Splenopentin-induced reconstitution of the immune response after total body irradiation: optimization of treatment regime.

In former investigations our group obtained an immuno-reconstituting effect in mice treated by immunosuppression after a continuous treatment with splenopentin (SP5). The goal of the present study was to test whether a small number of injections right after immunosuppression possibly produces the same result in the sense of an initial effect. Although a short-term therapy induced an increase of antibody formation which was measurable after 4 weeks, the study clearly showed that only a continuous treatment by SP5 led to an optimum, i.e. complete reconstitution of antibody formation.

Molecular characterization of an ependymin precursor from goldfish brain.

Ependymins are thought to be implicated in fundamental processes involved in plasticity of the goldfish CNS. Gas-phase sequencing of purified ependymins beta and gamma revealed that they share the same N-terminal sequence. Each sequence displays microheterogeneities at several positions. Based on the protein sequences obtained, we constructed synthetic oligonucleotides and used them as hybridization probes for screening cDNA libraries of goldfish brain. In this article we describe the full-length sequence of a mRNA encoding a precursor of ependymins. A cleavable signal sequence characteristic of secretory proteins is located at the N-terminal end, followed directly by the ependymin sequence. Also, two potential N-glycosylation sites were detected. A computer search revealed that ependymins form a novel family of unique proteins.

Flunarizine i.v. in the acute treatment of common or classical migraine attacks--a placebo-controlled double blind trial.

The efficacy and tolerance of 20 mg flunarizine i.v. were tested in comparison with placebo in a multicentre randomised double-blind trial in the acute treatment of migraine attacks. Sixty case reports were included in the evaluation; 31 patients were treated with flunarizine and 29 with placebo. Flunarizine proved to be significantly superior in its effect on the intensity of pain and the typical concomitant symptoms of the attacks. Patients were classed as responders who displayed a reduction in pain intensity by at least 50% within 60 minutes after the administration of flunarizine. 23 patients (= 74.2%) were responders, including 11 patients being without pain after 60 minutes. In the placebo group the responder rate was 27.6% The fact that both groups were comparable in all respects should be emphasized. The tolerance of intravenously administered flunarizine was excellent and corresponded to that of placebo. Apart from a sedative effect reported by 9 patients there were no side-effects. The circulatory conditions remained largely stable. The result of this study seems to indicate that an intravenous injection of 20 mg flunarizine might represent a genuine alternative, and as regards tolerance even a superior one, to the parenteral administration of ergotamine in migraine attacks.

Nonpalpable breast tumors: diagnosis with stereotaxic localization and fine-needle aspiration.

Modern mammography is the most effective means of detecting nonpalpable breast cancers, but correct diagnosis for malignancy is made in only 20%-30% of the cases. The conventional method of lesion localization usually results in approximate placement of the hookwire in the breast. The authors report the results of stereotaxic localization, combined with fine-needle aspiration and cytologic study, performed in 528 cases. Clinically occult breast lesions were localized precisely (within 2 mm 96% of the time), sampled by means of a 23-gauge needle, and marked with either methylene blue or a hookwire for subsequent open excisional biopsy. The results indicate a sensitivity of 95%, specificity of 91%, and accuracy of 92% for the fine-needle aspiration procedure. This technique offers a significantly improved preoperative method of diagnosing small breast lesions with minimal pain, no complications, reduced cost, and no disfigurement or scar interfering with subsequent mammographic follow-up.

Prevalence of Erythema migrans Borreliosis in blood donors.

European Erythema migrans Borreliosis and North American Lyme disease are closely related to syphilis. This implicates a potential risk of infection for blood recipients. Eighty-six of 3,157 blood donors tested showed IgG-antibodies against Borrelia Burgdorferi. From among 47 persons of this group who could be examined, clinical signs of diseased skin, joints or nervous system, not diagnosed before, were found or could be suspected in 13 cases. Since intrauterine transmission of Borrelia infection has been described, the inevitable question of whether this disease can also be transmitted as a result of blood transfusion becomes a major concern. As the pathogen can persist even in the presence of serum antibodies, it seems advisable to examine blood donors serologically, whenever Erythema migrans Borreliosis is suspected. Though further research is required to document a transfusion-transmitted Borrelia infection, infected persons should be treated to avoid serious or late manifestations.

[Laser micro-spectroanalytical determinations of aluminum in lumbar vertebrae of non-nephrectomized and 5/6 nephrectomized rats after application of dose- and time-dependent aluminum chloride solutions].

In order to obtain further informations on the mode of action of Al, we carried out lasermicroanalytical determinations (LMA) of Aluminium (Al) on different parts of lumbar vertebrae in 54 non-nephrectomized rats and 54 ones 5/6 nephrectomized which were divided in 3 groups. The animals were treated with solutions of physiological NaCl in the control group (0.25 ml/100 g b.w.) and different Al-doses (0.05 and 0.5 mg/kg b.w.) intraperitoneally and daily for 4, 8, and 12 weeks. The results were compared with those of histochemical Al-detections by the method of IRWIN (1955). Lumbar vertebrae of the non-nephrectomized rats in the control group did not contain Al by LMA- and histochemical determination investigated in all the time intervals. Al-doses of 0.05 mg/100 g b.w. induced only a positive Al-proof 12 weeks after the beginning of the injections in the 5/6 nephrectomized animals by LMA, whereas Al-doses 10 times higher effected an Al-deposit of different concentration in the substance compact and substance spongioid of the lumbar body of vertebrae from the 4th week in the 5/6 nephrectomized and the 8th in the non-nephrectomized rats. The sensitiveness of the Al-detection by LMA was higher in comparison with the histochemical procedure, and an Al-accumulation in the lumbar vertebrae of the 5/6 nephrectomized rats is distinctly earlier than in those of non-nephrectomized animals.

[Current developments in the radiologic diagnosis of the adrenal glands].

Cross-sectional imaging techniques have dramatically improved the diagnosis of adrenal disease. In most patients with endocrine-active adrenal disease, CT is the only imaging test needed to establish the correct diagnosis. Adrenal venography with blood sampling may provide important additional information in patients with Conn adenoma. Magnetic resonance imaging and (IMBG) scintigraphy appear to be the best imaging tests for the localization of multiple or extra-adrenal pheochromocytomas. Inactive adrenal tumors detected incidentally are a problem as for as diagnosis is concerned, since inactive adrenal adenomas have to be differentiated from carcinomas and metastases. MRI is rarely helpful in these cases. For patients with a known primary tumor, the authors recommend CT-guided biopsy. In all other cases a follow-up study often reveals that the adrenal tumor detected is benign.

[Heart surgery in terminal kidney failure and dialysis dependent patients. Intraoperative hemofiltration for the prevention of hyperhydration].

12 hemodialysis-dependent patients underwent heart surgery involving cardiopulmonary bypass. Their example outlines the special pre-, intra- and postoperative management in these patients. Intraoperative hemofiltration seems appropriate to prevent fluid overload caused by cardiopulmonary bypass. 3 patients died within the first two weeks after operation. 9 patients are well after 4 to 30 months. The results are compared to the reports found in the literature.

Topographic EEG brain mapping in cerebrovascular disease and dementia.

1. The results of brain mapping had been studied in 27 patients with different stages of ischemic stroke and in 20 patients with dementia, ten of SDAT and ten of vascular type. 2. It has been shown that in ischemic stroke the mapping correlates better with the severity of clinical symptoms than with anatomical and hemodynamic imaging methods. For the differentiation of SDAT and VD the mapping showed a clear cut possibility of differentiation.

Cloned gene encoding the delta subunit of Bacillus subtilis RNA polymerase.

Core RNA polymerase and several forms of RNA polymerase holoenzyme from Bacillus subtilis are found in association with a 21,500-kDa polypeptide called delta (delta). We have cloned the structural gene (rpoE) for delta by using a hybridization probe a synthetic oligodeoxynucleotide that was designed on the basis of a partial NH2-terminal amino acid (aa) sequence of purified delta protein. The rpoE gene was found to encode a 173-aa polypeptide of a predicted Mr of 20,400. Genetic and physical mapping experiments placed rpoE at 325 degrees on the B. subtilis chromosome and established the gene order rpoE-ctrA-spo0F. The delta subunit is known to enhance the specificity of transcription in vitro by bacterial and phage SP01-modified forms of polymerase, but replacement of rpoE by an in vitro-constructed deletion-mutated gene was found not to impair viability, sporulation, or the growth of phage SP01.

[A case report of pharyngeal diphtheria].

Report about a case of toxic diphtheria in a 25-years-old woman. In consideration of new findings about the serum antibody level to diphtheria toxin in the population of the GDR the speciality in this case is described. The risk of possibility on more diseases is demonstrated. In result of all findings will it be necessary, that the disease of diphtheria must be recapitulated. It must bear in mind, that such severe cases can be avoided by systematic revaccination even of adults.
