Density distribution, cytomorphologic features and immunologic characteristics of ovarian and endometrial clear cell carcinomas.

Three subpopulations of cancer cells with different morphologic features were separated by density gradient centrifugation of two ascitic fluids and one cystic fluid from one patient with ovarian clear cell carcinoma and one with endometrial clear cell carcinoma. Immunophenotypic analyses of isolated fractions using polyclonal and monoclonal antibodies against ovarian carcinoma-associated antigens revealed significant immunologic heterogeneity among the tumor cells. The identical histopathologic structures of the ovarian and endometrial clear cell carcinomas and the similar distribution and immunologic reactivity of the cell types isolated from the ascitic and cystic fluids confirmed the common histogenesis of both cancers. These findings suggest that the conventional cytologic diagnosis of clear cell carcinomas could be supplemented by immunofluorescent staining. Density gradient centrifugation appeared to be a useful method for the separation of mesothelial cells.

Density distribution and antigenic characterization of cell subpopulations isolated from cystic fluids of ovarian serous neoplasms.

The immunological reactivity of serous ovarian tumor cells was evaluated, taking into account their density and morphological features. Discontinuous density gradient centrifugation was applied to fractionate cell subpopulations from cystic fluids of patients with ovarian serous carcinomas, cystadenomas and benign serous cysts. For phenotypic characterization of tumor cell subpopulations the immune sera against perchloric acid extracts of ovarian serous (anti-PCA-CaOs) and mucinous (anti-PCA-CaOm) carcinomas were used. The expression of carcinoembryonic antigen (CEA) and nonspecific cross-reacting antigen (NCA) on tumor cell fractions was also checked. Some relationship between immunological reactivity, cell morphology and cell density was found; however, individual patient-to-patient variations in cellular composition and antigenic expression were also observed. The presence of ovarian serous carcinoma-associated antigen (CaOs-Ag) was related mainly to frankly malignant cells. Neither CEA nor NCA were constant and characteristic markers for serous ovarian neoplasms. These neoplasms were unreactive with anti-PCA-CaOm serum. Our results indicate the possibility of correlation between morphological features, density distribution and immunological phenotypes of ovarian tumor cell populations.

[Shoulder girdle and shoulder joint tuberculosis].

From 1955 to 1980 inclusive, 50 patients with tuberculous omarthritis and 11 with specific shoulder girdle disorders (nine sternoclavicular joints, one isolated clavicular shaft focus, as well as one acromioclavicular joint focus) were treated at this clinic with a combined tuberculostatic and surgical therapy. The youngest patient was nine years old, and the oldest patient was 89 years old (36 females, 25 males). The patients (average age 49.4 years) comprised 11 foreign workers. From the beginning of the symptoms up to diagnosis, an average of 1.4 years elapsed. 34 patients (56%) had already been treated earlier for tuberculosis, and 12 (20%) were suffering at the same time from an active specific infection of other localisation. Independently of abscesses and fistulae (35 patients = 57%) as well as other simultaneous tuberculosis, the one-hour value of the erythrocyte sedimentation rate was in the normal range, or it was only marginally to slightly raised (under 20 mm according to W.). In 28 patients (46%), arthrodeses were performed in the shoulder joint, whereas the focus was cleared in the remaining patients. One patient (89 years old) died of senile debility. All complications (16 reactivations, two dislocations of the bone chip implantations, one fail joint) healed without complications after secondary operations. In the follow-up examination (on average, after 3.1 years), we diagnosed one recurrence (sternoclavicular joint process). Of the 45 patients (74%) followed up, 36 (80%) had started working again on average 12 months after the end of hospital treatment, or had resumed their former occupation (pensioners, housewives). Three (7%) of those followed up needed a different occupation or were given a new job within their firm.

Infant mortality in Newark, New Jersey. A study of sociodemographic and medical factors.

Newark, a metropolitan industrial town, experienced the highest infant mortality of any major city in the United States in the 1960s and early 1970s. Between 1970 and 1973, however, infant mortality among non-whites in this city declined strikingly. This decline could not be directly related to declines (a) in birth rates, (b) in the proportions of babies of low birth weight, (c) in the proportions of babies born to mothers in unfavorable age groups, (d) in the general fertility rates, or (e) in the illegitimacy rates. The decline may have been related (a) to the removal from childbearing cohorts of the group of females in the population--as yet undefined--whose babies would have been at high risk of infant mortality, (b) to the falling birth rate, (d) to better postnatal care--or to all of these factors. The study data suggest a multifactorial basis for the precipitous decline and also suggest that further major reductions in infant mortality among both nonwhites and whites will require better definition of the causes of low birth weight.

[My fingers itch, but my hands are bound. An exploratory psychosomatic study of patients with dyshidrosis of the hands (cheiropompholyx)].

The purpose of a pilot study with 20 patients, suffering from dyshidrotic exzema of the hands, was to investigate the relevance of a psychosomatic approach for its etiology and therapy. A set of psychological and allergological tests revealed the following results. - 10 patients suffered from a contact allergy (epicutaneous test), 6 of them were allergic to nickel - the patients were less aggressive and more permissive (Freiburger Personality Inventory; FPI) as compared to a standard population. They reported more somatic complaints (HHM) as healthy subjects. - the results of a semistructured interview support the hypothesis, that the dyshidrotic exzema often has an expressive function: the patients present their dependency conflict in a symbolic manner by means of their hands. They try to take their life in their own hands; autonomous actions however are hampered by their own dependency needs. The development of the dyshidrotic symptoms corresponds to the autonomy, which has been achieved with regard to the individual conflict of ambivalence. According to our findings the contact with nickel is not predominant, neither causative nor triggering, factor. Following the psychosomatic concept of Engel and Schmale, the authors discuss the pathogenesis of dyshidrosis with the meaning of a complication of (pregenital) conversion, with the hands as the part affected.

[Premature rupture of the fetal membranes in term pregnancies].

The significance of premature rupture of membranes (PROM) prior to 24 hours before delivery in term pregnancies has been retrospectively studied by comparing 202 deliveries after PROM with 202 controls matched by gestational age and parity but presenting intact membranes until less than 24 hours before delivery. The PROM group was shown to be characterized by a statistically significant accumulation of nulliparous women (80 per cent vs. 50 per cent), occiput posterior presentations (18 per cent vs. 7 per cent), caesarean sections (28 per cent vs. 8 per cent), fever intra partum (8,4 per cent vs. 2,5 per cent), Apgar scores below 7 one minute after birth (15 per cent vs. 6 per cent), and umbilical artery acidosis below pH 7,20 (28 per cent vs. 17 per cent). These results indicate term pregnancies presenting PROM as compared to matched controls to be charged by a threefold risk concerning dystocia and infection, and a doubled risk for neonatal depression, respectively.

[Sinusoidal fetal heart rate pattern indicating severe anemia due to fetomaternal transfusion (author's transl)].

A case of feto-maternal transfusion with an estimated loss of 125 ml blood is presented. After an uncomplicated pregnancy nothing but a sinusoidal fetal heart rate pointed to the severe illness of the fetus as shown by a hemoglobin level of 48 g/l. Immediate cesarean section was performed on the base of this typical cardiotocographic pattern only. Consequently the significance of sinusoidal fetal heart rate indicating fetal suffering is discussed.

[The correlation between blood pressure values and personality score in the FPI-R in a non-clinical sample].

Personality traits of persons with high blood pressure were examined again. However, in contrast to most studies, it was decided to employ non-clinical samples of hypertensives and controls who were homogeneous for age and gender and also for actual life-style. This was done with draftees in the frame of a blood donation when blood pressure was recorded by routine. In addition, subjects completed the Freiburg Personality Inventory (FPI-R). Data of 390 persons were suitable for evaluation. Groups with different blood pressure levels did not differ significantly in any of the scales with the exception of higher emotionality scores in the high blood pressure group. This seems best explicable as a chance finding in the frame of multiple comparisons. Neither emotionality nor other personality traits showed any correlation with blood pressure values across the whole sample. Overweight, however, correlated repeatedly with the scale scores and should thus be taken into account in further personality research with hypertensives.

[Coping and life satisfaction in patients with Hodgkin's disease in remission. A contribution to the question of adaptive aspects of coping processes].

Widening the focus from survival time to the "quality of survival" this cross-sectional study was designed to find correlations between coping strategies, quality of life, sociodemographic and medical data. 61 patients (31 women, 30 men; mean age 41 [22-74] with Hodgkin's disease receiving no treatment since 1 month to 16 years (mean 5 years) were assessed for coping and quality of life. Two standardized questionnaires were used: "Freiburg Questionnaire of Coping with Chronic Illness" (FKV 102) and "Questions for Life Satisfaction" (FLZ). Coping strategies were evaluated. The most used strategies were compliance strategies and trust in doctors. Some coping strategies correlated significantly with age, sex, marital status, education, and medical data as B symptoms and occurrence of recidivs. The scores for life satisfaction were absolutely high in comparison with postoesophagectomy patients and healthy controls from other studies. Life satisfaction was correlated with age, B symptoms and type of therapy. These correlations and correlations between FKV and FLZ were discussed and basic problems were reflected.
