Doppler echocardiographic analysis of left ventricular filling in treated hypertensive patients.

Early detection and prevention of cardiac dysfunction is an important goal in the management of hypertensive patients. In this study, Doppler echocardiography was used to evaluate the pattern of left ventricular diastolic filling in 38 subjects: 18 treated hypertensive patients (blood pressure 141 +/- 17/83 +/- 10 mm Hg, mean +/- SD) without other coronary risk factors and 20 risk-free normotensive subjects of similar age (47 +/- 10 and 49 +/- 13 years, respectively). Peak velocity of late left ventricular filling due to the atrial contraction was greater in hypertensive compared with normotensive subjects (69 +/- 14 versus 52 +/- 13 cm/s; p less than 0.001). Peak velocity of late filling was significantly greater in hypertensive versus normotensive subjects in those aged 50 years or younger and those older than age 50 (65 +/- 12 versus 50 +/- 11; p less than 0.01 and 75 +/- 15 versus 56 +/- 15 cm/s; p less than 0.05, respectively). In hypertensive subjects, peak velocity of late filling did not correlate with routine indexes of hypertensive heart disease (including posterior wall thickness and left ventricular mass), systolic and diastolic blood pressure or duration of hypertension. These results indicate that increased velocity of late left ventricular filling may be independent of left ventricular hypertrophy and persist despite effective blood pressure control.

Academic research and industry relationships.

Economic forces are such that industry-university relationships will occur. It is impossible to pull up the draw-bridge and refuse to deal with industry. The challenge is to structure arrangements which facilitate the transfer of new knowledge to industry, for the ultimate benefit of society, without doing violence to the traditional values of the university. Good arrangements are made when both parties bring something to the deal. There must also be compromise. Each party must be willing to concede some points. It has been said that good deals are those with which neither party is completely happy. Satisfaction and dissatisfaction must be equally distributed. Finally, as in all things, success depends on trust and understanding between good people on both sides.

Late false aneurysm following replacement of ascending aorta: the problem of the teflon graft in combination with a silk suture anastomosis.

Two patients underwent resection and replacement of the ascending aorta using a low-porosity Teflon graft anastomosed with silk suture. In both patients false aneurysms developed that required operation 13 and 23 years postoperatively. The clinical courses of these patients, along with data from the literature, suggest that the combination of a low-porosity Teflon graft and a silk suture anastomosis presents a major potential hazard for the development of anastomotic false aneurysm.

A parlous state of storm and stress. The life and times of James B. Herrick.

James B. Herrick saw medicine becoming more dependent on science but was unwilling to let the human values of the family doctor vanish. His philosophy was summarized in the phrase, "The true physician must possess a dual personality, the scientific toward disease, the human and humane toward the patient."

Presidential address. Coronary bypass surgery: status 1979.

Coronary bypass surgery is highly effective in relieving angina pectoris. Early results of several well controlled studies show a small, but definite positive effect of surgery on life expectancy. The effect on prognosis is greatest in patients with the most severe arterial and myocardial disease. The discrepancy between the large effect on symptoms and the small effect on survival is best explained by postulating more than one mechanism of action for the operation.

Research related to medical treatment of cardiovascular disease.

Medical treatment of heart disease has improved significantly in the past 30 years. The spectacular change in the natural history of rheumatic heart disease is apparent from a 1948 article that reported that 42% of children with rheumatic heart disease died of rheumatic infection or bacterial endocarditis. Antibiotics and cardiac surgery have improved the outcome from rheumatic heart disease. Cardiopulmonary resuscitation has had a major impact on the treatment of myocardial infarction and on the management of sudden death. The fundamental principle underlying the discovery of cardiopulmonary resuscitation is reviewed, and recent developments emphasizing the importance of intrathoracic pressure in the hemodynamics of cardiopulmonary resuscitation are highlighted. The important new drugs of the last 30 years include the oral diuretics, the antihypertensives and the antiarrhythmic agents. The development of the beta-blocking agents is cited as an example of the translation of basic physiological research to medical care. Finally, the role of epidemiologic techniques in the design of clinical trials to evaluate medical therapy and hence improve medical management is discussed.
