[Tuberculous mastitis--a rare differential diagnosis of non-puerperal mastitis].

Tuberculosis in a case of non-puerperal mastitis is a rare clinical finding. A special case of a 28-year old Thai patient was diagnosed and successfully treated with an antituberculous chemotherapy. The most important diagnostic procedure was the microbiological and histological investigation to enable the diagnosis to be made. Surgical therapy was not necessary.

[Genital tuberculosis in the woman--only of interest in medical history?].

Genital tuberculosis still occurs and is difficult to diagnose. The symptoms are mostly uncharacteristic, whereas infertility and chronic pelvic infections are frequently diagnosed. In the period between 1970 to 1990, we found 21 women with this diagnosis. The occurrence seems to have shifted to greater age and milder clinical disease and are mostly observed within the framework of the sterility complex. We conclude, that primary treatment should be conservative. Intrauterine pregnancies after treatment are very rare, but, in one case, a normal pregnancy and delivery was occurred.

[Risk assessment in twin pregnancy].

248 twin pregnancies had been examined retrospectively between 1980 and 1988. Each case of twin pregnancy was confronted with a singleton pregnancy of similar age and parity as matched-pair. Aim of the study was to examine the course of delivery as well as fetal outcome. Main reasons of prematurity have been preterm uterine contractions and rupture of fetal membranes. Rates of cesarean sections had been 41.2 per cent in twin pregnancy and 18.8 per cent in singleton ones. Apgar score had been better in singletons. Ph of umbilical artery had similar values in singleton pregnancy and first twin, but lower values in the second twin. Also nowadays twin pregnancy is a risk pregnancy with a 2.3 fold higher perinatal mortality compared with singleton pregnancy in our matched-pair-group.

[Ultrasonic diagnosis of fetal triploidy].

It can be assumed that among the most frequent pointers to triploidy are the sonographic proof of the existence of a voluminous and vacuolic placenta, early retardation of growth, and the presence of an oligohydramnios. Occasionally, other malformations are also observed, such as hydrocephalus or neural tube defects. Sonographic findings are demonstrated in two cases of triploidy found to exist before the 24 weeks of pregnancy in the living foetus. The clinical signs and patterns are discussed.

[The course of pregnancy following cerclage. 1. Singleton pregnancies].

In a retrospective study about value of cerclage 160 patients with singleton pregnancies had been studied in two subgroups dependent from the moment of operative cervix closure and opposed a matched pair control group without cerclage. In both subgroups there is a more loaded obstetric history with reference to previous abortions. Preterm labour and premature rupture of membranes and tocolysis could be observed significantly more frequent in both subgroups than in the controls. Obstetric results are worse in the cerclage group with respect to birth weight, height and pregnancy duration, especially correlations between prognosis and opening of the womb at the moment of cerclage could be proved. Amniotic infection syndrome was three times more frequent in the cerclage group than in the controls. Contrary to the literature no increased frequency of cervical laceration or caesarean sections could be observed. In conclusion our results are contrary to the optimistic view on cerclage, important not only for indication, but also for instruction of patients before therapy.

[Pregnancy outcome following cerclage. 2. Multiple pregnancy].

In a retrospective study pregnancy and labour after cerclage had been studied in comparison to 160 patients with singleton pregnancies and a matched pair control group of multiple pregnancies without operative cervix closure. Cervical incompetence in multiple pregnancy observed more frequently than in singleton ones is not correlated to obstetric history, but has a bad prognosis because of necessity of tocolysis during pregnancy, marked shortening of duration of pregnancy, lowering of length and weight of newborns. An increase in frequency of amniotic infections had to be calculated following cerclage in multiple pregnancies, too. Our results especially in multiple pregnancies are not so optimistic. This statement is valid also for the indication to its prophylactic use.

[Primary aldosteronism in pregnancy].

Primary hyperaldosteronism (Conn's syndrome) rarely occurs during pregnancy. The concurrence of hypertension combined with hypokalemia and revealing subjective symptoms such as paresthesia, muscular weakness and lassitude can suggest this infrequent diagnosis. The diagnosis is confirmed on the one hand by chemical tests demonstrating an aldosterone level far above normal and clearly suppressed renin activity, and on the other hand through ultrasound for a morphological diagnosis. We consider causal surgical treatment with adrenalectomy as the therapy of choice during early pregnancy.

[Results of testing tumor resistance (in vitro--in vivo) in breast cancer].

In-vitro sensitivity to cytostatics was tested in 281 carcinomas of the breast using the test method introduced by Volm. The Adriamycin 3H-uridine test system was chosen for the analysis. Altogether, 84.7% of the tumors were testable. A correlation with the subsequent in-vivo result was found in 219 of the breast cancer patients. Only 8% of the breast cancers were unequivocally sensitive to the test (uridine incorporation rate less than 55% for verification). The mean uridine incorporation rate was 577 CPM. Neither tumor size, lymph node status, nor the estrogen and progesterone receptor content of the tumor cells influenced the test result. With regard to the breast carcinoma, no relationship was found between the predictive information and the subsequent in-vivo response to chemotherapy. This applied to both short-term observation periods of three to six months as well as longer-term follow-ups. Furthermore, no connection was found between sensitivity in vitro and probability of recurrence (p = 0.69) under cytostasis. Probability of survival was higher among patients with tumors that were not sensitive to the test than among those with tumors which were (p less than or equal to 0.001). In the authors' experience the Volm chemosensitivity test only appears to furnish prognostic information about the breast carcinoma; in the light of their data the test appears rather unsuitable as an aid to deciding whether or not to institute cytostatic therapy.

[Thrombocytopenia following intracervical prostaglandin priming].

Thrombocyte aggregation with normochromic anaemia occurred in a gravida III of 27 years of age with sonographically confirmed foetal hydrocephalus, after prostaglandin E2 cervical priming. The authors discuss the differential diagnostic possibilities, but the actual genesis of the changes remains unclear. Hence, it is recommended to check with particular care especially in induction of abortion, the coagulation system with the thrombocytes, over and above the well-known prostaglandin side effects. Special attention must be paid to the occurrence of allergic reactions, and it must always be borne in mind that induction of abortion after the 14th week of gestation is a risky matter (1).

The influence of autonomous nervous activity on total and regional gastric blood flow.

Using 8 mu radioactive microspheres total and regional gastric blood flow were measured in dogs under electrical splanchnic stimulation (SS), following splanchnicectomy (SE), under electrical stimulation of the vagal truncs (VS) and following truncal vagotomy (VE). The values were compared to controls and were taken while cardiac output (CO) and mean aortic pressure ( AOBP ) were registered. Total GBF was 16 +/- 4 ml/100g/min. in 8 animals. Regional flows were highest in the mucosa of corpus and fundus (35 +/- 7 and 27 +/- 6 ml/100g/min.) and lowest in the seromuscularis (4 +/- 1 ml/100g/min.) SS decreased total GBF to 7 +/- 2 ml/100g/min. and this was due to a most dramatic fall in corpus and fundus mucosal flow by 75%. In contrast SE increased total GBF to 30 +/- 11 and regional corpus mucosal flow achieved 85 +/- 24 ml/100g/min. Blood flow changes under VS were similar to those following SE but were less elevated. In the same manner flow reduction following VE was similar to the changes seen after SS but in average less profound. CO and AOBP increased during SS (less than 30% of control) but were almost the same for SE, VS and VE.

How to achieve total gastric sympathectomy.

Experimental examinations on acute, stress induced gastric ulcers have shown the outstanding role of the sympathetic innervation in the pathogenesis of this ischemic disease. As reduction of sympathetic tonus resulted in a significant prophylaxis against "stress ulcers", further experimental work was done to achieve total but selective gastric sympathectomy. In a histomorphological study in dogs (Method: Hillarp and Falck in the modification of Heene ) the effects of three different surgical procedures on gastric sympathetic denervation were investigated: 1. Selective proximal vagotomy (= SPV, 8 dogs) 2. SPV and periarterial denervation of the main gastric arteries with severance of the gastrolineal ligament (= PASD , 8 dogs) 3. PASD alone (7 dogs) Selective proximal vagotomy resulted in a sympathetic denervation of major parts of the right site of the fundus and corpus. Following selective proximal vagotomy with periarterial denervation ( PASD ) there was a complete sympathetic denervation of the entire stomach. Only a minimal area of resting sympathetic innervation was observed on the esophago-cardial border when periarterial denervation ( PASD ) was performed alone.

[Sympathectomy of the stomach: an effective preventive measure in stress-induced gastric mucosa lesions in swine].

15 mini-pigs were bled to a mean aortic blood-pressure of 40 mm Hg, which was sustained for 3 hours. 2 groups were studied: Control-Group: Shock alone without splanchnicectomy (n = 8); Test-Group: Shock after splanchnicectomy 14 days previously (n = 7). In all animals a stimulated gastric secretion test was performed 3 days before and 8 days after this operation. All animals of the control-group showed severe gastric mucosal lesions after shock. Piglets with splanchnicectomy (test-group), conversely, developed no (5 animals) or only minor changes (2 animals). The efficiency of splanchnicectomy was proved by a stimulated gastric secretion test: Basal acid output did not change after operation, peak acid output however increased significantly. This study in piglets shows, that splanchnicectomy of the stomach prevents gastric stress-lesions almost completely.
