Single-step percutaneous transhepatic cholecystoscopy.

A 58-year-old female patient with several concrements in the gallbladder submitted herself to pump-controlled percutaneous transhepatic chemolitholysis with methyl-tert-butyl-ether (MTBE). Shortly before the treatment terminated a calcified residual concrement caused an acute blockage of the cystic duct. After opening the puncture track with a bougie, followed by endoscopic lithotripsy, the cystic duct was made passable again.

[The "depressed adenoma" of the stomach].

Depressed adenoma of the stomach, whose gross appearance is similar to that of type IIc early gastric carcinoma, was detected in 40/357, or 11%, of gastric resection preparations. Depressed adenoma is primarily found in the two distal thirds and the lesser curvature of the stomach. It presents as a flat brownish depression and measures between 2 and 39 mm across (mean 13 mm). Histology typically reveals tubular adenoma with closely spaced atypical columnar epithelial cells of the intestinal type. Depressed adenoma probably arises from pre-existing depressed lesions (erosions, ulcer scars, atrophic zones) rather than from conventional protuberant adenoma. Depressed adenoma appears to show a greater tendency for malignant differentiation than do conventional adenomas.

[Malignant somatostatinoma of the papilla major and minor].

In a 50-year-old man with extrahepatic cholestasis and iron-deficiency anaemia, local operative excision of a 4 cm submucous tumour of the papilla of Vater revealed on histological and immunochemical examination a somatostatinoma. Subsequent duodenopancreatectomy further demonstrated in the surgical specimen a lymph node metastasis and a 5 mm somatostatinoma in the papilla minor. The patient has now been free of symptoms for 12 months on substitution treatment with pancreatic enzymes.

Placebo-controlled trial with the somatostatin analogue SMS 201-995 in peptic ulcer bleeding.

A 5-day, double-blind, placebo-controlled, multicenter trial in 23 centers with a total of 273 patients [241 evaluable patients; 126 on placebo and 115 on a long-acting somatostatin analogue (SMS 201-995, octreotide, Sandostatin)] resulted in no difference in stopping bleeding and preventing rebleeding between placebo (70.6%) and SMS 201-995 (69.6%). Surgery rates, blood transfusion requirements, and time required before bleeding stopped were also not significantly different between the two groups. A retrospective subgroup analysis according to age, sex, localization of the ulcers, severity of the bleeding, and arterial spurting vs. oozing showed homogeneity and did not allow identification of a subgroup that might benefit from treatment with SMS 201-995. The tolerability of SMS 201-995 was very good. No difference was found between placebo and SMS 201-995 with regard to the side-effect profile.

Transpapillary placement of an endoprosthesis with the aid of the Frimberger Varioguide.

The guide wire, an important requisite for the placement of endoscopic transpapillary biliary prostheses, should combine two, normally incompatible, properties: a high degree of flexibility for atraumatic passage through irregular or "bent" stenoses, and a high degree of stiffness, to prevent sidewise deviation during advancement of the prosthesis. These opposing, formerly incompatible, properties are combined in a new type of guide wire, known as the Varioguide. With the aid of a tension controller, the stiffness of the Varioguide can be varied steplessly. In 88.2% of 68 patients with stenosis of the bile duct we succeeded in placing a transpapillary endoprosthesis with the Varioguide. In 13 cases in which a previous attempt with the conventional guide wire was unsuccessful, placement of the endoprosthesis was successfully accomplished with the Varioguide; the reverse was true in only a single patient. In the case of very narrow, rigid high stenoses, in which placement of a transpapillary endoprosthesis is especially difficult, the Varioguide has proved highly suitable (stenoses of the bile duct bifurcation).

Giant Brunneroma of the duodenum.

A case of giant hyperplasia of Brunner's glands in the duodenum is reported. Recurrent vomiting due to stenosis of the duodenum was the leading symptom. Endoscopy, ultrasound and histological examination were employed as complementary methods to establish the diagnosis. The 5 X 7 cm large tumor was surgically excised. Brunneromas are always benign and their incidence is about 0.08% in upper GI-tract endoscopy.

Common variable immunodeficiency syndrome and nodular lymphoid hyperplasia in the small intestine.

We report on a case of nodular lymphoid hyperplasia (NLH) of the small intestine in a patient with common variable immunodeficiency (CVID) syndrome. The CVID syndrome comprises a group of heterogeneous immunological disorders. It is characterised by hypogammaglobulinemia, recurrent sinopulmonary infections, gastrointestinal disorders (including diarrhea, infestation with Giardia lamblia, chronic-atrophic gastritis and nodular lymphoid hyperplasia (NLH), and an increased risk of malignancy. NLH is frequently associated with gastrointestinal lymphomas. It has also been found in the terminal ileum of children and in adult patients with Gardner's syndrome. NLH is found in about 20% of patients with the CVID syndrome. The diagnosis of NLH requires endoscopic and bioptic-histological examinations and the determination of the immunoglobulins.
