[Myelogenous system cells' activity in the bone marrow in children with acute leukemia remission].

Myelogenic system cells' activity was evaluated in 12 bone marrow samples taken from the children in acute leukemia's remission period and 7 samples from the patients in whom malignant disease of the hematopoietic system was ruled out. Cell activity was evaluated with the nFMLP induced chemiluminiscence test (CL) in the luminol environment. In the bone marrow of the patients in the period of acute leukemia remission the cells' chemiluminiscence was, considerably higher than in the control material. It appears that in the acute leukemia's remission period a functional regeneration of the myeloid line cells occurs.

[New possibilities of treatment with PEG-L-asparaginase in patients with acute lymphoblastic leukemia sensitized to l-asparaginase E.coli and erwinase].

L-asparaginase is widely used in the treatment of acute lymphoblastic leukemia in children and adults. Use of L-aspa E. Coli as well as Erwinase is not possible in all cases because of the side effects, mainly allergic reactions and disfunction of pancreas. Recently, the new form of the enzyme PEG-L-asparaginase was introduced. Binding L-asparaginase E. coli to polyethylene glycol a decreased its toxicity, extended its plasma half-live, not significantly affecting the efficacy. The aim of the study was to examine the results of PEG-L-asparaginase administration in five children with acute lymphoblastic leukemia, and the symptoms of intolerance to L-aspa E. Coli or Erwinase. There were three children with newly diagnosed ALL and two children with first relapse of ALL, treated according to New York Protocol and BFM-90 Protocol for ALL relapses respectively. PEG-L-asparaginase (Oncaspar) was administered in the dose of 2500 IU/m2. According to the protocol four children received 11 courses of treatment with the full dose of the drug. The number of doses for individual patient varied from one to six. The short-lived nettlerash was observed in one patient during two subsequent infusions of the drug. Hydrocortisone and antihistamine drugs were administered. Treatment with PEG-asparaginase was discontinued in one child, who developed dyspnea, nausea, vomiting and face rash during the third dose of the drug. Oncaspar is the valuable drug, which enabled continuation of treatment according to protocol in four out of five children with bad tolerance to routinely used L-asparaginase preparations.

[The assessment of the treatment of viral hepatitis C with interferon alpha in children with leukemias and lymphomas].

Interferon alfa in dose 3-6 MU IM was administered during six months 3 times per week in 30 children with chronic hepatitis C with leukaemias and lymphomas. Chronic hepatitis was diagnosed basing on anti-HCV, RNA HCV by PCR, high activity of ALT and histopathologic examination of the liver. The patients were followed-up for 12 months after IFN therapy. Sustained normalization of ALT levels, together with serum HCV RNA elimination was considered as a complete response and achieved in 33% children. Partial response (ALT normalization without serum HCV RNA elimination) was recorded in 57% patients. Anti HCV antibody was detected in all children during the time of observation.

Kinetics of humoral response in children with acute lymphoblastic leukemia immunized with influenza vaccine in 1993 in Poland.

A total of 49 children with acute lymphoblastic leukemia were immunized with a purified subvirion trivalent influenza vaccine (Wyeth-USA) and monitored for hemagglutination inhibition (HI) and neuraminidase inhibition (NI) antibodies before vaccination, and then three weeks and six months after vaccination. Results for HI antibodies were evaluated as geometric mean titre (GMT), mean fold antibody increase (MFI), protection and response rates and those for NI antibodies as geometric mean titre (GMT) and mean fold antibody increase (MFI). Six months after vaccination GMT for hemagglutinin 1 (H1) was much higher than previous values. GMT for hemagglutinin 3 (H3) and hemagglutinin B (HB) was lower than three weeks after vaccination, but much higher than the original values. In the control group GMT for H1 was on a low level all the time and for H3 and HB it was lower when compared with the original values. The proportion of vaccines to antibodies > or = 40 ranged between 45% and 88%. Six months after vaccination GMT for neuraminidase 1 (N1) increased when compared with the second sampling; for neuraminidase 2 (N2) and neuraminidase B (NB) it was slightly lower. In the control group GMT for all antigens was on a low level all the time. The results point to a significant seroconversion for both components after vaccination when compared with the control group.

[Vaccination against hepatitis B in children with chronic hematologic diseases treated with immunosuppression].

Prevention of hepatitis B infection is an important factor in the successful management of cancer and aplastic anaemia cases. Our result suggested that children with Hodgkin's disease and solid tumors vaccinated during early stage of immunosuppressive therapy are good responders to hepatitis B vaccine. Active immunisation with hepatitis B vaccine (Engerix B), was also effective in children with leukaemia after completing immunosuppressive therapy. Protective levels of antibodies remained 6 years after vaccination. Vaccination according to shortened schedule (0-10-20 days) was not effective in these children. Passive immunisation is indicated in children with chronic neoplastic haematological diseases during immunosuppressive therapy. In 6 children the lack of seroconversion after vaccination was due to immune disorders.

[Vaccination against influenza in children with acute lymphoblastic leukemia].

In November and December 1993, 49 children, ages 4 to 20, suffering from acute lymphoblastic leukemia, were vaccinated against influenza in the Department of Paediatric Haematology and Oncology, Medical Academy in Warsaw. These patients were vaccinated either in the course of maintenance treatment or after treatment. Each dose of Wyeth USA subunit trivalent influenza vaccine contained 15 micrograms of hemagglutinin of strains recommended for that season. The level of antibody production was determined in pre- and post vaccination sera in the group of children with leukemia and the control group. It was determined that in the investigated group, the GMT increased more than four times for hemagglutinins H1N1 and H3N2. A somewhat lower increase was observed in case of hemagglutinin HB. The proportion of subjects protected after vaccination was 35% for hemagglutinin H1N1, 76% for H3N2 and 100% for HB. The response rate was 33% for hemagglutinin H1N1, 47% for H3N2 and 45% for HB. In the control group the proportion of subjects protected and the response rate were very low. The results show the significant immunological efficacy of the vaccine used in the vaccination against influenza in high risk groups.

Peripheral blood leukocytes and lymphocyte subpopulations as determined by flow cytometric measurements in healthy children.

Peripheral venous blood leukocyte subpopulations were determined in 69 healthy children and compared in two age groups: 8 to 12 years and 13 to 17 years, the period of pubescence and intensive growth of the organism. As compared to work of other authors, more leukocyte subpopulations were recognized in the examined large group of children. Whole blood monocyte and total lymphocyte absolute counts decreased with age as well as the absolute counts of T lymphocytes and CD8+ T lymphocytes. Percentage of CD4+ cells and CD4/CD8 ratio were higher in the older age group. Percentage as well as absolute counts of CD19+ CD5+ cells and CD19+ CD10+ cells are given.

[Influenza vaccination of children with hemophilia].

51 haemophilic children aged from 7 to 16 years was vaccinated against influenza in Paediatric Department of Haematology and Oncology in November and December of 1993. Each dose of subunit vaccine manufactured by Wyeth-USA, contained 15 micrograms of each haemagglutinin strains as recommended for the season. The antibody level was studied before and after influenza vaccination in vaccinated and control group as well. 5-time rise of geometric mean antibody titre was found for H1N1 and H3N2 antigens after vaccination of the children group. Slightly lower, 2.7-time rise of GMT was showed for HB haemagglutinin of influenza virus. There was no significant rise of GMT for any among three studied virus haemagglutinin.

[Diagnosis of hemophilia A carrier state using analysis of intergenic and intergenic polymorphism of factor VIII gene].

Carrier state of haemophilia A was assessed in women at risk by using Bcl I polymorphism of factor VIII gene in intron 18, CA repeating sequence polymorphism in intron 13, and intergenic polymorphism at DXS52 locus. Appropriate fragments of DNA in intron 18 and 13 were amplified by PCR. Polymorphism at DXS52 locus was determined by the method of Southern after digesting DNA with endonuclease Taq I and employing F814 and ST14-1 probes. The diagnosis of carrier state of haemophilia A was made in 12 women from 9 families.

[Use of filgrastim (RHU G-CSF) in supplemental treatment of non Hodgkin's lymphoma (NHL) in children].

Clinical application of glycoproteins stimulating the growth (G-CSF), differentiation and activity of the cells of granulocyte line has become a landmark in the treatment of patients with neutropenia. It has been proved that filgrastim (rHU G-CSF) applied after intensive chemotherapy shortens the duration of neutropenia and decreases the frequency of occurrence of infections. The paper discusses the efficiency and tolerance of filgrastim in children treated for NHL. Filgrastim was applied in children with NHL T cell and B cell treated according to BFM 86 protocols. It was given in a dose of 5 micrograms/kg daily i.v. The duration of therapy ranged from 5-20 days. Altogether 47 cycles were performed. 25 cycles were performed in 12 children during granulocytopenia (< 1 x 10(9)/l). The median time of neutropenia recovery, the frequency of severe infection and chemotherapy retardation were significantly lower in the examined than in the control group (p < 0.05). In 6 children 22 courses of filgrastim were given prophylactically after the cycles of intensive chemotherapy of NHL. Only during two courses due to neutropenia the chemotherapy retardation was necessary. Toleration of filgrastim was good. Application of filgrastim right after the intensive chemotherapy in children with NHL has considerably improved persistent realization of treatment programmes. Tolerance of the drug was good.

[Use of rHu GM-CSF (Leucomax) in treatment of neutropenia during intensive chemotherapy for acute lymphoblastic leukemia].

15 children with ALL and chemotherapy related neutropenia were treated with GM-CSF (Leucomax-Sandoz/Schering-Plough) in a dose of 5 micrograms/kg b.w./day during 7-10 days. Altogether 21 cycles were performed. Increase in the number of neutrophils in the majority of cases was observed already after 3 days of GM-CSF administration. The median time of neutropenia recovery was shorter in children treated with GM-CSF than in the control group. The frequency of severe infections was also significantly lower. No serous side effects were observed.

Active immunization of children with leukemias and lymphomas against infection by hepatitis B virus.

Engerix B vaccine was administered to 54 children with leukemias and lymphomas aged from 2 to 15 years. In 36 cases chemotherapy was completely stopped, and 18 cases were receiving maintenance treatment. Engerix B was given at 0, 1, 2 and 6 months in a dose of 20 micrograms to children < 10 years, and 40 micrograms to older patients. The effectiveness of active immunization was demonstrated after complete therapy cessation in 88% of cases. The levels of antibodies determined 1 year after primary vaccination remained high, and in most of the vaccinated children they were > 1000 mIU/mL. In children vaccinated in the course of maintenance treatment the levels of antibodies did not give sufficient protection against infection.

[Results of treating chronic myelogenous leukemia in children].

Retrospective analysis of the results of treatment chronic myeloid (CML) leukemia in 80 children was done. From among 72 children treated by conventional methods the probability of 10 years survival had 17%. Statistically significant better results was obtained in the group of children with adult than juvenile type of CML (23 and 8%). Allogenic bone marrow transplantation was performed in six children, two children were treated with interferon. New approaches of treatment may offer the better chance for constitution of normal bone marrow function.

[Interferon alfa in treatment of chronic hepatitis B in children with leukemias and lymphomas].

The course of the hepatitis B virus infection (HBV) in the majority of children with cancer who have undergone intensive chemotherapy is characterized by scanty symptoms and often leads to chronic hepatitis and prolonged carrier state. Twenty three children with chronic hepatitis B virus (HBV) were treated with interferon alfa after the completion of chemotherapy of leukemias and lymphomas. In the majority of cases a disproportion between symptomless clinical course of the hepatitis B virus and the rate of its active replication was observed. Presence of HBsAg and high activity of polymerase DNA HBV (pDNA) were found in all children. In the course of a 6-month treatment with interferon in a dose of 3 mil IU/m2 of the body surface a gradual decrease of pDNA activity was observed. A statistically significant difference of mean values of pDNA before and right after the treatment was determined (p < 0.05). Tests performed 3 months after the completion of the therapy showed a repeated increase of pDNA activity. Boundary values of pDNA were observed in 5 children. Complete elimination of HBs and HBe antigens has not been achieved in any of the children. While analyzing the obtained results it has to be taken into consideration that there were numerous unfavourable prognostic factors determined in the studied group of children such as: coexisting malignant disease, undergone intensive chemotherapy, long period which has passed from the moment of HBV infection until the administration of the treatment with interferon, high activity of pDNA, scanty symptoms of inflammation process. Coexisting HCV infection in 12 children, could also have a undesirable effect upon the treatment results.(ABSTRACT TRUNCATED AT 250 WORDS)

[Diagnosis and treatment of central venous catheter related infections].

The central venous catheter related infections are relatively common in clinical practice. These infections are particularly frequent in the patients with long-term central venous catheter, such as the oncology patients or patients with short intestine syndrome. The diagnosis of infection should be based on repeated positive blood cultures. In some cases the infection responds well to antibiotic treatment. Unless there are indications for removal of catheter antibiotic therapy should be applied for 1-2 weeks.

Cytomegalovirus infection in children with blood diseases.

The study included 68 children aged from 1 to 16 years treated for acute leukemias and bone marrow aplasia. Cytomegalovirus antigen (CMV) was detected by immunofluorescence in urinary sediment cells and in cell cultures after their inoculation with CMV. Besides, the activity of IgG and IgM classes of antibodies against CMV was determined. Presence of one or more markers of CMV infection was demonstrated in 31 children, i.e., 45.5%. In eight children (11.7%) clinical manifestation of CMV infection were demonstrable with fever, hepatitis, pneumonia, rash. In all the children who completed the treatment with hyperimmune globulin, regression of clinical symptoms and signs of CMV infection with the elimination of virus antigen from urine was achieved.
