Acne mechanica is a papulopustular eruption caused by the physical factors of pressure, occlusion, friction, and heat acting upon the skin, either separately or in unison. Athletes in certain sports are at particular risk for this condition. Dermatologists, if familiar with its presentation, can play a critical role in relieving its distracting symptoms during a competitive season, and eliminating it completely at the season's end.

Freed from much of the time-consuming burden of attending to elemental needs, the present generation seems committed as never before to the reshaping of its own physical proportions. Paradoxically, the skin, positioned at the interface between the organism and its environment, embodies the visible envelope that surrounds pleasingly restructured muscle groups; at the same time it exhibits unsightly lesions produced as a consequence of the fitness program.

Tetracycline, because of its effectiveness, safety, and economy, remains the treatment of choice for chronic maintenance of uncomplicated acne. However, a certain percentage of patients with very difficult acne will not respond satisfactorily to tetracycline. The study presented herein described forty-two patients with nodulocystic acne who did not show imporvement on adequate trials of tetracycline and were switched to low-dose clindamycin. A majority of these patients then showed significant improvement with nine showing essential clearing of the condition. Two patients experienced diarrhea, but recovered immediately after the clindamycin was discontinued.
A group of genodermatoses that differ in clinical appearance and cellular kinetics are included in the classification of ichthyosis. This condition is characterized by the formation of thick fish-like scales over the skin surface and may cause significant disability. New forms of treatment have recently been discovered that have greatly improved the prognosis for patients with all degrees of involvement.
