Calcaneal involvement in inflammatory disease.

The calcaneus may be involved in a variety of inflammatory conditions. Localized swelling, redness, and pain do not rule out metabolic pathologic conditions. One must initially determine that trauma has not created the malady. Personal or family history of systemic inflammatory disease is also necessary to create a proper differential diagnosis. Pedal radiographs can provide significant information in formulating an opinion. Osseous architecture, abnormal calcifications, and other alterations should be observed. Surrounding tissues may appear inflamed as well. Bone scintigraphy and nuclear medical studies can provide further information. Laboratory tests alone often are not diagnostic but, when coupled with other objective factors, can aid the clinician. Proper treatment must be determined once a diagnosis is established. Although many nonsurgical modalities are available, operative intervention is inevitable under certain circumstances. The clinician is cautioned that when surgery becomes necessary, appropriate perioperative considerations of the inflammatory process are mandatory.

hyperthermia for lower extremity neoplasms: a review of the literature.

Hyperthermia appears to be an important adjunct to present day cancer therapies. A literary review indicates that at least 60 of the patients studied showed improvement-primarily relief of pain. Radiographically, approximately 50% of the patients demonstrated a remission or decrease in the size of the tumor (4-7, 9, 10). Although further research in hyperthermia continues, amputation of a limb because of osteogenic sarcoma or malignant melanoma may no longer be the primary treatment of choice.

Bone healing after adjacent metatarsal osteotomies.

In this study, the authors reviewed the postoperative progress of 32 patients with simultaneous adjacent metatarsal osteotomies to determine the effect of surgical location on postoperative healing. Results of their study supported their suspicions that more bone callus was formed and edema was more prolonged when osteotomies of the same type were performed on adjacent metatarsal bases than when they were alternated (i.e., one at the surgical neck and the other at the adjacent metaphyseal base).

Surgical repair of lateral ankle instability by modification of traditional approaches.

Recurrent instability following ankle sprains which fail to respond adequately to conservative treatment has been the focal point of discussion among physicians in recent years. A variety of surgical regimens have been implemented primarily over the past 40 years in an attempt to control this problem. The authors discuss these modalities and present their successful approach in treating lateral ankle instability.

Identification and treatment of the diabetic neuropathic foot.

The authors discuss neuropathic osteoarthropathy in the diabetic foot, often associated with Charcot joints. Identification of this malady as well as indications for treatment are reviewed. Representative cases are illustrated.
