[Familial adenomatous polyposis: early diagnosis by genetic mapping].

The use of probes detecting polymorphic loci within the human population has enabled accurate localization of the genetic defect responsible for familial adenomatous polyposis on chromosome 5. This was used to screen two families for the presymptomatic diagnosis in children of an affected parent. In both cases, the use of 8 polymorphic probes located on either side of the gene provided information which could be used in the management of children born from the patients at risk. The set of probes used in this work should be informative in most of the affected adenomatous polyposis families.

When problems seem overwhelming: emphases in teaching, supervision, and consultation.

The intractability of many of today's problems creates unusual stress for social workers. Traditional direct practice content taught by teachers, field work instructors, agency supervisors, and consultants provides a sound foundation on which to teach and learn practice skills. However, it also is essential for teachers, instructors, supervisors, and consultants to recognize the unusually stressful nature of practice and the frequent sense of pervasive hopelessness felt by practitioners. This article focuses on the preparation of social workers for effective work in today's complex social environment and suggests five major areas that need to be emphasized in teaching, supervising, and training.

Circulating antibodies against c-myc oncogene product in sera of colorectal cancer patients.

We developed a Western blot assay using purified human c-myc protein expressed in E. coli in order to look for circulating anti-c-myc antibodies in human sera. The presence of IgG antibodies against c-myc was observed in 25 out of 44 sera from patients with colorectal cancer diagnosed and treated at the Hôpital Curie in Paris, compared to the sera of 46 normal donors of which only 8 samples were positive (p = 0.001).

Effect of testosterone on regulation of the level of sex steroid-binding protein mRNA in monkey (Macaca fascicularis) liver.

Two specific oligonucleotide probes complementary to different regions of human sex steroid-binding protein (SBP) cDNA were used to study the levels of hepatic monkey SBP mRNA during different hormonal states. In females the SBP mRNA level was higher than in males and paralleled the serum SBP level. After castration, the SBP concentration increased in the serum but was reduced after testosterone treatment. In contrast, the hepatic SBP mRNA level decreased after castration and was restored by testosterone treatment. These results suggest a high homology of the nucleotide sequence between human and monkey SBP mRNAs. The changes in liver SBP mRNA levels may explain the sex difference in plasma SBP concentrations, but mechanisms other than the regulation of transcription may regulate the plasma concentration in monkeys.

[Intra-epithelial cancer of the anal canal. Pathogenic study apropos of 5 cases].

Anal carcinomas are rare and their precancerous conditions are not well known. Two populations at risk are described, elderly women and, recently, homosexual males. Early detection of dysplastic lesions or intraepithelial carcinoma in the anal mucosa could lead to preservation on the anal sphincter and consequently to improvement of quality of survival. The present study included 3 women and 2 men. The pathological examination of the surgical specimens in these 5 cases (hemorrhoidal procidence, rectal prolapsus, fibrous polyps, fissure) showed, in all cases, an intraepithelial carcinoma developed in the squamous epithelium of the anal canal. In one of the 5 cases, the anal lesion was concomitant with an intraepithelial carcinoma of the uterine cervix. Complete local resection of the lesions were performed in all cases, associated with complementary radiotherapy in one patient. On follow-up, we observed one recurrence after 8 months in one out of 5 patients. All patients are alive. Histogically, the specimens showed an intraepithelial carcinoma in all 5 cases, with a microinvasive carcinoma in one case. The transitional mucosa of the anal canal showed dysplatic modification in all cases. Immunohistochemical study of the 5 cases did not discern the papilloma virus antigene. In the one case where it was performed, molecular hybridization showed a type 33 papillomavirus. The risk factors of anal carcinomas seems to be changing, in particularly the incidence is increasing in the homosexual patients. In this particular population, as in the uterine cervix, the human papilloma virus is probably one of the main etiological factors. The management of these lesions is not well-defined.(ABSTRACT TRUNCATED AT 250 WORDS)

Small bowel and plasma gastrin rhythms in cats.

The purpose of this experiment was to study the possible role of the gastric antrum and small bowel in the rhythm(s) of plasma gastrin. The cat was used as the laboratory animal. Three groups of cats were provided with a gastric fistula for the study of gastric acid and plasma gastrin rhythms. The first group (N = 7) served as controls. A second group (N = 3) was antrectomized and later subjected to a 80% small bowel resection. Gastric acid secretions were collected every 30 min from 0800 to 2400. Blood samples for determination of gastrin were drawn every 2 hr from 0800 to 2400. In control animals a circadian (i.e. approximately 24 hr) and 3 ultradian (i.e. less than 24 hr) rhythms were detected for acid output. In the antrectomized cats, circadian and ultradian rhythms were documented. After small bowel resection circadian and ultradian rhythms in gastric acid secretion were observed. For plasma gastrin, circadian and ultradian rhythms were found in the control cats. In the antrectomized cats no rhythms were observed. After small bowel resection an ultradian rhythm reappeared in these antrectomized cats. Removal of the antrum in the cat induces disappearance of circadian and ultradian rhythms of plasma gastrin but fails to modify the acid rhythms. Small bowel resection results in the reappearance of an ultradian rhythm for plasma gastrin and a shift in acrophase for the circadian rhythm in acid secretion.

[Paget's disease of the nipple without any associated mammary tumor clinically or radiologically detectable. Apropos of 51 cases treated at the Curie Institute].

Between 1960 and 1984, 51 patients with Paget's disease of the nipple, with no concomitant palpable or radiological breast tumour, were treated at the Institut Curie. The diagnosis was made in all cases through a simple biopsy of the nipple and/or the areola. All slides were reviewed for the present study. 20/51 patients were treated conservatively, with external radiotherapy alone (17/20 pts), or limited surgery and radiotherapy (3/20 pts). The mean age was 54 years (range 40 to 80). The median follow-up was 7.5 years. No patients died of breast disease. Three local recurrences occurred and were treated by mastectomy. All were located in the nipple or areola and were Paget's disease without intraduct or invasive carcinoma. The actuarial probability of living 4.5 years free of disease when the breast is preserved with a good cosmetic result is 81%. 31/51 patients underwent mastectomy; 30 a modified mastectomy, 1 a simple mastectomy. The mean age was 66 years (range 55 to 90). Associated intraduct carcinoma was found in 30/31 microscopic examinations. No axillary node invasion was found after dissection of the axilla. In the group treated by surgery, the median follow-up was 5 years. No patient died of breast disease. No chest wall recurrence occurred. The 5 years disease-free survival rate was 87%. This retrospective non randomized study of 51 patients treated for Paget's disease of the nipple suggests that radiotherapy is a valuable alternative to mastectomy.(ABSTRACT TRUNCATED AT 250 WORDS)

Cytogenetic study of twelve human near-diploid breast cancers with chromosomal changes.

The karyotypes of 12 fresh breast cancers, including two of the male, selected for their near-diploidy and their slight number of anomalies, i.e. less than 10 rearranged chromosomes, are reported. A clonal evolution could be demonstrated in 4 cases. Most of chromosomal imbalances result from structural rearrangement, frequently after breakage in juxtacentromeric heterochromatin. There does not seem to exist a specific breakpoint, but many of the imbalances are recurrent. They are, by decreasing order of frequency: gain of 1q, (7 cases), losses of 11q, 16q and 1p (5 cases), losses of 8p and 13q (4 cases), gain of 8q and losses of 6q and 17p (3 cases). None of these anomalies can be regarded as primary, but they are likely to be selected because they confer a slight selective advantage for the carrier cells during the tumoral progression.

Trisomy and tetrasomy for long arm of chromosome 1 in near-diploid human endometrial adenocarcinomas.

Karyotypic analysis by R-banding after short-term culture, carried out on 7 cases of human endometrial adenocarcinoma, showed in 4 of these a trisomy or a tetrasomy for the long arm of chromosome I. In the 4 cases, these imbalances were due to rearrangements involving centromeric or para-centromeric break-points: 46,XX,-16, +der(1q16p) t(1;16)(1p16q;1q16p); 46,XX,-21, +der (1q21q)t(1;21) (1p21p;1q21q); 46,XX, -21, +der(21) t(1;21)(q11;p13); 48, XX, +2, +i(1q). Two other cases showed only a numerical aberration: 47, XX, +10 and 47, XX, +12. In the last case, only cells with apparently normal karyotype were seen. In the 4 cases with an anomaly of chromosome I, two normal I chromosomes coexisted with abnormal elements. This shows that the rearrangement very likely occurred in G2 phase of the cell cycle.

[Rearrangement of the chromosome 17 in colonic adenocarcinoma].

Chromosome studies on five cases of large bowel adenocarcinoma show a systematic rearrangement of chromosome No. 17 after breakage in its juxtacentromeric region (band 17 q 11). A frequent involvement of chromosome No. 8 (juxtacentromeric break) and the loss of chromosome No. 18 are also noticed. A review of the literature strengthens the hypothesis of a preferential involvement of these chromosomes in large bowel cancer.

[Markers in the detection and prevention of colorectal carcinomas (author's transl)].

The ever increasing frequency of colorectal carcinomas in Western countries justifies the search for, and development of early detection and prevention methods. The use of markers should improve the prognosis of such cancers in the near future. With the exception of occult blood testing, which has been used in mass screening, most markers are still reserved for high risk population studies. These markers will also help to detect pre-neoplastic conditions where prevention would be possible.

Bile reflux esophagitis. A critical study of two models in the rat.

This study was designed to determine the relative contribution of acid and/or duodenal contents on development of reflux esophagitis in the rat. Gross and microscopic changes in the esophageal mucosa at 10, 15, 30, and 60 days were recorded in sham-operated animals and in two experimental groups. One study group had a transsection of the esophagogastric junction (model I), and the other a total gastrectomy (model II). Bowel continuity was reestablished by esophagojejunal anastomosis. Subgroups of both animal models received either no treatment of cholestyramine or aluminum phosphate for 30 days in drinking water. At 30 dyas a similar degree of esophagitis was found in the untreated animals of both study models. Cholestyramine improved the degree of esophagitis in model II, whereas aluminum phosphate increased mucosal alterations in model I. At 60 days, 50% of the animals no longer had esophagitis and had regained satisfactory nutritional status. It is concluded that: (1) bile reflux is a major factor in esophagitis; (2) cholestyramine protects against bile reflux, and (3) proper nutritional status is of major importance in mucosal healing.
