[Ultrasonography and surgical complications after kidney transplants].

The authors conducted a retrospective analysis of 28 case-reports of patients with kidney transplants who had been investigated by ultrasonography. The most representative cases in the 8 patients in whom this examination revealed the presence of surgical complications are described. They included three cases of urinoma, in one case with associated obstruction, two patients with lymphoceles, one of whom had obstruction, and one case each of perirenal hematoma, perirenal abscess, and an arteriovenous fistula following needle biopsy.

[Program of early detection of breast cancer in Castilla-la-Mancha. Preliminary data of the first 5 months of performance. Regional Group on the Evaluation of the Program of Early Detection of Breast Cancer].

BACKGROUND: A Breast Cancer Screening Programme managed by the Regional Ministry of Health in Castilla-La Mancha (Spain) was started in November 1992 in two districts. The purpose of this study is to describe the short-term results after five months of running. METHODS DESIGN: descriptive, cross-sectional study. SETTING: female population of Talavera de la Reina and Cuenca cities, from November 1992 to the end of March 1993. PARTICIPANTS: 10.050 healthy women between 45-64 years old, who were invited, by personal letter, to attend a mammography unit. RESULTS: 6.503 women accepted the screening invitation (participation rate of 65%) without differences between both districts. The personal letter was the main information source to women, and the presence of beliefs, attitudes and favorable experiences were the main reasons to accept the screening. The rescreening was needed by 4% of women, and finally, 67 women (1.1%) were classified "positive". The programme cost for every women into the screening was 4.067 ptas. CONCLUSION: According to these results, is it feasible as economically as from the point of view of the organization, to extend the programme in Castilla-La Mancha.

A new sodium-nitroprusside-infusion controller for the regulation of arterial blood pressure.

Automatic optimization of the infusion rate of sodium nitroprusside (SNP) is achieved by an integrated hardware-software closed-loop controller implemented as a small bedside device. A microprocessor-based blood pressure monitor controls an infusion pump. The shape of the arterial pressure wave is digitally sampled; its analysis incorporates artifact-detection and -rejection routines. The implemented algorithm applies a rule-based closed-loop control that incorporates fuzzy logic. The system models the decision-making ability of the expert, instead of trying to model the patient's physiologic dynamics. Several internal fuzzy-state variables are defined to achieve a clear understanding of mean arterial pressure (MAP) evolution with time. The system performance is very robust, employing, under all possible situations, a sort of "common sense." A clinical trial of the controller was conducted in 60 patients requiring vasodilation therapy for systemic arterial hypertension following cardiac surgery, 20 who had conventional manual control by an experienced nursing staff and 40 who had automated closed-loop control. The first 240 minutes of the postoperative period were closely watched, taking into account 1) the percentage of time during which MAP was within the 10-mmHg wide frame above the target pressure (target gap); 2) the mean difference of pressure values that crossed the boundary of the target gap; 3) the mean SNP-infusion rate. With automatic control, the time mean arterial pressure values were located in the target gap during the first hour amounted to 72.8 +/- 6.7%, vs 51.2 +/- 10.3% in the manual-control group. In the second hour, it was 79.3 +/- 2.5% vs 67.4 +/- 11.7% (p < 0.005).(ABSTRACT TRUNCATED AT 250 WORDS)

nm23 expression in advanced and borderline ovarian carcinoma.

Twenty-four advanced (surgical stage III and IV) ovarian carcinomas and 15 borderline ovarian tumours were studied for the overexpression of nm23 and HER-2/neu (c-erb-B2) by means of immunohistochemistry on sections from routinely processed, paraffin-embedded, archival tumour blocks, using the NCL-nm23 and the NCL-CB11 monoclonal antibodies and the streptavidin-biotin-peroxidase technique. Significantly more advanced ovarian carcinomas (p = 0.034) expressed high levels of nm23 when compared to borderline tumours. HER-2/neu (c-erb-B2) expression, as could be expected, was also significantly more frequent in advanced ovarian carcinomas (p = 0.006). We were not able to find the previously reported association between nm23 and HER-2/neu overexpression in our tumours. Our results on nm23 overexpression in ovarian cancer are coincident with those previously reported using nm23-mRNA measurements on fresh ovarian tissues. Thus, ovarian carcinoma seems to belong to the group of tumours, like colon carcinoma and neuroblastoma, in which nm23 overexpression is associated with a more malignant phenotype. Immunohistochemistry performed on archival samples from ovarian carcinomas seems adequate for the demonstration of nm23 overexpression in ovarian cancer. This opens the possibility for larger studies on series of patients with a closed follow-up, which could help to establish the role of this gene in this kind of tumour.
