The case history is presented of a woman aged 38 who received warfarin treatment because of a mitral valve replacement. During the first trimester of pregnancy, her prothrombin time was 20% of normal and kept at 40% until the 38th week of gestation. Then the patient received heparin until a few days after delivery of a normal child.
Successful removal of a left atrial myxoma in a pregnant woman has not been previously reported. The patient postoperatively had an uncomplicated pregnancy, delivering at term a normal, healthy baby.

A phonomecanocardiographic study in 33 patients who had undergone aortic valve replacement with a glutaraldehyde-preserved Hancock bioprosthesis, has been carried out 6 to 42 months after surgery. All xenografts were considered well functioning on the basis of clinical, electrocardiographic and radiographic evaluation. Typical phonocardiographic features of a well functioning porcine heterograft in aortic position were considered 1) an opening snap of variable intensity recorded in about 50% of the patients; 2) a normal closing sound; 3) a protosystolic ejection murmur which was always present and whose intensity was inversely related to the prosthesis size; and 4) the absence of a diastolic murmur. The anatomical and functional characteristics of the aortic valve of the pig are suggested to explain both the absence of a diastolic murmur and the pattern of the closing sound, which is undistinguishable from that of a normal aortic valve. Moreover, the reduced pliability of the right coronary cusp of the porcine valve, due to the muscular shelf at its base, with consequent limitation of the effective prosthetic orifice and relative stenosis of the device correlates well with both the opening snap and the sytolic ejection murmur.

Phonocardiographic examination was carried out in 40 patients who had undergone to aortic valve replacement with the Björk-Shiley prothesis. The regular function of the prothesis was evaluated on the basis of the physical, radiological and electrocardiographic data. In the majority of the cases the first sound was slight. The prosthetic closing sound was made of two components (a2, A2) and the opening sound by one or more components (C1, C2, C3). A sistolic aortic murmur was always present, and a diastolic murmur has been found in 20% of the cases.
The Authors describe a case in which an anterior mediastinal tumor manifested as a cardiovascular disease. Phonocardiographic findings and their modifications induced by respiratory manoeuvres are presented and discussed. Particular enphasis is given to the disappearance, with deep inspiration, of the pulmonary ejection murmur associated with the simultaneous paradoxical behaviour of pulmonary component of the second sound. The Authors suggest that such findings can be indicative of pulmonary outflow obstruction by an external mass.
From May 1964 through December 1976, at the Department of Cardiovascular Surgery, University of Padova, Medical School, 148 patients underwent surgery for coarctation of the aorta; 62 of them where 18 years of age or older. The Authors deal particularly with the clinical symptoms, the operative findings, the surgical technique and the postoperative complications of their series. The early (only three operative deaths) and late results are considered satisfactory, and are discussed according particularly to the type of surgical intervention.
