Psychotropic drugs: another survey of prescribing patterns.

We surveyed the psychotropic drug treatment of our hospital's 220 patients, and compared our findings with those of the only previous hospital survey in this country which showed much allegedly inappropriate prescribing. Using simple classifications of the patients' diagnoses and of the drugs prescribed, 30 per cent of our prescriptions appeared inappropriate in the absence of knowledge of the patients' symptoms. With such knowledge we show that only 2 of our 460 prescriptions were inappropriate.

Position effect in translocation (2;8) in acute lymphocytic leukemia with kappa light chain immunoglobulin expression.

We report a correlation between t(2;8) translocation in acute lymphocytic leukemia and kappa light chain immunoglobulin production. Since the kappa chain genes are on chromosome #2, this, as well as data on Burkitt lymphoma, points to the possibility of position effect on the level of gene action. Chromosome #2 in the translocation together with chromosome #8 is concerned with malignancy, while the normal homologous chromosome #2 transcribes kappa chains. This model applies to B cell leukemias and lymphomas with changes in chromosome #2 which will predictably express kappa chains. The model also applies to B-cell malignancies with changes in chromosome #22 which will predictably express lambda chains.

Complex cytogenetic findings in acute leukemia.

Although certain types of acute nonlymphocytic leukemia (ANLL) are now characterized by specific chromosome translocations, certain other cases of ANLL elude simple cytogenetic classification. We describe here a case of acute myelomonocytic leukemia with a complex and unusual karyotype picture including double minutes, three definite translocations, two other possible translocations, and additional morphologic and numerical chromosome changes. The three definite translocations were t(2;11), t(2;12), and t(5;15), fitting the criteria of major karyotype abnormalities. These complex and unusual chromosome findings may relate to the rapid course of the disease.

Acute myeloblastic leukemia (AML) with t(6;9) (p23;q34): a specific subgroup of AML?

A case of acute myeloblastic leukemia (AML) of M2 type in the FAB classification without Auer bodies in the leukemic cells was shown to have t(6;9)(p23;q34) in the marrow cells. Four hematologically similar cases with identical karyotype changes have been published. We propose, in support of others, that this may constitute a subgroup of AML characterized by a translocation between chromosome #6 and #9.

Employability of the mentally disabled in the 1970s.

Of 156 discharged patients, the forty who succeeded in open employment had shown before they left hospital a combination of significantly less clinical disability, less abnormal behaviour, better work performance and better relationships with other people. We specify the approximate levels of these variables that were required to get and hold a job in the 1970s.

Predicting the outcomes of psychotherapy by the Penn Helping Alliance Rating Method.

The Penn Psychotherapy Project reports further progress in its search for the factors predicting the outcomes of psychotherapy through the construction of the Penn Helping Alliance Rating Method. This method is based on the following two types of patients' statements during psychotherapy sessions: type 1, that the therapist is helping the patient, and type 2, that the patient and therapist are working together in a team effort to help the patient. The scales were applied reliably to the transcripts from ten patients who had improved the most and ten who had improved the least among the 73 in the project. Significant predictive correlations were found for the early helping alliance measures (eg, with status at the termination of treatment and with the composite of "success, satisfaction, and improvement"). In contrast, other theoretically important treatment variables were not significant predictors.

Family Walk-In Centre--Eaton Socon: evaluation of a project on preventive intervention based in the community.

The Family Walk-In Centre opened November 1978 as a pilot project at Eaton Socon in Cambridgeshire with the aims: to encourage parents to visit the centre on "their terms," including parents who have abused or who fear they will abuse or neglect their child who may be difficult or handicapped, or because of chronic or acute family problems; further to mobilize "Self Help" groups and offer professional support. "Help" offered to the families included informal and formal group work, family casework and education in social and physical skills. Volunteer and professional support was given at the Centre and at home. Close liaison with specialist agencies was maintained to enable families to receive appropriate help. Periodic reviews and visits were made to maintain contact. An analysis of 50 families who attended the Centre between October 1979 and May 1980 was made to assess: the attendance and reattendance rate which indicated their acceptance of help offered and their motivation to use the Centre; the types of problems presented to the Centre, the types of action taken, the progress made when reviewed after three months and the degree of involvement by the volunteers. A final analysis will comprise 100 families. A preliminary summary found: significant reduction in the registration of new cases of child abuse; self-referrals exceeded significantly other modes of referral; loneliness and parenting difficulties formed the major proportion of families seeking help; 48% of families continued to attend for 3 months or more; work done centered upon discussion of parenting problems in 30% of the cases and involvement in activities with other families in 28%; 18% were assessed as showing improved child care after 3 months.(ABSTRACT TRUNCATED AT 250 WORDS)

Prognosis in chronic mental disability.

In a replication study three performance variables measured at the beginning of 200 patients' rehabilitation courses were combined to form an index to quantify the degree of initial disability. The Index score was significantly associated with the ten-year outcome, and hence has predictive value. With it a level of disability can be specified beyond which a patients' prospect of resettlement is predictably hopeless from the outset, despite prolonged and intensive rehabilitative efforts. On the other hand, many less disabled patients cannot achieve resettlement unless such efforts are made. There are, therefore, humanitarian and economic reasons for seeking to distinguish these two groups.

Renal cortical levels of adenosine triphosphate: restoration after prolonged ischemia by in situ perfusion of ATP-MgCl2.

The metabolic consequences of low-temperature kidney preservation by pulsatile perfusion and cold storage were evaluated in canine kidneys by serial adenosine triphosphate (ATP) determinations. Serial cortical ATP levels were determined (1) after optimal harvest, (2) after 60 minutes of warm ischemia, and (3) after 60 minutes of warm ischemia followed by an intra-aortic infusion of ATP-MgCl2. After 24 hours of preservation, cortical ATP levels were significantly greater when pulsatile perfusion was used-both after optimal harvest and after 60 minutes of warm ischemia. Cortical ATP levels were significantly greater at 24 hours in both perfused and cold-storage kidneys when an intra-aortic infusion of ATP-MgCl2 was used following ischemia. These results may explain the 24-hour limitation of cold storage as well as its inferior results following ischemia injury. Furthermore, since ATP is critical in cellular metabolism, the technique of intra-aortic ATP-MgCl2 infusion warrants further investigation in the field of organ preservation.

Improved survival of patients with glioma in the CT era.

At the RoyaL Brisbane Hospital, 143 patients with intracranial gliomas were analyzed in a retrospective study, to evaluate the impact of the CT scanner upon their diagnosis, management and survival. 95 were diagnosed before and 48 after CT became available. There was a significant reduction in the number of invasive diagnostic procedures performed in the CT era; cerebral angiograms by 67%, pneumoencephalograms by 82% and exploratory craniotomies by 65%. However, the total number of diagnostic investigations and the time taken to establish a diagnosis of glioma were not significantly decreased. Surgery was performed less frequently and with a lower complication and mortality rate than in the pre-CT era; there was a slightly increased use of chemotherapy. The number of patients having arteriography as a follow-up procedure was reduced by 80% and no follow-up pneumoencephalograms have been performed since CT was introduced. The average duration of hospitalization has remained unchanged. Survival times has increased significantly during this time, most markedly in the younger groups. The possible role of CT in this improvement is discussed.

In situ cold perfusion of kidneys for transplantation. An experimental and clinical evaluation.

Cadaver kidneys from donors who have sustained cardiac standstill are often unsuitable for transplantation due to prolonged warm ischemic time. In an attempt to increase the salvage rate of these kidneys, the efficacy of in situ intra-aortic cold perfusion in producing immediate function of allografted kidneys was assessed in the nonheart beating canine model. In the first experiment, kidneys harvested after cardiac standstill and immediate intra-aortic cold perfusion were demonstrated to function equally as well as kidneys harvested "optimally." In the second experiment, evaluation of intra-aortic cold perfusion on renal core temperature demonstrated rapid cooling of these kidneys to a protective range. Early clinical results in donors whose kidneys were removed after cardiac arrest demonstrate that most of the kidneys allotransplanted after in situ intra-aortic cold perfusion functioned immediately. These preliminary experimental and clinical results demonstrate the effectiveness of in situ intra-aortic cold perfusion. Wider application of this technique to donors with cardiac standstill should increase the available organs for transplantation.

Effect of Mycobacterium bovis BCG vaccination upon Mycobacterium lepraemurium infection.

Mice were infected with 10(8) Mycobacterium lepraemurium in the footpad (unsuppressed mice), and some of these animals were concurrently given 10(9) heat-killed M. lepraemurium intravenously (suppressed mice). These groups of mice were preimmunized with 10(7) viable organisms of Mycobacterium bovis BCG by several routes. BCG inhibited the proliferation of M. lepraemurium in the unsuppressed mice, but not in the suppressed mice. In effect, the intravenous administration of heat-killed M. lepraemurium suppressed the immunity to M. lepraemurium that BCG vaccination had engendered. BCG did not protect normal mice against intravenous infection with M. lepraemurium. It appears that normal mice against intravenous infection with M. lepraemurium. It appears that the inhibitory effect of BCG vaccination upon M. lepraemurium infection is due to cross-reactive immunity rather than to nonspecific immunity or immunopotentiation. Thus, the route of BCG vaccination was immaterial, and vaccination 12 weeks before M. lepraemurium infection was as beneficial as vaccination 4 weeks before infection. Moreover, spleen cells from M. lepraemurium-immunized mice conferred adoptive immunity to BCG. The implications of this study for the use of BCG as a prophylactic and therapeutic agent in human leprosy are discussed.
