[Depressive patients in primary care].

The study aimed at investigating out-patient treatment of patients with depressive disorders referred to a psychiatric hospital by general practitioners (GP) and psychiatrists in private practice (PP). Data of the German psychiatric basic documentation system (DGPPN-BADO) of all depressive inpatients admitted to a psychiatric hospital in 2003 were analysed (n = 360). GP-patients had significantly less often a psychopharmalogical treatment before admission than PP-patients (59.4% vs. 89.7%). GP give significantly less often antidepressive drugs than PP do (53.1% vs. 87.9%). 4,7% of GP-patients and 17.9% of PP-patients received supportive psychotherapy, special psychotherapeutic treatment regimes were seldom in use during outpatient care (4,7% vs. 5,4%).

Multi-center evaluation of analytical performance of the microparticle enzyme immunoassay for sirolimus.

OBJECTIVES: This study evaluated the analytical characteristics of the new Abbott microparticle enzyme immunoassay (MEIA) for sirolimus. DESIGN AND METHODS: The protocol consisted of nine sections: evaluation of antibody specificity, linearity, detection limit, quantification limit, endogenous interferents, exogenous interferents, precision, proficiency testing panel, and method comparison. RESULTS: The mean analytical detection limit was 0.68 microg/L. The sirolimus concentration corresponding to a total CV of 20% was 1.5 microg/L. Linearity of response was demonstrated across the dynamic range of the assay. Total precision (CVs) at QC control levels from 5 to 22 microg/L ranged from 5.7 to 12.6%. Assay standardization was found to be in good agreement with LC/MS/MS as compared with target values for spiked sirolimus proficiency samples from an international sirolimus proficiency testing program. Good correlations (R values) of the immunoassay were observed in comparisons to LC/MS/MS. R values tended to be lower in comparisons with LC/UV methods. Across both LC-based methods and all study sites, there was approximately 25% overall positive slope bias due to cross reactivity of the MEIA antibody to metabolites of sirolimus. The assay cross-reactivity to metabolites of sirolimus parent drug ranged from 6 to 63%. Assay interferences were minimal with the exception of hematocrit, which presented a negative relationship to measured sirolimus concentration. CONCLUSIONS: The MEIA demonstrated acceptable analytical characteristics for use for routine monitoring of sirolimus immunosuppressive therapy, and is a viable alternative to HPLC-based methods for sirolimus monitoring.

[Reduction of traumatic dislocations and facet fracture-dislocations in the lower cervical spine].

BACKGROUND: Traumatic facet dislocations and facet-fracture dislocations in the lower cervical spine (C2/C3 to C7/T1) are frequently associated with devastating neurological symptoms. A good outcome can only be achieved if the operator has wide and sound knowledge of reduction techniques and the best possible strategy is devised for the subsequent treatment of these severe lesions. PATIENTS AND METHODS: Between 1973 and 1997 a total of 117 of our patients met at least one of the following inclusion criteria: unilateral locked facet dislocation (48%), bilateral locked facet dislocations (23%), unilateral "perched" facet subluxation (14%), bilateral perched facet subluxation (12%), uni- or bilateral dislocation/perched subluxation with facet fractures (3%). RESULTS: Most of the lesions were located at the levels of C5/C6 and C6/7 (n=46 for each). Associated neurological deficits were present initially in 65% of patients: 35% had complete or incomplete spinal cord injuries (tetraplegia), 2% were paraplegic, and 28% had cervical radiculopathies. CONCLUSIONS: Closed reduction (e.g. with the aid of a halo ring) should be carried out as soon as possible after lower cervical spine dislocation or facet-fracture dislocation, as both the success rate of reduction and the potential for recovery from neurological deficits are clearly higher when reduction is achieved within the first 4 h after the initial injury.

A rare case of primary rectal rhabdomyosarcoma in an adult.

We report an extremely rare case (1:200 000) of primary rectal rhabdomyosarcoma (RMS) in an adult. In the literature, this case report is the first one dealing with an adult patient. The diagnosis was assessed by computed tomography (CT), magnetic resonance imaging (MRI), positron emission tomography (PET), endoscopy and histological examination. Neoadjuvant systemic chemotherapy with adriamycin and isofosfamide followed by anterior rectum resection was performed. This article should implicate RMS as a rare differential diagnosis of atypical rectal tumours in adults.

Caregivers' adaptation to change: the impact of increasing impairment of persons suffering from dementia on their caregivers' subjective well-being.

As progressive dementia involves changes in patients' behaviour and cognitive and functional abilities, dementia caregiving can be considered as a process that demands continuous adaptation to change. The current intervention study investigated associations between the course of dementia patients' impairment and their caregivers' well-being over two years. One hundred and twenty-eight care recipient-caregiver dyads participated in a controlled randomised intervention study (psycho-educational group intervention), 99 dyads in which the care recipients still lived at home participated in the second assessment, and 75 dyads in the third assessment. Caregivers were interviewed about their subjective well-being (emotional well-being and life satisfaction) as well as care recipients' behavioural problems and functional disability. Care recipients completed various neuropsychological tests. The assessments were repeated one and two years later. The psycho-educational intervention had a positive impact on caregivers' well-being. Level and increase in behavioural problems and increases in cognitive and functional impairment negatively affected caregiver well-being over time. For participants from the control condition the negative association between increase in impairment and decrease in caregiver well-being over time was stronger than for the caregivers in the intervention group. Our results suggest that it is not only the severity of current problems and stress, but also the rate of change, that is important for caregivers' well-being. Psycho-educational group intervention may help caregivers to adapt to the increasing impairment of care recipients with dementia.

Calculation of rotational partition functions by an efficient Monte Carlo importance sampling technique.

The evaluation of the classical rotational partition function represented by a configuration integral over all external and internal rotational degrees of freedom of nonrigid chain polyatomic molecules is described. The method of Pitzer and Gwinn is used to correct the classical partition function for quantum mechanical effects at low temperatures. The internal rotor hindrance and all coupling arising from the external and internal rotational degrees of freedom are explicitly taken into account. Importance sampling Monte Carlo based on the adaptive VEGAS algorithm to perform multidimensional integration is implemented within the TINKER program package. A multidimensional potential energy hypersurface is calculated with the MM3(2000) molecular mechanics force field. Numerical tests are performed on a number of small n-alkanes (from ethane to octane), for which the absolute entropies calculated at three different temperatures are compared both with the experimental values and with the previous theoretical results. The application of a more efficient importance sampling technique developed here results in a substantial reduction of statistical errors in the evaluation of the configuration integral for a given number of Monte Carlo steps. Error estimates for the calculated entropies are given, and possible sources of systematic errors, and their importance for a reliable prediction of the absolute entropy, are discussed.

Preoperative short-term radiation therapy (25 Gy, 2.5 Gy twice daily) for primary resectable rectal cancer (phase II).

To evaluate the feasibility, effectiveness, and long-term bowel function of preoperative hyperfractionated accelerated radiotherapy in primary resectable rectal cancer. A total of 184 consecutive patients (median age 65 years, male : female=2 : 1) with clinical T3Nx rectal adenocarcinoma received preoperative pelvic radiation therapy with single fractions of 2.5 Gy twice daily (interval 6 h between fractions) to a total dose of 25 Gy within 1 week. Surgery was conducted the following week. Postoperative histology revealed UICC stage I in 33%, stage II in 26%, stage III in 34%, and stage IV in 7% of the patients. Median follow-up was 43 months (53 months for surviving patients). The actuarial 4-year-local-recurrence rate was 2.1%, overall recurrence 23%. Disease-specific and disease-free survivals at 4 years (excluding stage IV) were 82 and 69%, respectively. Overall survival for 4 years was 68%. Postoperative mortality was 0.5% (one patient), early anastomotic leakage occurred in 11.4%, and anastomotic stenosis requiring treatment in 6%, of 132 patients with primary anastomosis. Seven of 184 patients (3.8%) died of abdominal complications, all within the first year. Bowel function was satisfactory after more than 5 years. Local control in primarily resectable rectal cancer after 10 x 2.5 Gy is excellent, warranting further evaluation of this treatment.

[Magnetic Drug Targeting--a new approach in locoregional tumor therapy with chemotherapeutic agents. Experimental animal studies].

BACKGROUND: Advanced squamous cell carcinomas of the head and neck region were often treated with combined radio-chemotherapy. Radiotherapy allows a focused treatment of the tumor, and healthy tissue can be protected from radiation. Chemotherapy, however, is mostly given systemically and the unwanted negative side effects also develop in many other organs. AIM OF THE STUDY: Locoregional application of chemotherapeutic agents with Magnetic Drug Targeting on an animal experimental study. METHODS AND RESULTS: Magnetic Drug Targeting is a new approach to the locoregional treatment of tumors. Ferrofluids (colloidal dispersion of magnetic nanoparticles) were reversibly bound to chemotherapeutic agents and injected intra-arterially, while focused with an external magnetic field to a certain body compartment (i.e. the tumor). With only 20% or 50% percent of the regular systemic chemotherapeutic dose, we achieved an up to 26 times higher concentration in the tumor region with this application compared to the usual systemic administration. CONCLUSION: Magnetic Drug Targeting offers an unique opportunity to treat tumors locoregionally with chemotherapeutic agents.

[The situation of siblings of mentally ill people].

OBJECTIVE: The purpose of this article is to provide an overview of the current research regarding the situation of mentally ill persons. METHODS: The relevant literature was identified by means of a computerized MEDLINE search on the years 1996 - 2004 with the key words "sibling" and "mentally ill"/"chronically ill"/"psychiatric disorder" (631 articles). Also the related articles in the MEDLINE search were viewed. Finally 170 articles of journals and books were included in the review. RESULTS: The situation of siblings of psychiatric ill has manifold aspects, which can be classified into the following categories: Burdens due to the relationship to the ill sibling, burdens because of the relations to the shared parents and other family members, and burdens in the daily life of the siblings due to the illness of his/her sibling. On the other side there are rewards and positive developments in their life because of the illness of the sibling. Personal variables like sex, family size and age at onset of the illness are an influence on the experience of the sibling. CONCLUSION: The revealed burdens of the affected siblings require necessary answers by political representatives, professionals in and outside a clinic, and the involved families.

[Pericardial empyema due to oropharyngeal infection -- a rare entity].

Both the descending necrotizing mediastinitis (DNM) and the pericardial empyema are rare but life threatening situations, in which rapid aggressive surgical approach is important for the outcome. A 37-year-old man presented with reduced general health and new establishment of thoracal pain after an infection in the oropharyngeal region. The dissemination of the infection into the mediastinum (mediastinitis) diagnosed by clinic and radiography required surgical intervention. A pericardial empyema was detected intraoperatively. The patient was discharged home on the 14 (th) hospital day.

Interstitial glucose in skeletal muscle of diabetic patients during an oral glucose tolerance test.

AIM: The presence of a transcapillary arterial-interstitial gradient for glucose (AIG(glu)) in skeletal muscle may be interpreted as a consequence of intact cellular glucose uptake. We hypothesized that the AIG(glu) decreases in Type 2 diabetes mellitus as a consequence of insulin resistance, whereas it remains intact in Type 1 diabetes. METHODS: Glucose concentrations were measured in serum and interstitial space fluid of skeletal muscle during an oral glucose tolerance test (OGTT) in patients with Type 1 and Type 2 diabetes and in young and middle-aged healthy volunteers, using microdialysis. RESULTS: The area under the curve for glucose in serum (AUC(SE)) was higher than in interstitial space fluid of skeletal muscle (AUC(MU)) in healthy young (AUC(SE) = 1147 +/- 332 vs. AUC(MU) = 633 +/- 257 mM/min/ml; P = 0.006), healthy middle-aged volunteers (AUC(SE) = 1406 +/- 186 vs. AUC(MU) = 1048 +/- 229 mM/min/ml; P = 0.001) and in Type 1 diabetic patients (AUC(SE) = 2273 +/- 486 vs. AUC(MU) = 1655 +/- 178 mM/min/ml; P = 0.003). In contrast, in Type 2 diabetic patients AUC(SE) (2908 +/- 1023 mM/min/ml) was not significantly different from AUC(MU) (2610 +/- 722 mM/min/ml; P = NS). CONCLUSION: The present data indicate that AIG(glu) is compromised in Type 2 diabetes in contrast to Type 1 diabetes where it appears to be normal. Because no changes in muscle blood flow were detected, insulin resistance appears to be the main cause for the observed decreased AIG(glu) in skeletal muscle in Type 2 diabetic patients.

Pupil campimetry in patients with visual field loss.

Pupil campimetry is an objective test of the visual field. In pupil campimetry, the pupil light response elicited by focal light stimuli at different locations within the visual field is measured. Pupil campimetry was performed in patients with visual field loss because of different pathologies. The match between reduced pupil response and visual field loss was assessed. The pupil field of 23 patients was recorded by infrared video-pupillography with light stimuli generated on a computer screen. A visual field was obtained on the same day with the Tuebingen Automated Perimeter. Both fields were compared by subjective judgement. Six of nine patients with large, two of five patients with incomplete, and three of six patients with minor field loss showed matching defects in their pupil and their visual fields. In functional loss of visual field sensitivity, no pupil field defect was seen (three patients). In most patients, the loss of visual field sensitivity could be revealed objectively by pupil campimetry.

Effect of exercise on transdermal nicotine release in healthy habitual smokers.

OBJECTIVE: Transdermal nicotine patches have become a frequently prescribed tool in smoking cessation programs during the past years. However, there is circumstantial evidence that transdermal nicotine release substantially varies with physical activity producing toxic plasma concentrations that may account for severe adverse events. METHODS: We, therefore, compared nicotine release from two different transdermal nicotine systems (TDNS) at rest and during strenuous physical activity in a two-period crossover study in healthy smokers (n = 10). The subjects were randomly assigned to receive either 21 mg/day of formulation A or B on study Day 1 and 2. Patches were applied eight hours before starting standardized physical activity, and nicotine concentrations were measured in plasma and topically in the tissue layers underneath the application site by microdialysis. RESULTS: There was no difference between groups in the mean values for area under the time-concentration curve at rest from 0 - 8 hours AUC(0-8) (p < 0.799) and during exercise from 8 - 11 hours AUC(8-11) (p < 0.878). C(max) values between groups with C(max) values of 16.4 +/- 9.5 ng/ml and 16.0 +/- 10.7 ng/ml at rest (p < 0.919, NS) and 10.05 +/- 6.8 ng/ml and 10.2 +/- 6.9 ng/ml (p < 0.959, NS) during exercise did not differ significantly. Nicotine tissue concentrations increased two-fold during exercise versus baseline (p < 0.878). Skin blood flow increased significantly during exercise compared with baseline (p < 0.001). No adverse events were observed. CONCLUSION: The present study provides evidence that transdermal nicotine release from TDNS increases during exercise. However, this increase has no significant effect on overall plasma pharmacokinetics. Our pharmacokinetic data further indicate that the two TDNS formulations are equivalent during conditions of rest and exercise.

[Expectations and satisfaction from relatives of psychiatric patients in inpatient treatment].

Based upon a content analysis of interviews(n=32), a questionnaire evaluating expectations and satisfaction with psychiatric inpatient care was developed and sent to all caregivers whose ill family members gave their agreement to the survey (n=1 39). The response rate was 41.7% (n=58). Successful treatment of the patient by competent physicians and psychologists, understandable and sincere explanations to caregivers regarding the patient's disease, support of the patient's self-confidence and gains in confidence, psychotherapy for the patient, and by no means psychopharmacological treatment were assessed as most important by caregivers. From their point of view, gaps between expectations and satisfaction with inpatient care exist, especially regard-ing the following aspects: patient psychotherapy, information to caregivers about therapies, medication, side effects and treatment progress, understandable and sincere explanation to care-givers, support in dealing with the patients, and information about the possibilities of rehabilitation after inpatient care.

[Whole-body MRI and PET/CT in tumor diagnosis].

Tumor staging according to the TNM-system influences prognosis and therapeutical options of patients with a malignant disease. It is the challenge of diagnostic imaging to depict the exact localization of the primary tumor and to detect or rule out lymph node involvement or distant metastases. In doing so, the complete body anatomy should be covered with a modality that offers high sensitivity and specificity. As these requirements could not or only partially be achieved by previous ordinary procedures, the use of multiple different modalities became necessary. Last but not least, in consideration of the costs it would be preferable to replace this cascade of different modalities by a "whole body examination", preconditioned that the same accuracy is achieved.With PET/CT and whole-body MRI, two newly available promising methods for a systemic tumor staging have been developed. First experiences indicate PET/CT as a method of first choice. With the introduction of new whole-body MRI scanners using parallel imaging technique (iPAT) and free table movement, MRI plays a more and more important role in whole body tumor staging.

Hyperfractionated accelerated radiochemotherapy (HFA-RCT) with mitomycin C for advanced head and neck cancer.

To investigate efficacy and feasibility of hyperfractionated accelerated radiotherapy combined with mitomycin C, patients with locally advanced unresectable squamous cell carcinomas of the head and neck region were administered 64-66 Gy in four weeks and mitomycin C (20 mg/m(2)) on day five. Twenty-one consecutive patients were included between November 1997 and June 1999 (median age: 57 years). All tumours were stage T3-4 and 18/21 were N2-3. Eighteen patients experienced grade 3 and three patients grade 2 mucosal toxicity. With median follow up for surviving patients of 42 months, loco-regional control was 55% at three years, overall survival was 33% at three years. This treatment is at the edge of local tolerability, but there is a good curative chance even for very advanced localised tumours, provided a complete remission is induced at primary treatment.
