Serum vitamin A and E levels in patients with lichen sclerosus and carcinoma of the vulva--effect of oral etretinate treatment.

Serum vitamin A and E concentrations were determined in six patients with vulvar epithelial carcinoma and in 25 patients with lichen sclerosus and atrophicus (LSA) of the vulva. No difference could be found in these levels between the two groups. LSA patients received oral etretinate for two months at least; improvement was observed in 93% of the patients. Etretinate therapy had no effect on the serum levels of vitamins A or E.

Clonidine in the treatment of hypertension during pregnancy.

Eighty two hypertensive pregnant patients were treated with clonidine during the last trimester of pregnancy. Clonidine was effective hypotensive agent in all groups treated; in essential hypertension (N 31), in mild pre-eclampsia (N 7), in severe pre-eclampsia (N 19) and in superimposed pre-eclampsia (N 24). The drug was well tolerated by the mothers. One fetus died in utero in severe pre-eclampsia group for hypoxia and another infant died at the age of 1 day for sepsis. If antihypertensive treatment is needed during pregnancy, clonidine is one choice.

The use of radiolabelled monoclonal antibodies to human milk fat globule membrane antigens in antibody-guided tumour imaging, and administration of therapeutic dose of labelled antibody in wide-spread ovarian carcinoma. A preliminary report.

Monoclonal antibodies generated against human milk fat globule membrane antigens were used in antibody-guided tumour imaging and palliative therapy in a case of wide-spread ovarian carcinoma. Antibody III H 2, which previously has been shown to react with 100% of ovarian cystadenocarcinomas showed a strong reactivity with tissue sections obtained from the primary and metastatic tumours of the patient. Immunoglobulins were purified from mouse ascitic fluid containing III H 2 and labelled with 123I or 131I with the iodogen method. 80 MBq (2 mCi) of 123I-labelled antibody was given intraperitoneally in 500 ml of PBS and the uptake of radiolabel was followed daily with emission tomography. Radiolabel was mainly located in the peritoneal cavity; only a very low activity was seen in the thyroid gland and urinary bladder. A therapeutic dose consisting of 600 MBq (15 mCi) of 131I-labelled antibody was followed nine days later and the localization of the antibody was followed.

Vitamin A and vitamin E in umbilical artery and vein.

Vitamin A and vitamin E concentrations were measured in 27 umbilical arterial and venous plasma samples obtained from 24 singleton pregnancies and one triplet pregnancy. The levels of both vitamins were similar in arterial and venous plasma (mean +/- S.D. vitamin A 209 +/- 41 micrograms/l and 220 +/- 51 micrograms/l; vitamin E: 2.31 +/- 0.74 mg/l and 2.31 +/- 0.76 mg/l, respectively), but individual differences were observed in some cases. A significant relationship (p less than 0.001) existed between the arterial and venous levels of both vitamins. However, when arterial vitamin A levels were plotted against venous levels, the slope of the regression line (0.61) was significantly (p less than 0.01) different from unity, which suggests that the placental transfer or fetal storage has an effect on the arterial-venous ration of this vitamin. The vitamin A values from the triplet pregnancy were quite low, but comparable with each other.

Prolapse of the vagina after hysterectomy.

Twenty-two patients were operated upon for posthysterectomy vaginal prolapse. The original operation had been abdominal hysterectomy in 11 patients and vaginal hysterectomy in an additional 11 patients. All of the corrective operations were performed abdominally. Vaginal sacropexy was performed upon eight patients with our own modified method using a fascial strip taken from the rectum sheath. Dexon sutures were used in the attachment of the strip to the apex of the vagina and to the periosteum of the sacrum. The fascial strip was peritonealized. A high resection of the enterocele sac was performed. Excellent permanent vaginal support was achieved in all of these patients. Other methods of operation used included direct fixation of the vaginal apex to the presacral fascia, fixation of the vagina with round ligaments and the method according to Williams and Richardson. More than one-half of the patients had recurrences.

Cytoplasmic estrogen receptor concentrations in the endometrium of Finnish and Japanese women.

Cytoplasmic estrogen receptor concentrations in the endometrium were determined in 13 Finnish and 20 Japanese women. All the women were premenopausal and aged between 40 and 50. The concentration of estrogen receptors in the proliferative endometrium for Finnish women was 246.9 +/- 46.2 fmol/mg protein (mean +/- S.E.M.) and that for Japanese women 45.7 +/- 17.1 fmol/mg protein. The difference is significant (P less than 0.01). In the secretory endometrium the corresponding figures were 97.8 +/- 33.7 fmol/mg protein for Finnish and 20.0 +/- 6.4 fmol/mg protein for Japanese women (P less than 0.05). During the proliferative phase the serum estrone concentration was significantly higher in Finnish than in Japanese women (P less than 0.05).

Vasopressin in fetal cerebrospinal fluid.

Vasopressin concentrations were measured in the cerebrospinal fluid in two hydrocephalic fetuses at 33 and 34 weeks of gestation. The vasopressin concentrations in cerebrospinal fluid were 0.8 and 0.6 pg/ml, and in amniotic fluid 0.4 and 1.0 pg/ml, respectively. Previously measured vasopressin concentrations in amniotic fluid of the normal fetuses have been of the same magnitude.

Skinfold thickness and long-term post-menopausal hormone therapy.

Skinfold thickness was measured in 130 post-menopausal women treated with long-term hormone therapy. One group of 50 women took oestradiol valerate 2 mg/day for 3 wk out of 7, a second group comprising 19 women received oestriol succinate 2 mg/day and the remaining group of 61 women used oestradiol valerate 2 mg/day combined sequentially with norgestrel 0.5 mg/day. The duration of treatment in these groups was 6.3 +/- 0.4, 6.4 +/- 0.4 and 3.3 +/- 0.3 yr, respectively. The control group was made up of a further 89 post-menopausal women. The skinfold thickness in all the treated groups was significantly greater than that in the controls.

Effect of prostaglandin inhibitor and oestrogen on climacteric symptoms and serum free fatty acids.

Oestradiol and naproxen were compared in a double-blind, cross-over study of 20 women suffering from climacteric symptoms. Subjective symptoms were assessed before and after treatment. In addition, serum levels of prostaglandin precursors and of certain other free fatty acids were determined. Oestradiol was found to have a slightly more pronounced effect than naproxen on symptoms and on fatty acid levels. Nevertheless, a large number of the patients reported significant alleviation of their symptoms on naproxen. Some of the climacteric symptoms may thus be mediated by prostaglandins. In most patients the fatty acid levels decreased after treatment with both substances, but there were two exceptions. It is suggested that there may be two separate types of metabolic response to these drugs.

Multicentre study of effects of Org OD 14 on endometrium, vaginal cytology and cervical mucus in post-menopausal and oophorectomized women.

A multicentre study covering 69 post-menopausal or oophorectomized women was performed to determine whether Org OD 14 [7 alpha, 17 alpha)-17-hydroxy-7-methyl-19-norpregn-5(10)-en-20-yn-3-one) administered orally in a daily dose of 2.5 mg for 90 consecutive days induces endometrial proliferation. The treatment with Org OD 14 was continued in combination with 1 mg/day of lynestrenol from day 91 for 10 days to ascertain whether secretory transformation of the endometrium and subsequent withdrawal bleeding would occur. Endometrial biopsies were obtained before treatment and on day 91. The effects of Org OD 14 on vaginal mucosa and cervical mucus were also evaluated. Org OD 14 did not display any effect on the endometrium in 56 of the study subjects (83.5%). Weak stimulation (initial proliferation) was seen in 11 of the subjects (16.4%) and withdrawal bleeding occurred in only 5 of these after cessation of the combined treatment with lynestrenol. However, moderate 'oestrogenic' effects on vaginal mucosa and cervical mucus were induced in all study subjects.

Androgen, estrogen and progestin cytosol receptor concentrations in the normal human endometrium. Effects of intrauterine device.

The specific androgen, estrogen and progestin binding was studied in normal human endometrium during various phases of the menstrual cycle. The simultaneous presence of these three hormone receptors was a common finding. Androgen receptor concentrations did not show cyclic variations. A copper intrauterine device had no effect on the endometrial androgen, estrogen and progestin receptor concentrations.

Late complications of laparoscopic clip sterilization.

Late sequelae after laparoscopic Hulka clip sterilization were investigated in 100 women, with a follow-up time of 1 1/2-3 years. Of the patients, 18% showed menstrual disorders, and one patient complained of climacteric symptoms. Increased libido was reported by 34% of the women and decreased libido by 8%. Of the husbands, libido had increased in 18% of cases, while none of them reported decreased libido. Sexual intercourse was felt to be more satisfactory after sterilization by 40% of the women, while the others did not find any change. All the patients were asked whether they would again choose laparoscopic sterilization as a contraceptive method and all answers were unhesitatingly in the affirmative.

External endometriosis in 801 operated patients.

Details of medical history, symptomatology, operative findings and certain other special features are reported for patients with external endometriosis treated operatively during the period 1969-76. The incidence of endometriosis in this survey was 19% of all gynecological laparotomies. The mean age of the patients at the time of the operation was 36.7 years. The most common sites of endometriosis were the retrocervix and the ovaries. Of the patients, 559 (70%) had complained of dysmenorrhea; the pain generally began before the bleeding. Also typical was the exacerbation of the pain with increasing age. Twenty-seven per cent of the patients also complained of infertility. Surgery was performed because of acute abdominal symptoms in 6% of cases. The incidence of endometriosis in postmenopausal women was 2.5% and that in young women, 1%. Endometriosis recurred in 15% of the cases during the follow-up period. Gestagen therapy after the operation had no effect on the recurrence rate.

Treatment and prognosis of metastatic gestational trophoblastic disease.

Three cases with metastasized choriocarcinoma are reported. The basic chemotherapeutic regimen was methotrexate and actinomycin D, combined with cyclophosphamide or vinblastine. Surgery was used in the treatment of both primary and metastatic lesions. The chief indications for hysterectomy arise when chemotherapy has obviously failed. Thoracotomy was performed for a pulmonary metastasis and nephrectomy for a metastasis of the kidney. Craniotomy was performed to control the brain hemorrhage. Aggressive multidrug chemotherapy is considered necessary as the primary treatment for high-risk cases of trophoblastic disease.

Analysis of intrapartum fetal deaths: their decline with increasing electronic fetal monitoring.

Over a 12-year period, from 1970 to 1981, 30 600 babies were born at the Department of Obstetrics and Gynecology, University Central Hospital, Turku, Finland. During that period, the use of electronic fetal monitoring increased remarkably, being involved in 9, 12, 33 and 95% of all vaginal deliveries during four consecutive 3-year periods. The number of intrapartum deaths during the same 12-year period was 52, giving an overall rate of 1.7 per thousand. When 15 lethally malformed infants are excluded, the rate becomes 1.2 per thousand. In the four consecutive 3-year periods, the death rates were 1.7, 1.9, 1.0 and 0.3 per thousand. Electronic fetal monitoring was not undertaken in any of the cases leading to fetal death. The main factor leading to fetal death could be considered to be hypoxia in approximately 90% of the deaths of normally formed babies. The most common reasons for hypoxia were placental abruption and cord entanglement, yet in many cases only the decreased placental perfusion could be suggested to have caused the hypoxia. The mean weight of those babies that died intrapartally decreased significantly, being approximately 1 250 g during the last 3-year period.

Subtotal hysterectomy in emergency obstetrics.

Emergency obstetric subtotal hysterectomy was performed in 11 women over a 20-year period in the University Central Hospital of Tampere. The incidence of these operations was 1 in 7,623 deliveries. During this period the incidence of uterine rupture was 1 in 13,976 deliveries. The other indications included placenta accreta, atonic postpartum haemorrhage and haemorrhage during caesarean section. There were no maternal deaths or major complications. The authors recommend safe subtotal hysterectomy operation of choice in these emergency cases.
