[What is new in plastic surgery of the head and neck for the practicing otorhinolaryngologist?].

On examples the plastic facial surgery is shown as an multidisciplinary territory, whereby main points are set. Most important for the oto-rhino-laryngology is the ear and nose correction as whereas the possibility to use collagen injections, the problems of secondary Rhinoplastics, the esthetic corrections in the area of the throat and head (dermabrasio, eye lid corrections, face lifting, hair transplantation) and considerations about education and advanced training. The maxillo facial surgery puts out the dento-alveolic dysgnathies, the ophthalmology the eye lid corrections. The experimental surgery can contribute to the still open questions, whether in the future in plastic operations allogene transplantates can fill the place of autogenous material or not.

[Open clinical trial of a new oral galenic form of ketoprofen].

Thirty-two patients with rheumatoid arthritis and several other rheumatic diseases were entered into a randomized study to investigate the correlation between the pharmacokinetics and clinical effectiveness of Bi-Profenid 150 mg. Each patient was given, in a variable order, one 150 mg Bi-Profenid tablet twice daily for 2 days, and two 50 mg Profenid capsules each morning and noon with one 100 mg suppository each evening for 2 days. Dosage convenience, effectiveness on pain during the second part of the night and resolution of morning stiffness were considered better with Bi-Profenid by 23 patients and with conventional forms by four, while five reported no preference. In 15 patients treated for one month, only 5 reported mild side-effects which did not require withdrawal from the study. The patients' preference appears to be a result of the bioavailability of Bi-Profenid.

Intercellular coupling in frog heart muscle. Electrophysiological and morphological aspects.

Passive electrical parameters of bullfrog atrial trabeculae were measured in a single gap arrangement. Attention was focussed on the resistance of internal longitudinal pathway. The influence of external Ca2+ depletion was tested using EGTA as chelating agent. Morphometry of trabeculae, fine structure of junctional complexes, and distribution of membrane-bound Ca were investigated by light and electron microscopic methods. The specific internal resistance to longitudinal current flow was 523 omega cm with normal Ringer as perfusing fluid and 1140 omega cm in EGTA-containing solution. These values are considered to represent the sum of myoplasmic and junctional resistivity. Morphometrical studies indicated an interstitial space of 12%, a mean cell length of 358 micron, and a mean cell diameter of 3.2 micron. In freeze-fractured preparations junctional structures were observed in the form of "atypical gap junctions" consisting of 10 nm particles arranged in a circular or linear array. The number of gap junctions was estimated to range between 20 and 50/cell which is equivalent to a junctional area of 0.01 or 0.03% of total surface area. A mean number of 55 particles/gap junction was calculated. After 20 min of exposure to EGTA the majority of junctional complexes were converted to clusters; the number of particles/gap junction was not significantly altered. The fluorescent dye CTC was used as a probe for membrane-bound Ca of isolated living cells. In normal Ringer a strong fluorescence was seen at the cell surface and in different intracellular compartments. With EGTA both superficial and internal fluorescence disappeared completely. From a combination of electrical and morphometrical data the resistance of intercellular junctions was calculated. Under normal conditions the specific resistance of junctional membrane amounted to 0.4 omega cm2 and the resistance of an individual connection was of the order of 10(11) omega. With EGTA, the respective values were increased by about 230%. The mechanism underlying this depression of junctional conductance is not clear. It seems not related to a rise of cytoplasmic free Ca2+. The EGTA-induced increase in internal resistance was reflected by a decrease of the length constant of a bundle. The nature of "atypical gap junctions" and their relation to tight junctions are discussed. It is concluded that the junctions observed in frog atrial muscle are analogous to gap junctions of insect or mammalian cells in spite of the different size and arrangement of the particles. A theoretical model is presented for the electrical behaviour of a bundle in a single gap arrangement.(ABSTRACT TRUNCATED AT 400 WORDS)

[Effect of elevated levels of natural radioactivity on the fecundity of male mice].

Male mice of the BALB/c strain were placed on the floor of a hut built at a site of Southwest France where the dose rate of gamma radiation amounts to about 10 mrad/h. Controls were kept near the radioactive site under comparable conditions. The doses of gamma rays received during a seven month period ranged from 0.1275 rad for the controls to 45 rad and 63 rad for the exposed animals. Due, possibly, to an effect on the libido exposure to 45 rad increased the fertility whereas the animals given 63 rad produced less offspring than the controls.

[Our concepts in the operative closure of septal perforations].

This is a short review of our concepts of the operative closure of small, medium sized and large septal perforations. We describe the details of our personal technique of the closure of medium sized supra- and retrocolummellar perforations with a vestibulum skin-mucoperichondrium-cartilage-flap.

[A case of double monster--duplicitas anterior with diprosopia].

Report on a delivery of a double monster, duplicitas anterior with diprosopia. The course of delivery is compared with literature. The exclusion of malformations in early pregnancy is possible and necessary. Perinatal mortality can be reduced by abortion in time. The psychic burden of women concerned needs a special regard.

Pharyngo-oesophageal reconstruction by myocutaneous flaps.

With the progressive development of ablative surgery of pharyngolaryngeal cancer in recent decades, the demands for an effective one-stage pharyngo-oesophageal reconstruction have spawned a wide variety of imaginative techniques. The pectoralis major myocutaneous flap has proven to be the most effective for immediate reconstruction of the cervical oesophagus after pharyngolaryngectomy in which the inferior limit of resection is at most 3 cm below the oesophageal orifice. This reconstruction may be circumferential, partially circumferential or only partial in the case of large pharyngostomas.

Serologic survey for leptospirosis in Arizona beef cattle in 1981.

Sera from 1,215 beef cattle in Arizona were evaluated by leptospiral microscopic agglutination test in 1981. Over 25% had agglutinins to greater than or equal to 1 of 5 serovars of Leptospira interrogans used as antigens (canicola, grippotyphosa, hardjo, icterohaemorrhagiae, and pomona) at a titer of greater than or equal to 1:100, and 8.2% had titers of greater than or equal to 1:400 to greater than or equal to 1 serovars. The most common serovar to which reactions were detected was hardjo; agglutinins were detected at titers of greater than or equal to 1:100 in 14.3% and of greater than or equal to 1:400 in 5.5%. Cross reactions were rare at serum dilutions greater than or equal to 1:100 (2%) and extremely rare at greater than or equal to 1:400 (0.7%). Because vaccination with leptospiral bacterins is seldom practiced in Arizona beef cattle, a titer of greater than or equal to 1:100 may be useful in estimating incidence and prevalence of the disease and as an aid to diagnosis of leptospirosis.

[Functional plastic surgery of the nose].

The authors emphasize the importance of the extramucosal approach to the septum and the cartilaginous and bony vault in the correction of the crooked nose, for elongation of the pig-snout nose with retracted tip, and for closure of septal perforation. Furthermore, they present their own procedure for surgical treatment of stenoses of the nares as well as for huge tissues losses of the septum.

[Etiologic aspects of Reiter syndrome. Role of yersinial infections (author's transl)].

Reiter syndrome occurs as a reaction to various infections, either by intracellular microorganisms (Chlamydia, Mlycoplasma) or by intestinal bacteria (dysenteric bacilli, Salmonella, Campylobacter, Klebsiella, Yersinia). Four personal observations of Reiter syndrome following yersinial infections (Yersinia enterocolitis in two cases and Yersinia pseudotuberculosis in the two other cases) are reported. Data concerning the yersinial etiology of Reiter syndrome are reviewed.

A comparison of two interview schedules. The Schedule for Affective Disorders and Schizophrenia-Lifetime and the National Institute for Mental Health Diagnostic Interview Schedule.

Two structured interviews, the Schedule for Affective Disorders and Schizophrenia-Lifetime (SADS-L) and the National Institute of Mental Health Diagnostic Interview Schedule (NIMH-DIS), were compared as methods of reducing information variance in the diagnostic process. Forty-two patients newly admitted to an alcohol treatment unit were randomly selected and were independently interviewed using the SADS-L and NIMH-DIS. The order of the interviews was random and they were separated by three to four days. Interrater reliability for each interview schedule was calculated using the kappa statistic and was found to be high. The degree of diagnostic concordance between the two interview schedules for several diagnostic categories was also found to be high.
