[Project for the prevention of thyroid pathology and iodine prophylaxis education in the province of Campobasso].

A study carried out on primary school population in Campobasso district permitted us to evaluate the goitre prevalence and to effect advertising campaign about use of iodate-salt. In six months of 1997 we effected a screening of 7041 children aged 6-11 years. The project provided a clinical stage (goitre observation by means of inspection and palpation, auxological data picking) and an informative stage (divulgation of informative material, conferences with the teaching staff, television publicity). We attributed a score to the thyroid volume according to WHO classification. Among the 7041 children, 2093 (29.7%) presented goitre of 1A degree and 84 (1.2%) a goitre 1B. Our results showed in Molise a thyroid pathology prevalence higher to the threshold (10%) over that we can define a population affected with endemic goitre. From that the importance of screening programmes in the whole region and the promotion of interventions of iodine-prophylaxis.

[Traumatic tearing of the mesentery: immediate or deferred emergency surgery].

We have considered a 1 year first-aid survey and focused a 6 checked mesentery vascular traumatic tearing cases. We have valued some hypotheses on pathogenic mechanism assumptions about these vascular injuries in abdomen trauma. Among these, we have focused our attention on the relationship between visceral stretching from alimentary bolus and abdominal vascular trauma. During diagnosis, we valued the importance of abdominal sounding, of echotomography and Computerized Tomography; the first one seems to be remarkable for reliability and quickness of accomplishment. In one case we have performed laparoscopic diagnostic-therapeutic approach. We suggested some standards for surgery choice related to injury amount. Monitoring is particularly important for those patients it can performed by invasive or image means seen that the unacknowledged endo-abdominal injury increases mortality by about 5-65%.

[Heart failure in the population: prevalence data].

We report on the prevalence of chronic heart failure (CHF) in a random sample of a population (aged 20-64 years) from the Veneto region in northern Italy. The relationship between CHF and hypertension and obesity was also investigated. These data were collected during an international research project coordinated by the World Health Organization. The overall prevalence of CHF was 2.0% both in the male and female population. The prevalence of CHF increased significantly with age and was positively correlated with body mass index in both sexes. Patients with borderline hypertension showed a 3.5-fold increased prevalence of CHF. The prevalence of CHF was 4.9-fold higher in hypertensive than in the normotensive subjects. Patients treated with hypotensive drugs had a significantly higher prevalence of CHF than untreated patients.

[Usefulness and limitations of hormonal monitoring of pregnancy with HPL and estriol in improving fetal prognosis and preventing neonatal neurologic deficiency].

This multidisciplinary research has been subdivided into two parts. The first examines prognosis of the foetus and its weight development in relation to changes in the hormonal curves of HPL and E3 carried out in a batch of 339 3rd term pregnancies. A very high incidence of small-for-date newborns (30-64%) was noted in mothers with low hormonal values and generally a more frequent onset of hypoxia in pregnancy and in labour. The second part assesses the neurological profile of children born to the same mothers examined in the first part up to the 4th year of life. A neurological deficiency of a different extent was evidenced in 33.3% of the children born to mothers with low hormonal values in pregnancy, especially among those born underweight. The neurological deficiency in the control sample was 7.84%.

Interhemispheric transfer of visual information in schizophrenics.

Groups of 12 male schizophrenic inpatients and 12 normal controls of the same age, sex, and schooling underwent a tachistoscopic test. Each subject was shown an alphabetical letter which he subsequently had to recognize among various other alphabetical letters also shown tachistoscopically. The visual hemifields of the two displays were ipsilateral and crossed. Relative to the normal group the schizophrenics showed higher perceptual thresholds, lower over-all mean performance, and greater lateralization. The results are discussed in terms of rigidly and poorly integrated systems of analysis of information between one hemisphere and the other for schizophrenic patients.
