Focal acral hyperkeratosis.

We report fifteen cases showing focal acral hyperkeratosis. They were clinically identical to acrokeratoelastoidosis (AKE) but lacked the elastorrhexis which is a feature of AKE. The name focal acral hyperkeratosis is proposed for this condition which differs histologically from AKE.

Kwashiorkor: an electron histochemical study of the hair shaft.

Previous studies on hair in kwashiorkor have been equivocal regarding the normal or abnormal incorporation of amino acids into the hair shaft. In this study we have applied a sensitive electron histochemical technique to transverse sections of scalp hair from children with severe kwashiorkor in order to study its ultrastructure and the incorporation of sulphur-containing amino acids (cystine). We have found no evidence to support the idea that a protein deficient diet in the weaning or post-weaning period directly affects the distribution and relative amounts of sulphur-containing amino acids in the hair shaft in this disease.

Botryomycosis: a complication of extensive follicular mucinosis.

A case of extensive follicular mucinosis, without evidence of lymphoma and present for 9 years, was complicated by cutaneous botryomycosis which initially resembled sporotrichosis. Scanty Staphylococcus aureus was cultured with difficulty. No immunodeficiency could be demonstrated, and the importance of a local factor (namely follicular mucinosis) in the production of the characteristic histological reaction of botryomycosis is emphasized.

Azathioprine plus prednisone in treatment of pemphigoid.

Twenty-five patients taking part in a controlled trial to compare azathioprine plus prednisone with prednisone alone in the treatment of pemphigoid were followed up for three years. Results showed that the addition of azathioprine 2.5 mg/kg body weight daily reduced the total maintenance dose of prednisone needed by about 45%, with no increase in serious side effects or mortality. The suggestion that azathioprine might increase the risk of disseminated malignancy in elderly patients was not supported. We conclude that in future trials the combination of azathioprine with prednisone should be used as the standard treatment for comparison.

Axillary hyperhidrosis treated with alcoholic solution of aluminium chloride hexahydrate.

Sixty-five patients with axillary hyperhidrosis took part in a trial of treatment with a solution of 20% aluminium chloride hexahydrate in absolute alcohol, applied topically each night for a week and then whenever the patient thought it necessary. Excellent control of sweating was achieved in 64 patients, and occlusion of the area was found to be unnecessary. No troublesome side effects were reported. The results of this study indicate that 20% aluminium chloride hexahydrate in absolute alcohol is the treatment of first choice for patients with axillary hyperhidrosis.
