Interstitial irradiation of carcinoma of the prostate with 125iodine and simultaneous extraperitoneal pelvic lymphadenectomy in 32 patients: trials, tribulations and possible triumphs.

A series of 32 patients with carcinoma of the prostate who underwent 125iodine interstitial irradiation to the prostate and simultaneous extraperitoneal pelvic lymphadenectomy was reviewed. The patients were segregated into 3 groups: group 1-patients with early localized low grade disease, group 2-patients with advanced localized high grade disease and group 3-patients with early localized high grade disease and those with advanced localized low grade disease. Owing to the high degree of predictability of the absence or presence of pelvic lymph node metastasis in patients in groups 1 and 2 and the morbidity associated with superficial pelvic lymphadenectomy we do not advocate this procedure in these patients. However, we advocate pelvic lymphadenectomy in patients in group 3 because of the unpredictability of lymph node metastasis. Furthermore, our data indicate that 1) the morbidity of the combined procedures is relatively low, with no patients becoming incontinent and only 12.5 per cent becoming impotent, 2) 125iodine interstitial irradiation of the prostate after transurethral prostatectomy is safer in terms of postoperative complications than is radical prostatectomy and 3) while superficial pelvic lymphadenectomy is not an innocuous procedure it generally is safe and, at times, an important diagnostic staging procedure.

Screening tests for detection of bladder cancer.

Eighty-one individuals had urine cytologic studies, urinary carcinoembryonic antigen (CEA) determinations, and urinary immunoglobulin levels performed to evaluate various screening tests for the diagnosis of bladder cancer. Urinary cytologic studies detect the presence of bladder malignancy, and while false positive tests were troublesome, remain the primary screening test for bladder cancer. Urinary CEA was of little value in detecting vesical malignancy. Urinary immunoglobulins, particularly IGG and IGA, were significantly elevated in the presence of bladder cancer.

Use of nuclear Overhauser effect in the study of peptides and proteins.

The nuclear Overhauser effect (NOE) leads to changes in the intensity of signal(s) of a set of nuclei as a function of their respective distances. The use of NOE allows to obtain structural informations on peptides and proteins in solution as well as the study of interactions between small ligands and biomolecules. In this review, aspects of the basic theory of the NOE will be presented and the more recent applications of homonuclear and heteronuclear NOE's in biomolecules will be surveyed. Typical examples will be illustrated and limitations of the method will be discussed.

Inhibition of mosaic virus of Vigna sinensis Endl. with four chemicals.

Four chemicals--gibberellic acid, indoleacetic acid, 2,4-6 and thiouracil were tested for their inhibitory effect against a virus disease of Vigna sinensis (cowpea). All the four chemicals inhibited the virus to a considerable extent in in vitro studies. Pre and post-inoculation treatments were equally effective in inhibiting the virus, viz. local lesion formation on Chenopodium amaranticolor leaves. The level of inhibition subsequently decreased with lower concentration of the chemicals. Seed soaking in the chemicals controlled the virus infection.

Properties of the two common electrophoretic variants of phosphoglucomutase in Drosophila melanogaster.

Phosphoglucomutase (PGM) of adult stage in Drosophilia melanogaster has been characterized by gel filtration, ion-exchange chromatography, and isoelectric focusing. The two common electrophoretic variants, PGMA and PGMB, differ with respect to their kinetic and stability parameters. PGMA is more thermostable than PGMB but shows the same pH optimum, equal dependence on Mg2+, and identical molecular weight. There is no significant kinetic difference between the two allozymes at the optimum pH values, but at pH 6.0 the Km value for glucose-1,6-diphosphate of PGMB is significantly higher than that of PGMA. This difference might explain the observed selective advantage of the PgmA allele in population studies.

Adjuvant immunotherapy with bacillus Calmette-Guérin in prostatic cancer.

Ninety-two patients with histologically proved prostatic cancer were studied. Forty-six patients were treated with bacillus Calmette-Guérin (BCG) adjuvant immunotherapy, and 46 other patients, matched for stage and hormone therapy, served as controls. Survival from the time of histologic diagnosis was sixteen and one-half months longer (thirty-seven and one-half versus twenty-one months) in the BCG-treated patients. There was no mortality and only minimal morbidity. Changes in some immunologic parameters (white blood cell count, skin tests) suggest an immune response.

Use of albumin gradients for X and Y sperm separation and clinical experience with male sex preselection.

Semen samples obtained from 18 normal males and 37 husbands requesting male child preselection were separated on concentration gradients of human serum albumin. Separated semen obtained from the husbands was then used for artificial homologous insemination (AIH). A significant increase in the sperm motility, progressive drive, and percentage of Y-bearing sperm along with a decrease in the total sperm count and percentage of abnormal forms were observed in separated specimens. Fathers of three or more female children had a slightly smaller but statistically significant percentage of Y-bearing sperm in their semen samples than did normal males. Ten conceptions were achieved with separated semen. Seven pregnancies terminated in normal deliveries of five male and two female infants, one ended in a spontaneous abortion of a male fetus, and two patients are still expecting. The ratio of male to female conceptions in this small study parallels the ratio of Y to X sperm in the final specimen used for AIH.

Artificial insemination homologous with oligospermic semen separated on albumin columns.

Semen samples obtained from 27 infertile men were separated on human serum albumin (HSA) columns prior to artificial homologous insemination (AIH). The columns contained either a single 7.5% HSA layer or two 17.5% and 7.5% HSA layers. Separated specimens were free of seminal debris, had significantly improved motility and progressive drive, and had a decreased percentage of abnormal forms. The total sperm count was also significantly decreased after separation, especially with the two-layer technique. No conceptions occurred in 21 couples when two-layer separation was used, but four pregnancies in 12 couples resulted from AIH with semen separated on the single-layer column. The mean total count of motile sperm obtained for AIH was 5 million with the two-layer and 31 million with the one-layer technique.

Conception following insemination with a freeze-preserved retrograde ejaculate.

Retrograde ejaculation, an infrequent cause of male infertility, may be the sequela of prostate or bladder neck surgery or the result of interruption in the sympathetic innervation to the bladder neck. The diagnosis is established by history and examination of the urine. In infertile couples artificial insemination homologous (AIH) using retrograde ejaculate recovered from the bladder has been successfully accomplished, but conception has occurred only in isolated cases. A literature review of the subject is presented. The technique of semen recovery from the bladder is time-consuming and uncomfortable to the patient, while the quality of the semen recovered is variable and the risk of iatrogenic urinary tract infection is increased. To avoid the above problems, and to have sperm available for AIH at the time of induced ovulation in an anovulatory wife, we were able to freeze-preserve ejaculate recovered from the bladder for future use. A case of conception following AIH with freeze-preserved retrograde ejaculate is presented.
