An account of aortic regurgitation complicating ankylosing spondylitis is given. Twenty patients with lone aortic regurgitation and without overt spondylitis were examined clinically and radiologically and tissue typed. No evidence of sacroiliitis could be found in any patient. HLA B27 was absent from this group, and no significant disturbance in antigen frequency was noted.
Crystalline complexes of cytoplasmic aspartate aminotransferase of pig heart with the substrates L-glutamate and L-aspartate, and with other amino acids, have been prepared and polarized light absorption spectra have been measured. Striking differences in the directions of polarization of the absorption bands are seen. A complete half-transamination of pyridoxal phosphate to pyridoxamine phosphate by aspartate or by cysteine sulfinate can be demonstrated in the crystal as can the accumulation of a quinonoid intermediate with erythro-beta-hydroxyaspartate. X-ray diffraction studies show that the crystals with erythro-beta-hydroxyaspartate and alpha-methylaspartate are isomorphous with those of both alpha and beta subforms of the native enzyme.

A random sample of patients taking two or more drugs, at least one of which was digoxin or methyldopa, was drawn from a medical clinic population. After giving informed consent, the patients were randomized into control and experimental groups. The experimental group was seen by a specially prepared nurse interventionist who attempted to improve medication compliance. The levels of the drugs in the blood were taken as indicators of medication compliance. Our results document that patients exposed to nurse intervention were more compliant than the general clinic population, but were not more compliant than a nonintervention control group. Problems encountered in the collection and interpretation of compliance study data were identified and discussed.
Liposarcomas of the oropharyngeal regions are exceedingly rare. Their silent, slow growth, submucosal or deep location, circumscription, and firm but resilient texture often suggest a diagnosis of a cyst or benign soft-tissue neoplasm. Two cases of well-differentiated myxoid liposarcoma are presented. The literature of over half a century was reviewed, and all documented cases were critically analyzed. The clinical data correlated with the histopathology and biologic behavior of the tumors indicated that two thirds of all liposarcomas of the head and neck are of the myxoid type. The well-differentiated myxoid liposarcoma was the most common one and has an over-all favorable prognosis. Lipomas of the oropharyngeal regions are relatively common and small, generally not exceeding 2.5 cm. in greatest diamter. A lobulated firm tumor of fast tissue significantly exceeding this size should be strongly suspected of being a liposarcoma.

The experience with methadone maintenance treatment by two major New York City methadone maintenance treatment programs using the Dole-Nyswander philosophy is reviewed. After more than 56,000 person-years in treatment over a 10-year period of time, a unit of medical experience exemplified by these two programs is evident. There was a high degree of patient acceptance, a small amount of recognized residual illicit drug abuse, and steeply reduced criminal activities of those patients who remained in treatment. A wide variety of other methadone treatment programs are recognized. Many use methadone differently from those in the present study. The treatment results reported herein may represent a standard by which narcotic (methadone and other) treatment programs can be assessed.
The purpose of this study is to present evidence that the glucose-insulin ratio in amniotic fluid is a prognostic indicator of the status of the fetus in utero and of the newborn in insulin-dependent diabetic pregnant women.
Milk fat globule membrane was shown to contain sialic acid, all of which could be released without disruption of the fat globule. Sialoglycopeptides were cleaved from the surface of intact fat globules by Pronase and fractionated on Sephadex G-50. Further fractionation of the major sialoglycopeptide peak on DEAE-Sephadex gave two groups of sialoglycopeptides eluted with 0.1 M NaCl (Group A) and 0.5 M NaCl (Group B), respectively. Refractionation gave a major sialoglycopeptide from each of the two groups together with a total of three minor sialoglycopeptides. All five sialoglycopeptides eluted as single peaks using shallow salt gradients on DEAE-Sephadex and contained a hydrophilic peptide chain together with galactose, mannose, N-acetylgalactosamine, N-acetylglucosamine, and sialic acid. Glycopeptides of Group A but not Group B contained fucose. The major sialoglycopeptide of Group B released 35% of its hexose and hexosamine on treatment with alkaline borohydride leaving a sialoglycopeptide which had reduced serine and threonine and elevated alanine levels and in addition contained 2-aminobutyric acid. An oligosaccharide fraction containing N-acetylgalactosaminitol, galactose and sialic acid in a molar ratio of 1:1:2 was partially characterised from the clevage mixture. The major sialoglycopeptide of Group A had a more complex carbohydrate structure and showed no released carbohydrate on treatment with alkaline borohydride. The sialoglycopeptides of milk fat globule membrane show many similarities with those of erythrocyte membrane and have a potential use in comparative and structural studies.
Recurrent dislocation or chronic subluxation of the temporomandibular joints may occur from long-term overclosure. Mandibular overclosure is due to loss of dentition. If the overclosure is not corrected by prosthetic rehabilitation of the proper vertical dimension, the result will be loosening of the temporomandibular capsule and stretching of the lateral ligament. The authors advocate capsule plication and ligamentorrhaphy via a preauricular or endaural approach. Occlusal splints are used during surgery to reestablish the vertical dimension. The patient is kept in intermaxillary fixation for 3 weeks. Postoperative prosthetic reconstruction is needed. Two cases are presented.
