[Early results of coracoclavicular ligament reconstruction with carbon fiber prosthesis in patients with acromioclavicular dislocation].

The results of surgical repair of coracoclavicular ligament with carbon fiber prosthesis in 34 patients with acromioclavicular dislocation have been presented. They were assessed on the basis of subjective opinion, clinical and radiological examination. Clinically 91 per cent results were rated excellent or good. Anatomical position of articular surfaces within acromioclavicular joint was found in 88 per cent of cases. The authors conclude, that carbon fiber ligamentous prosthesis proved its usefulness in surgical restoring of the stability of the acromioclavicular joint.

Corticosteroid supplementation during cardiac arrest in rats.

HYPOTHESIS: Corticosteroids will improve the rate of resuscitation from cardiac arrest. DESIGN: Prospective blinded randomized placebo-controlled trial. INTERVENTION: An 8-min cardiac arrest was induced by KCl infusion and chest restriction in 36 male Sprague-Dawley rats with continuous EKG and arterial blood pressure monitoring. At the start of CPR the rats received one of three study drugs: normal saline (placebo); 0.05 mg hydrocortisone (Group A) and 0.25 mg hydrocortisone (Group B). Mechanical ventilation, chest compressions and ACLS drug administration were provided following a standardized algorithm. RESULTS: The resuscitation rate was significantly higher (P < 0.05) in Group B (92%) compared to Group A (50%) and placebo (50%). For the rats resuscitated, the duration of CPR (placebo = 163 s, Group A = 126 s, Group B = 120 s) and the amount of epinephrine used (placebo = 0.007 mg, Group A = 0.005 mg, Group B = 0.005 mg) did not reach statistical significance (P = 0.15 and P = 0.21). CONCLUSION: Hydrocortisone significantly increased the rate of ROSC from cardiac arrest. There also appears to be a trend of decreasing duration of CPR and epinephrine requirements with hydrocortisone. Further studies evaluating the mechanism of action and long term effects of hydrocortisone in cardiac arrest need to be conducted.

Circadian rhythms in sleep and wakefulness and in salivary melatonin and cortisol concentrations in mothers of term and preterm infants.

We compared the effects of the demands of term and preterm infants on the daily rhythms of sleep and wakefulness and salivary melatonin and cortisol concentrations in mothers for up to 5 months after either birth (term group) or arrival of the infant home (preterm group). Although there were relatively small differences between the term and preterm groups in the daily patterns of infant sleep-wake behavior, there were more marked differences in the maternal sleep-wake parameters. During the first 8 weeks after the arrival of the infant home, the mothers of preterm infants had significantly less time asleep and fewer sleep bouts per 24 hours than did the mothers of term infants. The mothers of preterm infants spent a significantly longer proportion of each night awake (30-40%) for the first 8 weeks than did the mothers of the term infants (20-30%). There was also a significant difference between the term and preterm groups in the effect of time of day on maternal salivary melatonin concentrations. In the term group, maternal melatonin concentrations were higher at night (10 p.m.-6 a.m.) than at any other time of day. In contrast, in the preterm group maternal melatonin concentrations between 10 p.m. and 6 a.m. were only higher than those measured between 6 a.m. and 2 p.m. Salivary cortisol concentrations were significantly higher in the mothers of preterm infants than in the mothers of term infants throughout the 10-week study period, but the peak in salivary cortisol concentrations occurred between 2 a.m. and 10 a.m. in both the term and preterm groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Surgical treatment of funnel chest].

Fourty one patients (33 males and 8 females) with pectus excavatum were operated by Ravitch method modified by Politowski or Drews (36 cases and 5, respectively) from 1980 to 1989. The age of patients ranged from 5 to 25 years (mean 11 years), they were operated because of cosmetic reasons exclusively. Twenty nine of them reappeared to be examined after follow up of 15 months to 10 years (mean 3.5 years). Subjective opinion of the patient, clinical examination and radiological appearance were taken into consideration. There were 72.5% of excellent and good results. No recurrence of the deformity was observed, the most suitable age for the surgery is from 8 to 10 years.

Effect of busulphan treatment and elevated temperature on the expression of the beta-pol gene in rat testis.

Changes in the expression pattern of the DNA polymerase beta gene during inhibition of spermatogenesis by busulphan and by temperature (artificial cryptorchidism) have been studied. Transient arrest of spermatogenesis in two-month-old rats after injection of a single dose of busulphan (10 mg/kg) resulted in parallel but transient decrease in the 1.4 kb of beta-pol mRNA level to an undetectable value, followed by its reappearance after resumption of spermatogenesis. An artificial cryptorchidism also caused a drastic decrease of beta-pol mRNA level. Both results as well as morphological examination of testis after busulphan injection and artificial cryptorchidism revealed that spermatocytes and spermatids represent the testicular cell fraction containing the elevated amount of beta-pol mRNA. Involvement of DNA polymerase beta in meiotic recombination is discussed.

Presence of messenger ribonucleic acid for epidermal growth factor (EGF) and EGF receptor demonstrable in monolayer cell cultures of myometria and leiomyomata.

There is a paucity of data concerning the factors involved in the growth of uterine leiomyomata. The expression of mRNAs encoding EGF and the EGF receptor in myometrial and leiomyoma cultures suggest that EGF may be involved in the autocrine/paracrine regulation of human uterine leiomyomata and myometrial growth.

Development of circadian sleep-wake rhythms in preterm and full-term infants.

We have compared the roles of neurologic maturity and environmental time cues in the development of the entrained circadian sleep-wake rhythm in the preterm and term human infant. The preterm infants (n = 19) spent some time after birth in a hospital nursery with no environmental time cues, whereas the term infants (n = 22) were exposed from birth to a cyclical light and dark environment with one major caregiver. The circadian sleep-wake rhythm in the preterm infants entrained after a similar time of exposure to an environment with daily time cues but at an earlier postconceptional age when compared with the term group. We conclude, therefore, that it is the length of exposure to environmental time cues, rather than neurologic maturity, that determines the entrainment of the circadian rhythm of sleep and wakefulness in the human infant.

[Avulsion injury of spinal nerve roots in part of the lumbosacral spine].

A case of 40 years old male with left sided avulsion injury of L5, S1 nerve roots due to severe trauma. Diagnosis was confirmed with CT and radiculography. The presence of prolapsed sheath of the avulsed root in radiological examination combined with clinical picture establishes correct diagnosis.

[A trial of ultrasonographic use for evaluating leg lengthening].

Modified de Bastiani method of callotasis with Wagner apparatus was used as a lengthening procedure of 4 femurs and 1 tibia. The ultrasonography was applied to measure the degree of distraction, to determine the eventual axial deviation of the bone ends and to follow up the callus formation in the gap. The obtained results were compared with radiography.
