Clinical and histologic aspects of chronic delta hepatitis.

Clinical and histologic valuation of delta chronic hepatitis is performed by a comparison between 46 HDV markers seropositive patients and an analogous group of HBsAg seropositive and delta system negative patients. It is possible to point out a greater seriousness of clinical and biohumoral situation during a HDV induced disease which is joined up with a more marked histologic damage and a greater inflammatory activity. The presence of specific viral antigen in hepatocyte nucleus suggests a delta-virus direct cytopathic effect.

Retrobulbar optic neuritis in a patient with acute type B hepatitis.

A case is described of retrobulbar optic neuritis in a 33-year-old female with acute type B hepatitis. The ocular distress, principally affecting the left eye, followed the normalisation of liver function tests and subsided after steroid therapy. At the onset of ocular symptomatology, complement activation, involving both classic and alternative pathways, and high levels of circulating immune complexes were present. HBsAg was positive. No relapse was observed during follow-up for 3 years. An association between HBV-infection, optic neuritis and an immune complexes-mediated neurotoxic activity are hypothetized.

Mixed Schistosoma infection in an asymptomatic patient.

We report a case of a male, 26 years old Egyptian patient, who was investigated for a long-lasting hepatomegaly and altered transaminase levels. No symptom was reported. Liver biopsy and intestinal biopsy were positive for S. mansoni ova. Urine test initially showed negativity for S. haematobium ova, which only appeared with subsequent investigation, in spite of lack of urinary symptomatology.

A purified polysaccharide isolated from Candida albicans induces antibody response in vitro by human peripheral blood lymphocytes and discriminates between sera from normal and Candida albicans-infected individuals.

A purified polysaccharide extracted from Candida albicans (MPPS), stimulates in vitro synthesis of specific antibodies by human peripheral blood lymphocytes. These antibodies can be detected by a sensitive enzyme-linked immunoassay. The same assay can be applied to the quantitation of anti-candida antibodies in serum. Statistically significant differences were found between sera of normal and candida-infected individuals.

Survey of 259 cases of American cutaneous leishmaniasis in Nicaragua.

The clinical aspects of mucocutaneous leishmaniasis in Nicaragua are examined for the first time. Data regarding 259 cases were collected by the authors in two health centres of a central region of the country. On the basis of clinical observations the multiple aetiology (i.e. L. mexicana complex and L. braziliensis complex) of the lesions is suggested. The problem is of increasing importance in Nicaragua because of greater public awareness and new settlements in the endemic area and so it deserves further research.

Immunohistochemical staining on hydroxyethyl-methacrylate-embedded tissues.

Hydroxyethyl-methacrylate (GMA) embedding has recently been proposed for light microscopy studies. In the present investigation extracellular protein antigens were localized on GMA-embedded renal biopsy tissue. Conventionally frozen sections were compared with GMA sections from 55 renal specimens for the detection of extracellular protein antigens. Sections were directly stained with fluorescein- or peroxidase-conjugated antisera against immunoglobulin (Ig) G, IgA, IgM, C3, C1q, and fibrinogen. Results obtained using these two methods showed a 74-89% agreement, depending on the antigen under study. Some discrepancy between GMA and frozen sections was observed in three cases of renal amyloidosis and those cases presenting focal or trace reactions; the differences did not, however, influence the diagnosis. Prerequisites for antigen recovery on GMA sections were a) choice of fixative; b) abrupt dehydration of specimens; and c) treatment of sections with nonspecific protease. The improved localization and the lower background staining obtained led to easy and immediate detection of antigens on GMA sections despite the reduced antigenicity due to the embedding process.

Sporadic acute non-A non-B hepatitis complicated by aplastic anemia.

A 19-year-old woman developed acute jaundice and abnormal liver tests showing acute viral hepatitis, presumably non-A non-B. The viral hepatitis, on the 4th day after admission, was complicated by aplastic anemia. Therapy with hydrocortisone promptly instituted was ineffective and a bone marrow transplantation was not possible because we could not find a compatible donor. Sixty-five days after admission the patient died of infection by Candida tropicalis. Postmortem examination showed total medullary aplasia with massive impoverishment of all the lymphatic structures and multiple fungal infarctions in the myocardium, lungs, spleen, kidneys, and liver.

Immediate hypersensitivity to Cryptococcus neoformans.

The role of the capsular polysaccharide in anaphylactic reactions to Cryptococcus neoformans was investigated. Groups of mice were sensitized with viable cells of either a moderately encapsulated strain of C. neoformans or a non-encapsulated variant. Anaphylactic reactions were observed in both groups of mice to a similar extent when challenged with whole cells. Mice sensitized with the encapsulated strain and challenged with homologous polysaccharide showed only mild hypersensitivity symptoms. Mice sensitized with either the encapsulated or the nonencapsulated strain showed cross-reactivity when challenged with killed cells of the heterologous strain. These data indicate that the capsular polysaccharide plays a minor role in anaphylactic reactions to C. neoformans and that the sensitizing antigen is probably located in the cell wall of the yeast.
