[Behaviour of pertrochanteric fractures of the femur after condylar nailing under dynamical stress (author's transl)].

In continuation of our studies on dynamic tolerance after condylar nailing in pertrochanteric fractures, we have examined the femora of patients who died within the first 2 weeks after surgery and studied their stress tolerance. We appreciated the lack of the weight bearing medial cortical bone and the need for abduction at the fracture site by the behaviour under stress. The results under stress could be compared with an experimental fracture model. From these experiments conclusions have been made for the correct placement of condylar nails for their best static advantage.

[Homologous articular cartilage transplantation in animal experiments. Preliminary studies on sheep (author's transl)].

Homologous articular cartilage without subchondral bone instead of the usual osteochondral grafts was transplanted in sheep. The transplants were fixed to the host by a biological tissue adhesive based on fibrinogen clotting. Light- and electronmicroscopic studies showed viable transplants up to 7 months and a stable bony union with the host through endochondral bone formation. A additional external fixation of the joint after transplantation or the use of a conventional cyanoacrylate tissue adhesive failed to achieve positive results. The relations of these preliminary findings for restorative techniques of articular cartilage defects are discussed.

[Splenectomy for traumatic rupture of the spleen in childhood and its sequaelae].

This study was undertaken in order to assess the clinical and immunological consequences of splenectomy for traumatic reasons in childhood. Immunological testing of 22 persons 1 to 20 years subsequent to removal of the spleen for traumatic rupture between the ages of 3 and 6 revealed diminished or absent agglutinins in 10 cases and diminished or absent opsonins to E. coli in 16 cases. All patients were in good health and no clinical evidence of increased susceptibility to severe infections was found postoperatively. It is, therefore, assumed that the diagnosed defects had been compensated for by other immunological mechanisms. In contrast to the opinions of other authors, it is concluded that splenectomy in childhood between the ages of 3 and 6 does not appear to carry greater risks than in later years.

[Stress tolerance of different osteosynthetic procedures in pertrochanteric fractures (author's transl)].

To obtain valid information on stress tolerance of pertrochanteric fractures, we exposed three different methods of osteosyntheses to dynamical stress and the results were compared. The methods of osteosyntheses were application of a 130 degrees angle plate, performing an adjoining osteosynthesis with bone cement or using cephalocondylar nails. The measurements in connection with photographical and roentgenological examinations of osteosyntheses under stress gave the following results: Compared with the other methods treatment with condylar nails gave equal results in static behaviour yet was clearly superior in its dynamic reaction.

[Heterologous fibrinogen for tissue adhesion. immunological and histological investigations (author's transl)].

The immunological reactions of heterologous fibrin were investigated in a sensitive experimental system. In 10 rabbits autologous full-thickness skin grafts were glued with bovine fibrinogen cryoprecipitate. 5 of these animals had been immunized against this adhesive before transplantation. In a control group the graft was fixed with homologous fibrinogen cryoprecipitate. Examination of the sera of the animals for antibodies against fibrinogen was performed before immunization, 1 day prior to and 14 days after skin grafting. 14 days after skin adhesion all grafted areas were examined by histological methods. In the group without preoperative immunization antibodies were found against fibrinogen. The skin graft healed in this series in the same manner as in the sham group. In the series with preoperative immunization the glued skin graft was discharged early and the healing procedure of the defect was retarded.

[Fibrin adhesion of cartilage surfaces].

Basing on encouraging results with the fixation of pure articular cartilage with the fibrin sealant in animal experiments, pure cartilage fragments were replanted in human patients using fibrin sealant. From individual cases biopsies could be taken from macroscopically not well healed areas during arthroscopic examination. The histological results from these cases are presented together with overall clinical experiences. Six weeks after operation one side of the graft showed no union with the subchondral bone and was also separated from the cartilage bed. However, after three and six months, the grafted cartilage had fused with subchondral bone, but pannus-like tissue was covering the surface and filling the cleft at the border to the bed. After one and five years, complete fusion with the subchondral bone and the surrounding cartilage can be demonstrated. There is always covering pannus at the larger observation times. In the majority of the 73 cases treated so far excellent clinical results were obtained, enabling the patients to take up their sporting activities again.
