Late onset and very mild course of Xp21 Becker type muscular dystrophy.

We report a case of late onset of Becker's muscular dystrophy (BMD), diagnosed at the age of 60, which showed a very mild clinical course. Remarkably, the immunohistochemical pattern did not show significant alterations, while Western blotting disclosed low molecular weight dystrophin. DNA analysis showed a deletion of the exons 45-53 of the Xp21 gene, which is fairly typical of Becker's muscular dystrophy but not predictable of clinical course. The possibility of Xp21 muscular dystrophy must be considered in all myopathies of uncertain cause, also in elderly patients.

[Preventive anti-epileptic action of taurine administered by direct cortical perfusion].

To test taurine preventive antiepileptic effect a 90-minute taurine cortical perfusion was made to 8 cats in the left sensory-motor area. A cobalt lesion was then made in the same area following routine. In all cats the beginning and subsequent development of the interictal and ictal epileptic picture were remarkably delayed in comparison with a control group of animals without taurine pretreatment. The taurine preventive action, as the consequence of its brain uptake, is discussed in comparison with the lack of taurine preventive action, when administered intraperitoneally (MUTANI et al., in press). The possibility of the taurine uptake by the normal brain and, therefore, of the prevention of the epilepsy would be related to the different anatomo-physiologic features of the barriers met in the two routes of administration.

[Bisynchronous discharge of symmetrical cortical epileptogenic foci and thaurine resistence (author's transl)].

The possible preventive action of taurine administered through a cortical perfusion, has been studied in bilateral and simmetrical foci with bisynchronous discharge. The data obtained showed that: 1)taurine preventive action is less remarkable in bilateral and simmetrical foci; 2) when administered trough the appropriate route, taurine always has a preventive antiepileptic effect. Our data, compared to those of Marcus e al., confirme that bisynchronous discharge, resistant to taurine preventive and terapeutic action, is the obligatory expression of interactions of bilateral and simmetrical foci.

[Usefulness of neuropsychological studies in the documentation of functional damage in a case of deep frontal lesion].

A case of patient with deep frontal brain haemorrhage is reported. The lesion is in the striato-caudate region extended to the anterior arm of the internal capsule. The neuropsychological survey is reported: it shows an impairment in functions implying the central processing while not so markedly spoiled are functions classically considered depending on the damaged areas. A SPECT investigation shows an extension of the functional impairment corresponding to the neuropsychological data.

[Antagonism between focal epilepsy and taurine administered by cortical perfusion].

The therapeutic action of taurine cortical perfusion was tested in cats affected with Premarin and cobalt cortical epileptogenic foci. In all animals taurine provoked the disappearance of EEG epileptic abnormalities. In the case of Premarin focus the effect appeared more quickly than in the cobalt one. This different time-course, according to previous reports on the antiepileptic action of the parenteral administration of the amino acid, suggests the hypothesis of a taurine direct inhibitory action against Premarin focus and, on the contrary, a mediated action towards the cobalt's. The latter might be related to the metabolic production of some taurine derivative.

Neurophysiological mechanisms underlying epileptogenesis.

Several factors play a role in the transition from normal to interictal and ictal neuronal activity. While classical data in neocortex suggest that paroxysmal depolarization shift (PDS) accompanied by burst firing (BF) is a giant EPSP, recent data in hippocampal slices show that CA3 neurons are intrinsically capable of generating PDS-BF, mainly due to Ca2+ inward current. Factors of transition from normal to interictal activity are changes in transmembrane ionic currents, GABA disinhibition and BF synchronizing effect. Factors of transition from interictal to ictal activity are changes in extracellular concentration of Ca2+ and K+, cholinergic modulation and BF axonal generation.
