Analysis of mAb reactive with the porcine SWC1.

Among 54 mAb determined to be reactive with porcine T lymphocytes and/or activation antigens, eight mAb (workshop Nos. 005, 031, 080, 091, 092, 093, 094 and 110) derived from different laboratories grouped together in the T11 cluster and were ordered into the SWC1. One mAb (No. 111) which belong also to this group was lost during the workshop. The SWC1 antigen is a molecule expressed on the majority of leukocytes, resting T lymphocytes, monocytes and granulocytes, but not on B lymphocytes. On T lymphocytes it is down-regulated after activation. The molecular mass of the antigen is unknown. Epitope analyses revealed that seven out of the nine mAb recognized similar epitopes on the SWC1 molecule.

Report on the behaviour of monoclonal antibodies in the First International Pig CD Workshop identifying the Null cell families.

Clustering analyses were carried out on data from five independent laboratories testing 22 monoclonal antibodies (mAbs) reacting with CD2-sIg-lymphocytes on 14 pig blood and/or tissue lymphoid target cells using cytofluorometry. This was coupled with extensive further studies on blood lymphocytes from normal and thymectomised SLAb/b inbred pigs. These mAbs formed two groups: those mainly identifying the large blood-borne thymus-dependent Null T cells (N) and those reacting with tissue and a small number of blood-borne thymus-independent lymphocytes (N'). Based on their tissue cell reaction patterns, the 10 N' mAbs formed three main groups: N'1A and B; N'2A and B; and N'3. The 12N mAbs fell into four groups N4-N7; N6 was divided into subgroups A-D. One N' (032) and two N mAbs (010 and 063) were unclustered. Based on these data, swine workshop cluster numbers were designated to groups N5 (021, 022 and 059) as SWC4, N6 (061 and 117) as SWC5 and N7 (020 and 141) as SWC6, the latter exceptionally as a single antibody, MAC320, since it is the 'type' mAb identifying effectively all blood Null T lymphocytes. Future research and workshops will have to define with a wider range of techniques the relationships, molecular properties and functional roles of the several new, perhaps novel, antigens identified by this family of fascinating, as yet still poorly defined, mAbs.

[150 years since the birth of R. Koch--his life and work].

In December 1993 150 years have elapsed since the birth of Robert Koch. Koch began his research activities already as a rural physician. Later, from 1880 he worked in the Imperial Health Institute in Berlin, from 1885 he was as professor in charge of the Institute of Hygiene of the Medical Faculty of Berlin University. Greater opportunities for research were offered to him in the Institute for Infectious Diseases in Berlin. R. Koch laid the foundations of scientific microbiology, his activities were, however, also closely linked with epidemiology. His discoveries include evidence of B. anthracis, V. cholerae and in particular M. tuberculosis (1882). He studied also the etiology of wound infections. He enriched microbiology by new cultivation, staining and microscopic techniques, by using vapour during sterilization as well as investigations of the effect of new disinfectants. A major part of his research was devoted to tropical diseases which he studied during six expeditions. Koch was not only concerned with laboratory research, he implemented also many epidemiological field projects. Many scientists who later became famous worked with him. In 1905 Koch was awarded the Nobel Prize. R. Koch died on May 27, 1910.

[Extra-urogenital detection of Chlamydia trachomatis--perinatal transmission].

Infections produced by Chlamydia trachomatis reach an epidemic scale worldwide. Examination of 1,333 women with suspected cervicitis and other clinical symptoms revealed positive findings in 355 (27.5%). Of 72 examined men 25 (34.7%) were positive. Perinatal infection is confirmed also by positive evidence of Ch. t. in 28.9% neonates with conjunctivitis and 19% positive smears from the nasopharynx. The authors used the direct immunofluorescent method, Chlamyset of Orion Co., Finland.

The effect of food on pharmacokinetics and pharmacodynamics of fenoldopam in class III heart failure.

Eighteen patients with New York Heart Association class III congestive heart failure were given single 100 mg oral doses of fenoldopam with food or fasting in a random-order single-blind crossover trial. Before and after each fenoldopam dose, thermodilution cardiac output, right atrial pressure, pulmonary artery pressure, and pulmonary capillary wedge pressure (PCWP) were measured with a balloon-tipped pulmonary artery catheter, and heart rates and blood pressures were recorded with an automated sphygmomanometer. Compared with fasting, bioavailability of fenoldopam was decreased significantly when administered with food: mean peak plasma fenoldopam level decreased from 26.5 (+/- 4.1 SEM) ng/ml to 10.9 (+/- 1.7 SEM) ng/ml (p = 0.0004) and mean area under the concentration-time curve was decreased from 44.7 (+/- 5.8 SEM) ng.hr/ml to 26.8 (+/- 4.1 SEM) ng.hr/ml (p = 0.0001). Fenoldopam administration to fasting patients resulted in decreases in mean arterial pressure, systemic vascular resistance, and PCWP and significant increases in cardiac index without change in heart rate. The maximum changes in mean cardiac index, systemic vascular resistance, and PCWP were greatest 1 hour after oral administration and did not persist beyond 3 hours after administration. In fasting patients, changes in cardiac index were correlated with plasma fenoldopam levels, whereas changes in PCWP and mean arterial pressure did not correlate significantly with the observed fenoldopam level.

Continuous monitoring of mixed venous oxygen saturation as an indicator of pharmacologic intervention.

This prospective study evaluated the ability of continuous SvO2 measurements to predict the onset and duration of action of the oral vasodilator, fenoldopam. Eight patients with New York Heart Association functional class 3 CHF received 100 mg fenoldopam in the fasted state. Serial hemodynamic parameters and SvO2 measurements were obtained at baseline and up to eight hours postdose. Although wide interpatient variability was observed, the SvO2-time response curve produced a similar trend as the CI-time response curve. The SvO2 may be a useful drug monitoring parameter in patients with NYHA class 3 CHF. Seriously ill patients may not have a good CI/SvO2 correlation. This is most likely due to an unstable oxygen consumption rate at the tissue level. Therefore, we recommend establishing the relationship between CI and SvO2 prior to its use as a drug monitoring parameter.

Effects of food ingestion on hemodynamics in chronic congestive heart failure.

This study evaluates the effects of a standardized meal on cardiovascular hemodynamics in 12 patients with New York Heart Association Class III congestive heart failure. This was done as part of a larger study in which an orally active dopaminergic agonist was given on two mornings, once with a standardized breakfast and once fasting, and one morning a placebo was given with the breakfast. The order of days was randomized. Hemodynamic data were obtained over 8 h each day. There were significant changes in several hemodynamic variables after the placebo-food regimen lasting up to 1.5 h: cardiac index rose from 1.8 +/- 0.4 to 2.2 +/- 0.5 L/min.m2 at 30 min (p less than .001) and to 2.0 +/- 0.5 L/min.m2 at one hour (p less than .05); stroke volume index rose from 20 +/- 7 to 24 +/- 7 ml/min at 30 min (p less than .05) and to 23 +/- 7 ml/min at one hour (p less than .05); systemic vascular resistance fell from 1890 +/- 685 to 1534 +/- 497 dyne.sec/cm5 at 30 min (p less than .001) and to 1668 +/- 524 dyne.sec/cm5 at one hour (p less than .05). Mixed venous oxygen saturation was measured continuously in seven patients and rose significantly at one and 1.5 h. We conclude that food ingestion can have a significant effect on cardiovascular hemodynamics and that this effect should be considered when therapeutic effects are to be guided by invasive hemodynamic monitoring in this population.

[Results of serological surveys for leptospirosis in animals and people in the environs of 2 water reservoirs in eastern Slovakia].

The authors present the results of serological surveys for leptospirosis in rodents, cattle and humans, carried out in the vicinity of two large water reservoirs in Eastern Slovakia. Of the total number of 243 rodents covered by the surveys, 8.2% yielded positive reactions with leptospiral antigens. In most cases the sera reacted with L. grippotyphosa and L. sejroe . Of 168 samples of cattle serum, 13 serum samples, i.e. 7.7.%, yielded positive reactions, also with L. sejroe and L. grippotyphosa. In the group of serum samples obtained from humans, 28 out of 876 samples studied in the surveys, i. e. 3.1%, were positive. Among humans 6 serotypes were established; of these, L. sejroe occurred most frequently among humans, and also among domestic animals and rodents, then followed L. bratislava and L. grippotyphosa. The positive results obtained in humans, cattle and rodents with the prevalence of the same serotypes indicate the existence of close contacts of humans with animals, as well as the possibility of acquiring water-borne infection in recreation zones.

Hyperglycinaemia without ketosis.

When screening mentally backward children we detected a 5-year-old child with elevated urinary glycine excretion and a high blood glycine level. The report discusses the results of a clinical, metabolic and genetic examination of the child and the members of his family with reference to this rare metabolic disorder.
