[Discriminatory function of serum proteins in liver and biliary tract diseases].

In 104 patients with acute virus hepatitis, chronic hepatitides, cirrhoses, fatty livers and biliary diseases with partial and complete obstructive jaundice, respectively, IgG, IgA, IgD, beta1A- and beta1E-globulin, cholinesterase, total protein, and albumin, in 45 of these patients additionally prealbumin, retinol binding protein, thymol turbidity test were determined as well as an electrophoretic separation of the serum was performed. 11 persons with healthy liver served as control group. According to the results of univariate and multivariate variance analyses with following test of redundance (test for indispensability) and analysis of discriminance with calculations of reclassification IgD, beta1E-globulin and retinol binding protein were identified as not evident or redundant. Electrophoresis and thymol turbidity test give sufficient basis informations and can further be recommended for orienting examinations. Immune globulinogrammes from IgB, IgA and IgM are suitable as so-called mesenchyma tests particularly for controls of the course. Prealbumin and cholinesterase prove to be the most sensitive parameter of synthesis, whereas albumin and beta1A-globulin possess a high prognostic evidence.

[Cellular immunoreaction in alcohol-induced liver damage].

Of 231 patients with alcohol-induced liver diseases of different degrees of severity 170 patients had a pathological result of the migration inhibition test, when autologous homogenate of the liver was used as antigen. Of them in 25 cases a stimulation, in the other patients an inhibition of the migration of leucocytes was present. With increasing degree of severity of the liver lesion the number of cases with migration inhibition increased. These findings refer to a cellular immune reactivity in alcohol induced liver diseases.

[Multivariant discriminance analytical computation methods for the evaluation of clinico-chemical parameters in liver and biliary tract diseases].

Multivariate variance and discriminance-analytic investigations on the value of evidence of clinico-chemical tests in the diagnostics of diseases of the liver and the biliary tract gave the following results: 1. By means of clinico-chemical methods and discriminance-analytic functions we succeed in 50% of the cases in formulating a correct diagnosis, in more than 75% of the cases a correct probability diagnosis (3 first places in the rank of diagnoses). Sensitivity and specificity of discrimination were established for 15 classes of diagnoses. 2. Due to the intercorrelations of test results biochemical maximum programmes contain numerous redundant parameters measured on diagnostic information values. With the help of a test for indispensability (redundance test) among 20 parameters owy 5-6 were characterized as indispensable. An optimal combination of parameters should not transgress an extent of 14 tests. 3. Results of reclassifications with discriminance analytic functions confirmed that the discriminance analysis is an extraordinarily suitable method for the computer-supported finding of a decision, on which basis aimed additional investigations might be performed.

[Studies on the functional ability of swine liver perfused with human blood in machine recirculation attempt. Experiment design, elimination of indocyanine green, galactose, bilirubin and ammonia, synthesis of urea, bile secretion].

In 11 machine experiments of recirculation the abilities of survival and functioning of livers of dwarf pigs perfused with human preserved blood were tested with the help of the eliminations of indocyanine green, galactose, bilirubin and ammonia from a closed circulation. Further urea-nitrogen were measured as indicator of the urea synthesis and bile secretion. The perfusion of the animal livers was carried out in 9 cases only through the portal vein. The values of the blood flow were between 0.4 and 1.0ml pro g liver tissue and minute in portal pressure values about 15 cm water column. Two recirculations were carried out with additional flow through the hepatic artery with about 1/5 of the whole blood flow. The results of the experiments described prove a pretty good functional performance of the perfused with human blood livers of dwarf pigs during several hours. The different animal livers, however, showed at the beginning as well as during the further course of the perfusion clear differences concerning their functional capacity. Decrease of the blood flow values below 0.5 ml pro g liver tissue lead to functional loss. The additional flow through the hepatic artery yielded no better results, compared with the only perfusion of the animal liver through the portal vein.

[Studies on the functional ability of swine liver perfused with human blood in machine recirculation attempt. 2. Behavior of serochemical parameters].

In the present paper is reported on the behavior of different serochemical parameters in heterologous perfusion of the liver of pigs. The perfusion of the animal livers was carried out with preserved human blood in 11 recirculation experiments (closed machine circulations). In addition to this an empty circulation with human blood without attachment to an animal liver was carried out, whereby otherwise the arrangement of the experiment was the same, in order to take into consideration the effects of the blood traumatisation in the interpretation of the results. The enzymes GOT and LAP proved as sensible indicators of the lesion of the liver cells occurring in the perfusion of the liver of the pigs. The behavior of the mitochondrial GDH as well as of the lysosomal enzymes SP and BETA-GC which scarcely increased during the recirculations in the perfusate blood, however, allows the conclusion that there did not appear a severe lesion of the liver cells (necrobiosis) during several hours of perfusion. A cholestasis did not develop during the perfusions, when one takes as basis the behavior of the enzymes AP, GGTP and AAP indicating cholestasis. Compared with GOT the enzyme GPT showed by far less elevations in the perfusate blood so that with increasing duration of the perfusion the De-Ritis-quotient significantly increased. Increased LDH-activities above all revealed the increasing during perfusion haemolysis and less reliably a lesion of the liver of the pig. The increase of ADA in the perfusate blood proved as nearly exclusively conditioned by haemolysis. Total protein, albumins, immunoglobulins, cholinesterase and thymol turbidity test remained unchanged in the course of the perfusions.

[Simultaneous extra-corporeal oxygenation and haemodialysis in intesive medicine (a case report) (author's transl)].

In the field of intensive care there is a rise in the number of patients with distrubed or defunct vital functions, in whom multilateral therapy on traditional lines fails. The necessity of introducing new forms of treatment has been the occasion for the development of a universal perfusion device for temporary artificial multiple-organ substitution in the Faculty of Medicine at the Martin Luther University Halle-Wittenberg. This apparatus makes possible the substitution of, or support for, the functions of lungs, kidneys and liver singly or combined, via the extracorporeal treatment of the blood. The shortterm use of the apparatus is intended to provide, in addition to functional substitution of the organs, the best possible conditions for the regeneration of damaged organs. A critical account is given of the clinical application of the apparatus.

[Discriminant studies of immunoglobulins in diseases of the liver and biliary tract].

Discriminant-analytical studies of immunoglobulins G, A and M in 54 persons with a healthy liver and in 476 persons with various diseases of the liver and biliary tracts, and also calculation of correlations between the extent of proliferation of the hepatic mesenchyma and the concentrations of serum immunoglobulin led to the following conclusions: changes in the behaviour of immunoglobulins were specific of the organ and the disease, of limited significance in the diagnosis of diseases of the liver and biliary tracts. Dependence of the quantitative values of immunoglobulins on the activity of hepatic mesenchyma permitted to determine the significance of the indices for the observation over the course of the disease and for its prognosis. However, in the assessment of these changes one should take into consideration the influence produced on the immunoglobulinogram of extrahepatic concomitant disease, and also different individual capacity of the organism to immunoglobulin synthesis.

[Aminopeptidases in the serum following myocardial infarct].

In 19 patients wiht myocardial infarction during 7 weeks after the onset of the infarction the serum activities of the leucine aminopeptidase (LAP) and of the alanine aminopeptidase (AAP) were determined. Here an increase of the LAP-values with a maximum in the 1st week was found. Apparantly a hypoxic lesion of the hepatocytes is to be regarded as cause for this. In patients with shock symptomatology as well as with heart decompensation the AAP-activities were on the average higher than in the cases with myocardial infarction without these alterations during the whole period of observation. In all myocardial infarctions the AAP-zymograms showed an additional anodic AAP-fraction as it was observed in previous investigations in cholestasis.
