Mortality and incidence rates for pancreatic cancer in the United States were examined by various demographic characteristics. Disease rates have continued to increase over time but at a much slower pace than in earlier years. Most recently available rates for blacks were significantly higher than for whites and rates for males of each race were higher than for females. Income and education levels had little influence on incidence rates among either blacks or whites. Incidence rates were not significantly higher in urban as compared with rural areas of Iowa and Colorado. The two-year survival rate for pancreatic cancer was about 5% in recent years and did not vary significantly by race or sex. Smoking and diabetes, the two risk factors most consistently associated with the pancreatic cancer, explain only a small proportion of the disease. Much epidemiologic work remains to be done.
Demographic and pathologic information on over 1,000 newly diagnosed patients with nasopharyngeal carcinoma was obtained from population-based registries in the United States. Age-adjusted incidence rates were similar for whites and blacks and both were significantly lower than for Chinese Americans. Age-related differences in cell type were observed in white NPC patients, lymphoepithelial carcinomas having a younger age distribution than either squamous-cell or transitional-cell carcinomas. Mortality rates for nasopharyngeal cancer were substantially lower than incidence rates for nasopharyngeal carcinoma, but both indices revealed a minor peak in rates among teenaged whites and blacks. The five-year survival rate for nasopharyngeal carcinoma was less than 25% and has not changed in recent years. Prognosis was better for females and for young patients. Despite the difficulties in obtaining uniform pathologic classification in such a large study, the interrelationship between pathologic subtype of nasopharyngeal carcinoma and demographic features emphasizes the need for adherence to a more uniform histologic classification.
Antibody titres to EBV-associated antigens in Chinese NPC patients were analysed according to length of survival after diagnosis and to disease stage. Geometric mean titres of ADLC antibody were highest in the long-term survivors, whereas VCA, EA and EBNA antibody titres showed an inverse relationship to survival. High VCA, EA and EBNA titres were less frequent, and high ADLC titres were more frequent in long-term survivors than in intermediate or short-term survivors. The association of geometric mean titres of EBV antibodies with prognosis could not be entirely explained by stage of disease. A functional role for the ADLC antibody is suggested by the association of a high ADLC antibody titre with a good prognosis regardless of stage of disease.

One hundred and eighty-five patients with untreated Hodgkin's disease (HD) comprising 86% of all new cases diagnosed in Denmark over a 2-year period, were individually matched with healthy controls of the same age, sex, and social class. In comparison to controls, HD patients showed significantly elevated mean antibody titres to Epstein-Barr viral capsid antierences in mean titres were found for adenovirus common antigen. Subdivision of the patients by age, sex, social class, HL-A antigens, stage of disease and histology did not alter this pattern, except that significant case-control differences in EB-VCA titres could be demonstrated only for the nodular sclerosis and lymphocyte predominance subgroups. After 1 year of treatment, a significant rise in EB-VCA mean titre had taken place. Splenectomy seemed to promote this titre elevation. Neither the initial titres to EB-VCA and EBV-EA nor changes in titres over time were related to prognosis. The results of our study, which are more representative of a population of HD patients and controls than previously reported studies, confirm that the reported relationship between EBV and HD exists, but that the elevated EBV titres are probably not of etiologic significance in most HD patients.
The American Burkitt Lymphoma Registry (ABLR) investigates the cause of Burkitt's lymphoma through comparative studies involving endemic and nonendemic regions. In 3 years 114 cases have been confirmed. Diagnosis was based on evaluation of pathologic materialy submitted to ABLR pathologists. Similarities between American and African patients included abrupt clinical presentation with involvement of the gastrointestinal tract, jaw, gonads, and central nervous system. Response to high-dose cyclophosphamide and an apparent high cure rate distinguish American Burkitt's lymphoma from poorly differentiated lymphocytic lymphoma (lymphosarcoma) of childhood. Time-space clustering and absence of cases in high-altitude regions are seen in American and African patients; young American patients resemble Africans in a predominance of men and high Epstein-Barr virus titers. American patients have bone-marrow and peripheral lymph-node involvement more frequently and are less likely to have Epstein-Barr virus genome in their tumors; most are white. Identification of specific causal factors requires the study of more cases and better pretreatment material for pathologic review.
Although elevated antibody levels to the Epstein-Barr virus (EBV) have been reported in a number of lympho-proliferative neoplasms, it has not been possible to determine whether these antibodies were the result of a specific response to an oncogenic agent (EBV), whether they were a non-specific humoral compensation for depressed cell-mediated immunity (CMI), or whether a different mechanism was responsible. We have previously shown in a group of lymphoma patients that depressed cellular immunity to a number of standard antigens (Candida, SKSD, etc.) is not associated with an increase in antibody to EBV. In this study, we tried to compare CMI to possible EBV and lymphoid cell line antigens with humoral antibody to EBV. The two basis CMI assays utilized were lymphocyte cytotoxicity (LC) and skin testing (ST) for delayed hypersensitivity. In the LC assay, an EBV-containing cell line (F265) was used as the target. Reactivity against F265 was stronger in normal individuals than in cancer patients, suggesting a relationship to general cellular immune competence. ST studies showed that membrane extracts from lymphoid cell lines derived from patients with Burkitt's lymphoma and nasopharyngeal carcinoma (NPC) were more likely to elicit a delayed hypersensitivity in lymphoma and NPC patients than cell lines derived from normal individuals. Patients with ST reactivity against the membrane preparations from the tumour-derived cell lines were as likely to have elevated EBV antibodies as patients without such reactivity. The data strongly indicated that the elevated EBV titres in lymphoma patients are not related to a specific or non-specific depression of CMI.
