Optimal resources for primary prevention of atherosclerotic diseases. Atherosclerosis Study Group.

This statement is an update of the 1970 Inter-Society Commission for Heart Disease Resources ( ICHD ) report, "Primary Prevention of the Atherosclerotic Diseases." The charge to the Study Group was to assess relevant new data and, where the evidence is less than definitive, to formulate conclusions and recommendations based on best judgment. Current developments are reviewed, and issues raised in response to the earlier recommendations are considered. Recommendations are intended to serve as a guide for individual behavior, physician practice, and the formulation of public health policy.

Clinical breast examination and breast self-examination experience in a family practice population.

A patient questionnaire, chart audit, and resident questionnaire were used to assess clinical breast examination and breast self-examination experience in a family practice patient population. It was found that approximately 50 percent of the women studied reported annual routine breast examinations during a five-year study period. However, the residency program was responsible for providing or documenting annual clinical examinations in only ten percent of the population. Although 99 percent of the women knew about Breast Self-Examination (BSE), only 19 percent practiced monthly BSE. A positive association was found between the physician's active teaching of BSE and the patient's confidence in and regular practice of BSE. The low number of annual clinical examinations and low performance of BSE may be explained partially by the physician's setting too narrowly the parameters of when a clinical breast examination and BSE teaching could be done appropriately, ie, a pap smear/pelvic or general examination. A more aggressive approach by the physician may increase the number of women who get routine clinical breast examinations and who supplement them by monthly BSE.

Evaluation of the effectiveness of a high school course in cardiovascular nutrition.

High school biology students participated in a cardiovascular nutrition education program. The program was evaluated for its effect on the student's knowledge, attitudes, reported eating behavior, and serum lipid levels by matching individual's pre-and post-program results. There was a significant improvement in cardiovascular nutrition knowledge and attitudes toward a low cholesterol pattern of eating. There was also a signigicant improvement in reported eating patterns. However, the rise in serum cholesterol levels one year later did not differ from the rise in serum cholesterol levels in a control school. Improvement in attitudes and reported eating behavior was significantly greater for students whose initial cholesterol level was in the upper quartile of U.S. 16-year olds.
