of 6 products were analyzed in duplicate by the HPLC method and the AOAC Lane-Eynon method. The AOAC method was modified to use water-alcohol (1 + 1) and Sep-Pak C18 cartridges for sample cleanup. The HPLC results indicate precision comparable to the lane-Eynon method and the chocolate method. The modified HPLC method has been adopted official first action.

Thiobarbituric acid reactive material in uremic blood.

The thiobarbituric acid (TBA) assay was applied to uremic blood. Significant TBA-reactive material was found in patients with chronic renal failure compared to normal controls. The presence of TBA-reactive material correlated with the degree of renal failure, and was removed by hemodialysis. The reactive material is not 2-deoxyribose or sialic acid. In addition, the presence of TBA-reactive material in uremic blood is not abolished by prior aspirin ingestion, suggesting it is not related to active prostaglandin synthesis. The possible origin of the TBA-reactive material in uremic blood is discussed.
