We describe a development of a malignant hyperthermia (MH) syndrome, partially aborted by therapy, in a child with central core disease and congenital dislocating hips. Patients with central core disease appear to be more susceptible to MH; possibly those with elevated serum creatine phosphokinase levels, as in our patient, are especially susceptible. We review the clinical and pathologic aspects, possible pathogenesis, and treatment of the MH syndrome. An increased calcium level within the muscle fiber is suggested as the major cytodestructive factor, and that increase could be consequent to a plasmalemmal susceptibility to the provoking drugs hypothesized to be the basic defect in MH. Prevention of the full manifestations of MH is predicated on (1) a high index of suspicion in the search for history of anesthetic complications in the patient and his family, with or without evident neuromuscular disease, (2) recognition that there is a somewhat greater risk of MH developing in a patient who has certain "musculoskeletal" abnormalities or muscle weakness but that is not-except for central core disease-a classic clinicopathologically defined disease, (3) close monitoring of patients during anesthesia, and (4) if the syndrome develops, prompt therapeutic measures, including cessation of anesthesia.

Mitral regurgitation due to 'floppy' valves is frequently associated with areas of medical necrosis in the ascending aorta. Application of the aortic clamp to such an area during valve replacement may produce an intimal tear followed by acute dissection. This complication occurred in three patients and was treated successfully by repair of the tear in the two cases where the dissection was observed at the time of operation. Is is suggested that infrequent clamping of the aorta and careful control of the pefusion pressure may decrease the risk of intimal trauma and acute dissection.

Intramitochondrial loci for delta-aminolaevulate synthetase and ferrochelatase, the initial and final enzymes in haem synthesis, have been found in rat liver. Two different methods of fractionation were applied to mitochondria: (a) sonication and density-gradient centrifugation; (b) treatment with digitonin and differential centrifugation. Similar results were obtained with each technique. delta-Aminolaevulate synthetase is distributed similarly to two known matrix enzymes, malate dehydrogenase and glutamate dehydrogenase. Ferrochelatase is firmly bound to the the inner mitochondrial membrane. These results are considered in terms of the regulation of haem synthesis and in relation to mitochondrial biogenesis.

Striking correlations between structure and function are found in the visual cortex of Old World primates. These include the co-localization of glutamic acid decarboxylase (GAD, the biosynthetic enzyme of the inhibitory neurotransmitter, gamma-aminobutyric acid) with the mitochondrial enzyme, cytochrome oxidase (CO) in functionally distinct subcompartments of ocular dominance columns. We report here immunocytochemical studies with a monoclonal antibody (CAT 301) showing that the antibody recognizes an uncharacterized antigen on surfaces of some neurones in certain layers of the monkey striate cortex (area 17), and in certain parts of the cat and monkey dorsal lateral geniculate nuclei (LGN). Patches of immunocytochemically stained neurones and neuropil, apparent in layers III, IVB and VI of the striate cortex of normal monkeys, become even more clearly delineated in animals from which one eye has been removed. The antibody-stained patches in the three layers line up radially with one another in lines passing through the centres of ocular dominance columns (demonstrable by CO staining in layers IVA and IVC). In layers III and VI the patches coexist with CO-positive patches and, in the horizontal dimension, both antibody and CO-positive patches are aligned to form rows. Stained neurones in the monkey LGN are primarily in the magnocellular layers and in the cat LGN are confined to laminae A and A1, the inter-laminar plexuses, the perigeniculate nucleus and the medial inter-laminar nucleus. The antigen we have localized is associated with particular cell populations, some of which may correspond to a specific, physiological class.
