The hemostatic effect of conjugated water-soluble estrogens in rats.

Estrogenic steroids have been used as hemostatic agents in various surgical procedures in humans. Their use to reduce bleeding has been based mainly on clinical observation. The present study was carried out to test experimentally whether intravenously administered Premarin reduces capillary bleeding. Skin-graft donor sites in rats served as the standard capillary bleeding source in this controlled double-blind experiment, in which Premarin was found to appreciably reduce capillary bleeding.

Urethral stricture following indwelling catheter in hypospadias repair.

We report 2 patients who were operated on for hypospadias in whom severe dysuria developed several weeks after the removal of a Foley catheter, which had been in place for two and eight days, respectively. Distal urethral narrowing was diagnosed by endoscopy and by micturating cystourethrography. Since we believe the urethral stricture was caused by the indwelling catheter, we now use cystotomy for urinary diversion in all patients operated on for hypospadias.

A model system for studying metastasis using the embryonic chick.

An assay capable of recovering individual viable rodent cells localized in various organs of the chick embryo is described. This assay is based on the differential sensitivity of chick and rodent cells to the cytotoxic drug ouabain. Utilizing this assay, the potential of the chick embryo as a model system for studying metastasis was examined. Several cell lines were characterized in three ways: (a) ability to form local tumors after cell application onto the chorioallantoic membrane; (b) ability to form macro- or microscopic metastasis in the embryo from chorioallantoic membrane tumors; (c) experimental metastatic ability following i.v. injection into chorioallantoic membrane veins. These results were compared with the results obtained from the ouabain-plating assay. We conclude that this assay permits detection of viable metastatic cells even when tumors cannot be detected and helps to overcome the time constraints that have, in the past, limited the usefulness of the chick embryo in modeling metastasis.

The value of angiography prior to use of the latissimus dorsi myocutaneous flap.

In 5 patients studied, preoperative angiography showed the thoracodorsal artery to be patent in two--both of them then had successful transfers of latissimus dorsi myocutaneous flaps. Obliteration of this artery was apparent in 3 patients, precluding the operation. We suggest that selective preoperative angiography be performed in all patients in whom a latissimus dorsi myocutaneous flap transfer is being considered, as a patent thoracodorsal artery is essential to the success of this procedure.

Genetic studies in a family with inverted nipples (mammillae invertita).

A Jewish Sephardi family is reported in which 16 members are affected (15 females and 1 male) with inverted nipples. The one affected male and his brother also have gynecomastia. Under the assumption that this trait is transmitted as an autosomal dominant, linkage studies were done but were not revealing. Further family and investigative studies are needed in this disorder to understand better its pathogenesis and precise mode of genetic transmission.

Use of the carbon dioxide laser in an abdominoperineal resection for epidermoid anal carcinoma: a case report.

An abdominoperineal excision of the rectum was performed on a 52-year-old male who had epidermoid anal carcinoma. A steel scalpel was utilized for the abdominal component, but the entire perineal resection was accomplished with a carbon dioxide laser beam. We believe this to be the first reported case of an abdominoperineal resection using this new modality, which may become a useful and modern adjunct in extirpative surgery. The haemostatic effects of the laser were well demonstrated in our patient.

"Pseudomelanoma" in a keloid.

A pigmented lesion resembling a superficial spreading malignant melanoma appeared in keloidal scarring resulting from partial excision of three closely set intradermal and compound nevi. Complete excision of nevi when feasible is recommended as the wisest course to obviate future uncertainty.
