Metabolic and nutritional sequelae in the non-steroid treated head injury patient.

Energy production, substrate oxidation, serum protein levels, and weight change were studied in 16 non-steroid treated patients with severe head injury. Patients were evaluated during an average of 31.3 days from hospital admission to discharge. The mean measured energy expenditure (MEE) was 1.40 +/- 0.5 times predicted energy expenditure. Caloric balance [calories received = calories expended] was achieved by the 2nd week. Despite caloric balance and the administration of at least 1.5 g of protein per kg of body weight per day, the mean nitrogen balance was negative. There was a positive nitrogen balance in only 2 patients. These patients received a mean of 1.43 times the MEE in total kilocalories and 2.3 g of protein per kg of body weight. Fat and protein oxidation exceeded protein and fat administration for 3 weeks postinjury. Albumin levels dropped from a mean of 3.09 +/- 0.2 on admission to 1.98 +/- 0.4 within 2 weeks. The initial retinol binding protein levels were within the normal range, and the levels increased over time. There was marked weight loss (mean, 15.6 +/- 5.9 lb). Head injury induces a profound traumatic response identified by increased energy expenditure, a negative nitrogen balance, weight loss, hypoalbuminemia, and altered substrate oxidation. This response seems to be caused by the head injury alone and is not due to the administration of corticosteroids.

A procedure for splitting human teeth to obtain intact pulp tissue, enamel and dentin.

Calcified human permanent and primary teeth are often split to obtain pulp tissue for histochemical studies as well as unaltered dentin and surfaces for scanning electron microscopy. Various procedures have been used to cleave teeth, with different degrees of consistency. For rapid and fairly consistent splitting, a vise to the jaws of which triangular metal files have been welded has been found useful. The apices of the files were ground by an electric drill to the shape of typical teeth. Teeth to be split were grooved on their opposing external surfaces and were then cracked open between the file blades upon application of pressure by the vise. Teeth usually split lengthwise, exposing the entire pulp organ in one section and an empty pulp chamber-root canal in the other. This facilitated rapid penetration of fixative into pulp, and easier removal of pulp tissue in toto, as well as providing fresh enamel and dentin surfaces suitable for scanning electron microscopy.

A modified technique for perfusing blood vessels in pulps of extracted teeth.

The distribution of blood vessels in pulp tissue can be studied through the use of perfusion techniques on freshly extracted teeth. A procedure for the perfusion of extracted human and monkey teeth in vitro is described. As the process is not histologic in nature, a method of identifying the observed vessels is suggested.

Computed tomography in the evaluation of sacroiliac arthritis.

Computed tomography (CT) in the evaluation of the sacroiliac joints was compared with conventional radiography in a double-blind study. CT did not increase the probability of detecting sacroiliitis. Because only a few patients were evaluated near the onset of their disease, the sensitivity of CT as compared with that of routine radiography in detecting early morphological changes remains undetermined. An inexperienced observer may find it easier to evaluate CT scans than conventional radiographs of the sacroiliac joint; however, with optimal images, an experienced radiologist does not need CT to evaluate the sacroiliac joint in clinical practice.

Visualization of the longitudinal fold of the duodenum by computed tomography.

The longitudinal fold of the duodenum can be visualized by computed tomography (CT). Visualization of the fold helps localize the hepatopancreatic duct entrance into the duodenum. Many pathological processes of the head of the pancreas such as pancreatitis or malignancy can produce swelling involving the longitudinal fold, which can be visualized by CT.

The posterior pararenal space: an escape route for retrocrural masses.

In two patients with retrocrural tumors, dissection of the tumors selectively into the posterior pararenal space was observed by computed tomography. Cadaver studies confirmed the presence of potential communications between the retrocrural space and the extrapleural space of the thorax or the posterior pararneal space of the abdomen.
