Gastrointestinal tuberculosis.

In the developed countries gastrointestinal tuberculosis is no longer common in clinical practice. In this setting the importance of the condition lies in the vagaries of its presentation and the fact that it is eminently treatable, usually by a combination of chemotherapy and surgery. The clinical features and complications of gastrointestinal tuberculosis are highlighted by the seven cases which we report. Diagnosis and treatment of this condition is discussed and attention is drawn to the importance of case notification. Clinicians should bear in mind the diagnosis of gastrointestinal tuberculosis when dealing with any patient with non-specific abdominal symptoms.

Myoelectric control of prostheses.

The development of myoelectric control systems for powered limb prostheses has advanced rapidly in recent years. The main thrusts in this development have been in realizing self-contained prostheses and in realizing better prostheses control through improvements in the myoelectric signal processing techniques. This review considers the latter of these two areas. It first presents an historical look at myoelectric signal processing and identifies the problems. It then presents a general look at the myoelectric signal and those characteristics which give rise to these problems. A review of the literature related to various control strategies and signal processing techniques to overcome these problems is given. Finally, future trends to be expected in this area are discussed.

Safety considerations in clinical engineering.

Clinical engineering is a professional specialty which emerged in the 1960s primarily in response to concern about the hazard of electrocution in hospitals. It has already developed to encompass a much broader range of topics affecting the safe and effective use of technology in health care. However, safety remains as one major focus of clinical engineering. In this paper, issues arising in relation to electrical safety and treated in considerable detail. The next major safety area of concern involves medical gases. This area is newer and, consequently, the issues are less well resolved and therefore it is treated in less detail. Other safety issues, which are numerous, await review at some time in the future.

Hand problems in an accident and emergency department.

We present a prospective study of 1,074 consecutive hand problems which were presented to our Accident and Emergency Department over an eight week period. This number accounted for 1 in 5 of all accident attendances. Over 40% of these patients were males aged 12-29 years. Nearly half of the hand patients presented within two hours of injury. Over half the patients needed only reassurance or a simple dressing but 55 patients (5%) had to be admitted for surgery.

Bronchogenic carcinoma, leukemoid reaction, marantic endocarditis, and consumptive thrombocytopathy.

This paper details the simultaneous occurrence of a severe leukemoid reaction, non-bacterial thrombotic endocarditis (NBTE) (marantic endocarditis), and a consumptive thrombocytopathy without signs of micro-angiopathic hemolysis on peripheral blood smear in a patient with terminal metastatic, undifferentiated, large cell bronchogenic carcinoma. The case is presented and the condition is discussed in detail.
