Chylous ascites following aneurysm surgery.

A case of chylous ascites occurring after elective abdominal aortic aneurysm repair is described. The patient was initially managed conservatively with hyperalimentation and intermittent drainage of the retroperitoneal collection, but, this proved inadequate and a peritoneo-venous shunt was inserted. Although the shunt appeared to work well the patient finally died of septicaemia.

The surgical value of community hospitals: a closer look.

To assess and compare the safety and efficiency of a surgical unit at a District General Hospital and a Community Hospital for elective cold surgery. A retrospective survey was made using regional computer Hospital Activity Analysis data and monthly audit meeting records of all inpatients having selected elective general surgical operations on one consultant's firm over a period of one year, at Torbay Hospital, Torquay, (DGH) and Newton Abbot Hospital, Newton Abbot (CH). All inpatients and day cases admitted under CACC from April 1, 1987, until March 31, 1988, were studied comparing mortality, morbidity and efficiency for comparable elective surgical procedures. A postal questionnaire to assess patient satisfaction with surgical care at both hospitals was included in the study. 350 patients underwent elective surgical procedures at the DGH and 380 patients underwent similar procedures at the CH. There was no significant increase in mortality and morbidity at the CH, which was preferred by most patients for routine surgical operations. The Community Hospital unit was found to be satisfactory and more efficient for routine elective surgery as determined from the above parameters and the patients themselves.

Antigenic and structural analysis of Treponema denticola.

Polypeptide and Western immunoblot profiles of subcellular fractions of Treponema denticola ATCC 33520 have been determined by SDS-PAGE of Triton X-100-soluble and -insoluble fractions, a lipopolysaccharide-enriched fraction and purified flagella. Major Triton X-100-soluble polypeptides of 72, 68, 54 and 52 kDa were detected. The 54 kDa polypeptide appeared to be a breakdown product of a larger, heat-modifiable polypeptide. Based on the results of SDS-PAGE analysis and immunoblotting of proteinase K digests of T. denticola, a 'rough' lipopolysaccharide appeared to be present. Electron microscopy has been used to monitor the effect of detergent treatment on the morphology of the organism and to examine the detailed structure of the flagella. Treatment with Triton removed the T. denticola outer membrane, resulting in exposure of the flagella. The flagella were shown to have a complex sheath and core structure and polypeptide composition characteristic of that observed for other treponemes. Polypeptides of 38, 35, 32 and 28 kDa were present in purified flagella preparations. Immunoelectron microscopy, iodine-labelling and Western blotting were used to demonstrate the exposure of antigens on the T. denticola surface. Surface iodination located polypeptides of 72, 68 and 54 kDa. Antiserum raised against whole cells of T. denticola recognized these polypeptides and an additional polypeptide of 52 kDa. These data provide a basis for future detailed molecular analysis of the ultrastructure and antigenicity of T. denticola.

An Alberta Cree Indian with a rare Duffy antibody, anti-Fy 3.

Anti-Fy3, found in the serum of an Fy(a-b-) Cree Indian woman believed to have been transfused 5 years previously, caused moderate hemolytic disease of the newborn in her eighth live-born baby. Only three examples of anti-Fy3 are know. It is striking that two were made by non-negro people, amongst whom Fy(a-b-) is extremely rare, and only one by a Negro, in whose people Fy(a-b-) is by far the commonest phenotype. This suggests that Fy(a-b-) may be a heterogeneous phenotype, with the negro version conferring some impediment to immunization by Fya, Fyb or Fy3 antigens. The rare allele Fyx is also present in the kindred of the propositus, and is shown to correspond to weak Fy3 as well as weak Fyb anitgen.
