[Dynamic liver function study using the lidocaine test].

Lidocaine clearance test may reflect the integrity of various oxidative pathways of hepatocytes. Lidocaine is a highly sensitive indicator of hepatic dysfunction. This test is applicable to assessment of stage and prognosis of the liver disease, to timing of the liver transplantation, and to assessment of the pretransplant prognosis in the end-stage of chronic liver disease. Formation of the major lidocaine metabolite monoethylglycinexylidide (MRGX) depends on the hepatic cytochrome P-450 activity and on the liver blood flow.

[Orexins and orexin receptors].

The hypothalamus plays a central role in the integrated control of feeding and energy homeostasis. Two novel neuropeptides Orexin -A and -B were identified. Orexins activate two closely related G protein-coupled receptors and stimulate food consumption. They act also as mediators in the central feedback mechanism and regulates feeding behavior.

Computer assisted evaluation of lipid tests with regard to the risk of atherosclerosis.

We developed the algorithm for computer assisted evaluation of lipid tests with regard to the risk of atherosclerosis. This programme has been created as a part of the laboratory information system and is determined to estimate the metabolic risk of coronary artery disease in both screening programmes and lipid clinics. The programme has made an important contribution to diagnostic and therapeutic process of atherosclerosis and hyperlipoproteinemia. It contains evaluation of special indexes, as e.g. HDL-cholesterol/total cholesterol, apolipoprotein A-I/B.

[Plasma homocysteine levels in patients with indications for cardiac revascularization].

The amino acid homocysteine is a further possible risk factor for the development of atherosclerosis. The authors examined at the First Medical Clinic, Second Medical Faculty Charles University in the course of 1994-1996 59 patients with manifest IHD who were examined by coronarography and because of a major finding were indicated for revascularization surgery. This group was compared with a control group of subjects of equal age without major findings on the coronary circulation. The control group differed from the former group significantly only as regards the triglyceride level which was higher in the former group and the group comprised a larger number of diabetic patients. As regards cholesterol, HDL-cholesterol, uric acid, fibrinogen levels there was no significant difference. The authors found an insignificant difference in homocysteine levels. In the investigated group of patients indicated for revascularization surgery the mean homocysteinaemia was 12.3 mumol/l (sigma 4.3 mumol/l) and in the control group 11.8 mumol/l (sigma 3.8 mumol/l). The mean homocysteine level in the investigated group did not exceed the level of 15 mumol/l reported in the literature as the borderline of normal homocysteine blood levels. The authors did not reveal a statistically significant correlation between homocysteine levels, diabetes mellitus type II and smoking.

[Reference values, metabolism and findings of abnormal values of magnesium in childhood].

In the introduction we summarize specifics of reference values and metabolism of magnesium in childhood. We studied the frequency of abnormal values of magnesium in both children and adults hospitalized in Faculty Hospital Motol, Prague. We analyzed 6761 tests of S-Mg and 356 dU-Mg results located in archives of Department of clinical biochemistry (1. 1. 1992-1. 11. 1995). The frequency of low level of S-Mg (< 0.65 mmol/l) was 14%. Hypermagnesemia was present in 1.6% of the total number of cases. We found low values of dU-Mg in 20% of the total number of cases, hypermagnesiuria was present in 8.4% cases. We also proved that hypomagnesemia is more frequent in both boys and men than in girls and women. Critical values of S-Mg below 0.5 mmol/l are less frequent (84 cases). Estimated frequency of clinical significant hypomagnesemia in hospitalized patients was about 1.25%.

Urinary uric acid excretion in healthy male infants.

Blood and urine samples were collected from 23 healthy term male infants aged 2-12 months (mean 6.6 months). Data for establishing urinary uric acid reference values were obtained: urine concentration, 24-h urine output, weight-related urine output, urine output related creatinine, clearance, and fractional excretion. A negative correlation with age was demonstrated for all parameters studied.

[Computer-assisted interpretation of laboratory results].

The clinician-the patient-the laboratory are parts of one communication system. Computer based communication between laboratory and clinician is fast, highly reliable, and accurate. Computer assisted interpretative reports are a natural consequence of the requirements of problem solving strategies in clinical and laboratory medicine.
