Brainstem haematoma due to presumed cryptic telangiectasia.

Three patients with primary brainstem haematoma are reported. The clinical presentation suggested an initial diagnosis of pontine tumour in two and demyelination in one patient. The subacute course is characteristic of brainstem haematoma due to presumed cryptic telangiectasia, the abnormal vessels being destroyed by the haemorrhage. These findings emphasise the importance of considering haematoma due to cryptic telangiectasia in the differential diagnosis of subacute brainstem lesions.

A case of convergence evoked eyelid nystagmus.

The clinical and radiological features of a patient with convergence evoked, upward beating eyelid nystagmus, which occurred independently of ocular upbeating nystagmus, are presented. The eyelid nystagmus resulted from a large angiomatous lesion distorting the blood supply to the pontomesencephalic and pontomedullary junctions.

Abdominal and pelvic imaging as part of liver-spleen scintigraphy for the detection of mesenteric bleeding in trauma. A case report.

Liver--spleen scintigraphy in patients who have incurred blunt abdominal trauma is accepted as a useful diagnostic tool. Abdominal and pelvic imaging to detect extrahepatic or extrasplenic sites of hemorrhage should be a routine part of liver--spleen imaging in the evaluation of blunt abdominal injury. A patient with mesenteric bleeding and a normal liver and spleen detected by liver--spleen scintigraphy is reported.

Cecal volvulus. A case for nonresectional therapy.

Sixteen cases of acute cecal volvulus were retrospectively evaluated. A follow-up was obtained averaging 5.6 years. The average age of the 14 female and two male patients was 47 years. Sixty-two percent of the patients were younger than 50 years of age. Eighty-one percent of the patients had undergone prior operation, with four of these 13 patients having been operated on within the preceding 30 days. Gynecologic procedures accounted for 40% of the previously performed operations. Plain abdominal films and results of barium enema examination were diagnostic or highly suspicious of cecal volvulus. Fourteen nonresectional procedures were performed. No subsequent abdominal operations for recurrence have been required. The concept that this lesion requires resection in the absence of necrosis cannot be supported by our data.

Asymptomatic microscopic hematuria.

Asymptomatic microscopic hematuria is a prevalent condition that requires urologic evaluation. Complete assessment must include non-urologic conditions. Of 246 patients reviewed 10 per cent were found to have a urologic neoplasm, while 12 per cent had other significant urologic lesions. However, 51 per cent of the patients had no urologic abnormality discovered and 27 per cent had insignificant urologic findings in which the relationship to microhematuria is questionable. A method of following patients with unexplained microhematuria that may clarify the implications of this disorder is proposed.
