Intradural extramedullary spinal metastasis from an ovarian carcinoma.

Intradural extramedullary metastases from tumours are a rare occurrence, accounting for less than 2 percent of all spinal metastases. We report a patient who presented with progressive paraparesis one and a half years after being operated for an poorly differentiated serous cystadenocarcinoma. Tumour mass decompression was carried out and the intradural tumour metastasis was removed. Histopathology revealed an identical appearance to the original ovarian tumour. This is the first reported case of an intradural extramedullary metastasis from a poorly differentiated serous cystadenocarcinoma of the ovary.

Dieulafoy's lesion: a rare cause of massive upper gastrointestinal haemorrhage.

Out of 900 cases of Upper Gastrointestinal haemorrhage seen during 1985-1989, 6 (0.67%) cases had Dieulafoy's lesion as the causative factor. All patients presented with massive upper gastrointestinal haemorrhage. The mean age of the patients was 46.3 year (32-60 yrs) and 4 were males and 2 females. No consistent associated medical factors could be identified. The diagnosis was established by emergency endoscopy which showed an active arterial spurter in 4 patients and located the bleeding site to be close to the fundus in other 2 patients. Injection sclerotherapy tried in 3 patients was not successful. Four patients had bleeding lesion along the greater curvature close to the fundus and two had on posterior wall but all within 6 cm. of gastroesophageal junction. All patients underwent curative emergency surgery with wedge resection of the lesion. We conclude that Dieulafoy lesion should be suspected in a patient with massive, recurrent and obscure upper gastrointestinal bleeding. Emergency endoscopy for diagnosis and prompt surgical intervention can cure the lesion which is potentially fatal if untreated.

Effect of treatment on systolic time intervals in systemic arterial hypertension.

Twenty-six hypertensive patients and 25 control subjects were studied, and their systolic time intervals (STI) were measured. It was found that an alteration in STI, signifying left ventricular dysfunction, sets in most of the hypertensives earlier than any evidence of left ventricular hypertrophy (LVH) by clinical, radiological or electrocardiographic criteria. The parameters affected are the PEP interval which shows prolongation and PEP/LVET ratio which is increased. After treatment, the left ventricular performance was found to improve in those who did not reveal evidence of LVH. Thus, STI may be a sensitive indicator of early left ventricular dysfunction in hypertensives and a useful guide to assess improvement after treatment at a stage when other non-invasive methods are not so useful.

Characterisation of the Indian strain of duck hepatitis B virus (DHBV) and development of a carrier duck colony for antiviral drug testing.

Duck hepatitis B virus (DHBV) infected carrier ducks serve as a useful model for testing anti-hepadnavirus drugs. This needs a well characterised duck hepatitis B virus strain. We cloned and sequenced the complete genome of duck hepatitis B virus strain of Indian origin. It was 3021 nucleotides in length and had all the recognisable open reading frames (Polymerase: 20-2530 nucletides, Surface: 693-1787 nucleotides and Core: 2518-412). Using an inoculum of 50 microliters serum containing 1 x 10(11) virus particles/ml, we could infect 80% of one day old ducklings and develop a duck colony.

A power spectral study of surface EMG of muscles subjected to non-repetitive task.

For a proper understanding and interpretation of the changes in the power spectral characteristics of surface EMG from any fatigued muscle of a worker employed on repetitive assembly line tasks, it is important to study first the changes in the spectral characteristic of the surface EMG of the same muscle of the worker assigned with non-repetitive tasks. The information obtained from the EMG study with non-repetitive tasks would serve as a control to identify and evaluate the changes in the power spectral features of the surface EMG of the muscle due to, in particular, the repetitive nature of the task under similar work environment. In this study the EMG signals from thenar fascia and biceps of two healthy workers engaged in non-repetitive tasks in an electronic industry have been recorded in the morning (before the start of the work) and again the evening (after the completion of day's work). The frequency wise distribution of the number of peaks (NP) and the power content (PC) of the signal have been evaluated within the range of 60-200 Hz. It is found that there is no significant difference in the frequency wise distribution of NP and PC in the power spectrum of the EMG signals recorded in the morning and evening.

Single lung Rastelli repair.

A four year old severely cyanotic child was diagnosed to have double outlet right ventricle, large subpulmonic ventricular septal defect, severe pulmonic valve stenosis with absent left pulmonary artery and bilateral superior vena cavae without any connecting vein. A Rastelli type repair was performed utilizing the right pulmonary artery as the only run-off for the right ventricle. Immediate post-pump right ventricular (RV) pressures were 90 percent of the systemic. Six months later, echocardiographically estimated RV pressure was 60 percent of the systemic. The child is doing well at 6 months follow-up.

Calculation of 6-hour D/P creatinine ratio from the 4-hour peritoneal equilibration test. The effect of dwell duration on the results.

OBJECTIVES: Since the introduction of the peritoneal equilibration test (PET), the 4-hour dialysate/plasma creatinine (D/P Cr) has been used by several authors for determining continuous ambulatory peritoneal dialysis (CAPD) prescriptions. However, the results have been unsatisfactory because the 4-hr D/P Cr does not accurately reflect the D/P Cr in 24-hr collections. The PET and the 24-hr dialysate collections differ in the duration of dwell and the tonicity and volume of dialysate, all of which influence the equilibrated D/P Cr. It can be assumed that the D/P Cr in 24-hr collections in these patients is closer to a 6-hr D/P Cr. Because a 6-hr PET is inconvenient, we developed a mathematical model to calculate the 5- and 6-hr D/P using the results of a standard PET. DESIGN: In a retrospective analysis, D/P Cr ratios in 24-hr collections and D/P Cr ratios calculated from a mathematical formula were correlated. Using a mathematical model, the data collected fit an exponential relation of the type D/P = a(1-e-t/tau). The values of a and tau are unique for a given patient and were determined using a nonlinear regression technique. The formula performed well on our published data-the true and predicted 6-hr D/P Cr being 0.696 and 0.71, respectively. SETTING: The University Hospital and Clinics, Dalton Cardiovascular Research Center and Dialysis Clinic, Inc., Columbia, Missouri. PATIENTS: All CAPD patients on four 2-L exchanges/day at the time of the 24-hr collections were included. INTERVENTIONS: None. MAIN OUTCOME MEASURES: Closeness of 4-hr and 6-hr D/P Cr values to those of 24-hr ratios. RESULTS: The study group comprised 74 patients (age, mean +/- SEM: 56.4 +/- 1.8 yr) with 80 PETs and 145 (24-hr) collections. The interval between the two tests was 8.3 +/- 0.9 months (0-48.7 months). The median 24-hr D/P Cr of 0.760 did not differ significantly from the predicted median 6-hr D/P Cr of 0.755. A subgroup analysis, based on transport type, showed that this relationship was most precise in the high-average transporters. The predicted 6-hr D/P Cr was within 10% of the 24-hr D/P Cr in 48% of patients and within 20% in 77% of patients. The margin of error was greatest in the low transporters. CONCLUSIONS: To conclude, the 4-hr D/P Cr from a PET cannot be used interchangeably with the D/P Cr in the 24-hr dialysate collections, hence, the clearances calculated thereof will be inaccurate. Using the proposed model, it is feasible to use the 4-hr PET results to obtain 5- and 6-hr D/P Cr values. In our study, using this model, the extrapolated 6-hr D/P Cr is similar to the D/P Cr in 24-hr dialysate collections only in the high-average transporters. Hence, the best way to determine clearances in peritoneal dialysis patients is still by collecting 24-hr dialysates.

Aortoaortic bypass: indications, techniques and results.

The standard surgical treatment of complex cases of aortic obstructions is difficult and sometimes even hazardous, thus necessitating the use of alternative surgical methods to manage these cases. Between 1986 and 1995, nine such patients underwent ascending aorta to descending/abdominal aorta bypass graft as an alternative procedure at a premier medical institution. There was no hospital death nor any significant morbidity. Preoperative systolic blood pressure in right upper limb ranged from 150 mm Hg to 230 mm Hg (mean 180.5 mm Hg) while postoperative systolic blood pressure in right upper limb ranged from 126 mm Hg to 150 mm Hg (mean 134 mm Hg), thereby showing marked improvement. Preoperative pressure gradient across the aortic obstruction ranged from 50 mm Hg to 120 mm Hg (mean 87.2 mm Hg). It was relieved in all except two patients who had resting gradients of 10 mm Hg and 12 mm Hg respectively. All the patients were relieved of their symptoms. After a mean follow-up of 33.3 months (range 6 to 108 months), all the patients are in New York Heart Association (NYHA) class I with evidence of good distal perfusion. This technique of bypassing the aortic obstruction has the added advantage of avoiding the complications associated with standard technique.
