Interstitial temperature measurements during transurethral microwave hyperthermia.

Microwave hyperthermia is presently being investigated as a treatment for alleviating the symptoms of urinary outlet obstruction associated with benign prostatic hyperplasia. Two clinical techniques using intracavitary microwave applicators are being evaluated for safety and efficacy at various institutions. The transrectal technique uses a directional microwave radiator that is inserted into the rectum adjacent to the prostate. The transurethral approach uses a symmetrically radiating applicator located within the prostatic urethra. Transrectal prostatic heating techniques require surface cooling to prevent hazardous temperatures in the intervening rectal mucosa. Since transurethral applicators radiate from within the prostatic urethra, heating is confined to the obstructive tissue immediately surrounding the applicator. Concern has been expressed regarding the possibility of thermal injury to the prostate and adjacent rectum during transurethral hyperthermia treatment. In this report we present interstitial temperature measurements of prostatic and rectal temperatures in 5 patients. Temperature was observed to decrease at a rate of about 6C/cm. outward from the applicator. No clinically significant temperature increase was observed beyond 1 cm, outside the prostatic capsule or in the rectal mucosa.

Is there a place for conservative surgery in the treatment of renal carcinoma?

Since 1981, 31 patients have undergone conservative surgery for malignant renal tumours and have been followed up for at least 2 years. The techniques included enucleation or resection (wedge resection or partial nephrectomy). In 10 patients the indications for kidney-sparing surgery were absolute, while in the remainder the conservative surgical approach was a deliberate choice. The tumours varied in diameter from 1.3 to 12 cm and no metastases were detected on pre-operative screening. One patient died post-operatively from myocardial infarction. In the remaining 30 there were no local recurrences. Two patients died from skeletal metastases (1 with bilateral malignancy) and 2 underwent surgery in the post-operative period for haemorrhagic complications. The efficacy of conservative surgery in the local control of renal cancer is an argument in favour of its wider use.

Experience with the intraprostatic spiral.

Thirty-six patients have been followed up after insertion of an indwelling intraurethral device for prostatic obstruction. Follow-up exceeded 1 year in some cases. The main problem was dislocation of the spiral not only in the early stages but also after several months. Endoscopic assessment of the obstruction seems important during the pre-operative investigations. The ideal indication is prostatic obstruction in patients who are unsuitable for surgery and who need relief of acute retention for a limited time only. In selected patients the prostatic spiral has a place in the treatment of obstructive prostatism.

Intraarterial embolization in combination with surgery in the management of congenital pelvic arteriovenous malformation.

A case of congenital arteriovenous fistula of the pelvis is described. Arteriovenous malformations of the pelvis are rare and can give rise to problems in diagnosis and treatment. A long history of vague complaints precedes determination of the correct diagnosis. Because recurrence is not uncommon after treatment, intraarterial embolization in combination with surgery is mandatory.

Solitary or predominantly right-sided varicocele: a possible sign of situs inversus.

Among more than 850 patients undergoing outpatient sclerotherapy of the internal spermatic vein, only 1 unilateral and 1 predominantly right-sided varicocele were encountered. In both patients with this finding a total situs inversus was present, with inversion of normal anatomy, enlarged "right" internal spermatic vein draining into the renal vein, and the "left" internal spermatic vein entering the inferior vena cava directly. Total situs inversus should be considered whenever right-sided varicocele is solitary or predominant.

Seminal vesicle cyst and ipsilateral renal agenesis. Case report.

Cysts of the seminal vesicles are rare, and associated anomalies of the ipsilateral kidney are frequent. We report a case of a monolocular cystic mass in the seminal vesicle bed in a young male with recurrent epididymitis, with emphasis on the ultrasonographic and CT features. The embryological background is briefly mentioned.

Left spermatic vein in left-sided vena cava.

The left spermatic vein was found to drain directly into a left-sided vena cava in a patient with left varicocele. The implications for embolization of this anomaly are obvious.

CT demonstration of a ureterosciatic hernia.

Ureterosciatic herniation is probably not as uncommon as a literature review would suggest. It rarely produces symptoms and is usually an incidental finding on urogram. The posterior position of the ureter passing beyond the medial wall of the bony pelvis is easily detected on intravenous urography with ipsilateral oblique films and can be confirmed tomographically. This ectopic course of the ureter can be depicted clearly on pelvic computed tomograms.

Xanthogranulomatous pyelonephritis with emphasis on computerized tomography scan. Retrospective study of 20 cases and literature review.

The authors reviewed 20 cases of xanthogranulomatous pyelonephritis. An accurate preoperative diagnosis of this disease is difficult because of its clinical and radiologic similarities to various other renal lesions. A resistant urinary tract infection in a patient with a nonfunctioning kidney and a perinephric abscess should lead to the condition being suspected. Although xanthogranulomatous pyelonephritis is mainly a histologic diagnosis, computerized tomography opens new perspectives in the preoperative determination. This could allow less radical surgery in selected cases.

Solitary benign schwannoma in the psoas muscle.

Schwannomas, which also are referred to as neurinoma or neurilemmoma, are rare. We report a case of a solitary benign schwannoma in the psoas muscle, which was not associated with von Recklinghausen's disease. The tumor was excised successfully. Its appearance on clinical examination is described, the preoperative investigations and operative findings are presented, and the literature is reviewed.

Percutaneous sclerotherapy of the internal spermatic vein for varicocele through an open ended guide wire.

Percutaneous sclerotherapy of the internal spermatic vein through an open ended guide wire was performed in 50 patients with left sided varicocele. The procedure was successful in 47 patients, without major complications. Percutaneous sclerotherapy through an open ended guide wire offers the advantage of very distal and superselective catheterization with complete embolization of the internal spermatic vein, its side branches and parallel collateral channels. Further follow-up hopefully will confirm the reduction of recurrence rate in our patients.

[Ectopic renal artery].

A case of aberrant origin of the right renal artery from the aorta at level Th 12, above the origin of the visceral arteries is reported. The literature of these rare anomalies of the origin of the renal arteries is reviewed.

Ultrasound of the normal kidney: a sonographic, anatomic and histologic correlation.

Technical progress in sonographic equipment providing routinely high resolution images makes it appropriate that some criteria of normality have to be reviewed. In this paper normal sonographic images of the kidney are correlated with anatomy and histology. The effect of aging upon normal anatomical relations and histologic findings is analysed. Special attention is focussed on particular anatomical details as far as they can explain the particular topography or sonographic appearance of some pathologies.
