[Continuous Y-V-plasty].

Scar tissue contraction after healing of a burn may be a serious problem. The formation of cord-like scars in the region of a joint may lead to limitation of movement and deformity. Pressure bandages may reduce scarring but can not always prevent it and operation may be necessary. Operation may involve division of the contracted scars and skin grafting or re-arrangement of available skin and changing the line of the scar by means of Z-plasties. However, the blood circulation in the scarred flaps of the Z-plasties may be inadequate and necrosis of the tips of the flaps may occur. This complication can be avoided by the use of Y-V plasties where, in contrast to the Z-plasty the flaps are not separated from the underlying tissue. In the Y-V technique the cord-like scar is released by several transverse Y incisions. The 'tongue' formed by the upper part of each Y is advanced into the stem, thus lengthening the scar. This technique has been so successful that it has completely superseded the Z-plasty in the treatment of contracted burn scars in our practice.

Plastic surgical management of children with Down's syndrome: indications and results.

The face of a child with Down's syndrome is stigmatised by the large tongue, open mouth, saddle nose and epicanthal folds. Operative reduction of macroglossia facilitates nose-breathing, comprehensible speech and eating. Raising the nasal bridge with Cialit-preserved homograft cartilage obliterates the epicanthal folds and conceals the oblique slant of the eyelids. The child should be operated on from the third year onwards.

[Are there still indications for subcutaneous mastectomy?].

There are many indications for subcutaneous mastectomy with immediate reconstruction using silicone implants because of the relatively simple technique. The risk of malignant change in simple mastopathy is minimal. However, the inadequacy of an operative technique that leaves behind unknown quantities of breast tissue and the frequently bad cosmetic results make all indications appear doubtful. For these reasons subcutaneous mastectomy should be abandoned and simple mastectomy with immediate reconstruction is to be preferred for the following conditions: (1) Diffuse, painful, simple mastopathy that has been repeatedly biopsied; (2) Non-invasive carcinoma, either intra-duct or in situ.

[Advanced basal cell carcinoma].

306 patients underwent extensive plastic surgery in the last five years for advanced basal cell carcinoma. In 12 patients either due to delay in presentation or due to inadequate treatment squamous cell carcinoma supervened. In all 12 cases reconstruction was very limited or impossible. Prostheses were used to mask those defects which could not be reconstructed. The infiltrative growth of these squamous cell carcinomas towards the brain could not be controlled. Eventually all the patients developed wide-spread systemic metastases.

[Experiences with plastic surgery in mongoloid children].

The aim of plastic surgery in children with down-syndrome is the improvement of speech (by correction of macroglossia) as well as improvement of cosmetic appearance (by correction of saddle nose deformity, epicanthic folds receding chin and deformity of ears). Altering the typical mongoloid physiognomy facilitates the integration of these children into the community.

[Plastic surgery in mongoloid children].

The aim of plastic surgery in children with Down-syndrome is the improvement of speech as well as improvement of cosmetic appearance. Altering the typical mongoloid physiognomy facilitates the integration of these children into the community. In our Department during the past 6 months plastic surgery was performed in 27 children for the correction of macroglossia, saddle nose deformity, epicanthic folds, receding chin, fatty neck and deformity of the ears. Macroglossia was present in every case, the extent of further surgery varies according to the clinical deformity. It is essential to discuss the implications of surgery in these children with the parents in order to avoid later disappointment.

[Andromastectomy].

Andromastectomy, that is to say the removal of the male mammary glands, should prevent the development of Gynaecomastia which is both painful and deforming and may occur subsequent to certain illnesses or hormone treatment.

PVP breast implants after two years: initial results of a prospective study.

A prospective study of a new mammary prosthesis with PVP-based gel as filling material was carried out. Scheduled follow-ups were planned at 3, 6, 12, and 24 months post-implantation to assess all complications, Baker scores, and the patient's, and the physician's global assessment of each implant. PVP-filled implants were used in 95 breasts for augmentation (60%) or reconstruction (40%). Sixty-nine percent of all patients underwent a primary procedure, 19% had a history of severe capsular fibrosis. During follow-up, a hematoma was observed in 2% and a seroma in 5%. Leakage occurred in 3% (one iatrogenic and two cases of unknown reason), a Baker 3 in 6% (12 months). No volume increase of the implants occurred. The probability that a patient would be complication-free at 24 months was 0.86. Physician's and patient's satisfaction rating after 12 months remained high without any time effect (physician very good/good 63%, patient 75%). According to our current experiences, the PVP-filled implants are a remarkable alternative with an improved viscosity and enhanced x-ray transmission, compared to saline filled implants.

Successful surgery of multiple recurrent basal cell carcinomas guided by photodynamic diagnosis.

We describe a 56-year-old Caucasian man with history of multiple regressed basal cell carcinomas. During the last 20 years approximately 200 histologically proven basal cell carcinomas preferentially localized on the face were surgically treated. Several large skin grafts were necessary to cover the extensive tissue defects on the face and scalp. Although all excised tissues were histologically proven to be basal cell carcinomas with tumor-free margins, new tumors developed in proximity to the skin graft margins. The dissemination of the new tumors made it difficult to perform additional invasive operation procedures without influencing the cosmetic result. Thus, we used photodynamic diagnosis to improve detection and demarcation of the neoplastic tissues. This procedure facilitated surgical planning and enabled primary in toto excisions. Surgical trauma and a number of interventions were thus minimized with the consequence of improved cosmetic and functional results.
