Deep ocular socket reconstruction.

A technique to reconstruct totally contracted sockets forms spacious, deep ocular fornices to accommodate ocular prostheses. Fixation of the midperiphery of mucous-membrane-lined, custommade conformers to the superior and inferior orbital rims secures the posterior periphery of the conformer deep in the orbit. The method restores the normal anatomy of the ocular fornices that not only extends to the orbital rims but also penetrates deeply into the orbit along its roof and floor. The procedure contrasts with conventional, frequently unsuccessful methods of socket reconstruction that form fornices only to the superior and inferior orbital rims without extending posteriorly into the orbit. Using this technique, we reconstructed deep, spacious sockets in five patients with serious socket contracture, allowing the patients to retain cosmetically acceptable artificial eyes. The principles advocated apply to all contracted sockets.

Histological and ultrastructural changes in rat kidney following cadmium injection.

Cadmium injection in rats affects the genito-urinary system. The renal effects are generally considered to be mainly tubular. Light microscopy of 29 specimens confirms cellular damage in the glomeruli with changes in glomerular capillaries. Electron microscopic changes in the glomerulari confirm changes in the podocytes, thickening of the basement lamina, the occurrence of electron dense deposits in association with the capillary endothelium with platelet and polymorphonuclear cell accumulations which suggests opening of the endothelial junction of the glomerular vessels.

A comparison of ipratropium bromide, deptropine citrate and placebo in asthma and chronic bronchitis.

A new anticholinergic aerosol, ipratropium bromide, was compared in a double-blind cross-over trial with an established preparation, deptropine citrate, and placebo in 16 patients with defined asthma or chronic bronchitis. Ipratropium bromide produced a significantly greater and more rapid bronchodilation than did deptropine citrate or placebo in the doses used, and its effect was slightly greater in the asthmatic than in the chronic bronchitic group. No unwanted effects on secretion were seen with ipratropium bromide.

The prevalence of calcified upper urinary tract stone disease in a random population--Cumbernauld Health Survey.

It is difficult to obtain the true prevalence of stone disease in community. A proper random sample of a population has been studied and a figure of 3.83% of calcified stones have been found in 2,000 subjects. The significance of biochemical, bacteriological, skeletal and other surgical abnormalities is discussed. It is now possible to study individual groups within the population with respect to stone disease.

Carcinoma of the penis: a review of 42 cases.

A retrospective survey was made of 42 cases of carcinoma of the penis presenting over 14 years. Cases for attempted cure of the primary lesion were treated by either radiotherapy or surgery. It is recommended that radiotherapy should be the primary treatment as it is non-mutilative, allows secondary surgical salvage where necessary and has no greater occurrence of post-treatment lymph node metastases than surgery.

Human antibody-dependent cell-mediated cytotoxicity against target cells infected with respiratory syncytial virus.

A chromium release assay was established to study human antibody-dependent cell-mediated cytotoxicity (ADCC) of HEp 2 cells infected with respiratory syncytial (RS) virus. Human peripheral blood lymphocytes in the presence of specific antibody to RS virus caused in vitro lysis of RS virus infected target cells. ADCC was detected in sera of infants with RS virus infections and in specimens of colostrum. The ability of serum or colostrum to mediate the cytotoxic reaction appeared to be related to the level of specific IgG, or IgA antibody to RS virus, as detected by membrane fluorescence. Separation of effector cells by their glass adherence properties showed that the ability to produce cytotoxicity resided in non-adherent effector cells.

The incidence of deep vein thrombosis in prostatectomised patients following the administration of the fibrinolytic inhibitor, aminocaproic acid (EACA).

Forty patients undergoing prostatectomy for benign prostatic hypertrophy were included in a double blind trial of epsilon aminocaproic acid, and the incidence of postoperative deep vein thrombosis determined, using 125I-fibrinogen technique. There was no significant difference between the groups, the overall incidence of abnormal scans being 50 per cent, but of the patients undergoing enucleative prostatectomy 68 per cent developed significant scan findings compared with 33 per cent following transurethral surgery.

Clinical and biochemical abnormalities in coppersmiths exposed to cadmium.

5(18.5%) of a group 27 coppersmiths exposed to cadmium fume had stone disease. When compared with a control group of assembly workers in the same factory they had evidence of renal impairment as shown by blood biochemistry and proteinuria. A greater tendency to liver damage was found in the coppersmiths. There was evidence that restrictive airways disease was more common in the coppersmiths than in the control group. Blood-cadmium concentrations were significantly higher in the coppersmiths and in the assembly workers than in a reference population.

Breast-feeding protects against respiratory syncytial virus infections.

Eight out of 115 infants admitted to hospital with respiratory syncytial (RS) virus infection had been breast-fed compared with 46 out of 167 controls; this difference was statistically significant. Twenty-one specimens of human colostrum were examined, and all contained RS virus neutralising activity. Specific IgA and IgG were detected in 18 specimens, whereas IgM was detected in none. The titre of IgA antibody was usually higher and correlated more closely to the titre of neutralising activity than that of IgG. Infants inhale milk feeds and regurgitate them through the nose, and the IgA collecting in the respiratory tract might protect against severe respiratory infection. Alternatively, if severe RS virus illness is a sign of hypersensitivity to the virus breast-feeding might protect the infant from an early sensitising infection.

Radiotherapy in the management of stage C carcinoma of the prostate.

A small number of patients with limited stage C carcinoma of the prostate have been treated with combined interstitial and external beam radiotherapy with gratifying results. Whereas more patients will have to be followed for at least 10 to 15 years to completely evaluate this modality of treatment, the results continue to encourage us in its use.

The results of 100 small tissue biopsies of testis in male infertile patients.

The value of testicular biopsy in male infertility has recently been emphasized by Meinhard, McRae and Chisholm (1973), and the present authors agree with them that a biopsy is essential for the following reasons: (1) to establish a firm diagnosis; (2) to rationalize therapy on the basis of histological findings; (3) new developments in drug therapy and electronmicroscopic techniques will help to clarify many areas of doubt and uncertainty in this difficult field; (4) the diagnosis of 'sloughing' by itself may mask changes in germinal cell development which may be amenable to hormone therapy.

Detection of antibody to respiratory syncytial virus by membrane fluorescence.

An indirect membrane fluorescent antibody technique was established to study HEp 2 cells infected with respiratory syncytial (RS) virus. It was possible to detect IgG and IgM antibody to RS virus in the sera of patients with respiratory infections using this technique. The technique was further applied to the detection of IgA antibody to the same virus in colostrum.
