Detection of herpes simplex virus thymidine kinase and latency-associated transcript gene sequences in human herpetic corneas by polymerase chain reaction amplification.

Herpes simplex virus (HSV) latency in sensory ganglion neurons is well documented, but the existence of extraneuronal corneal latency is less well defined. To investigate the possibility of extraneuronal latency during ocular HSV infection, corneal specimens from 18 patients with quiescent herpes simplex keratitis (HSK) were obtained at the time of keratoplasty. Polymerase chain reaction (PCR) amplification followed by southern blot hybridization with a radiolabeled oligonucleotide probe was done to detect the presence of HSV-1 genome in these human corneal samples. Two pairs of oligonucleotides from the region of the HSV thymidine kinase (TK) gene and the latency-associated transcript (LAT) gene were used as primers in the PCR amplification. The DNA sequences from either the TK or the LAT gene were identified in 15 of 18 HSK corneas (83%). These results demonstrate that the HSV genome was retained, at least in part, in human corneas during quiescent HSV infection, giving further support to the concept of corneal extraneuronal latency.

Effects of gangliosides on the methionine uptake in crushed sciatic nerves of rats with alloxan diabetes.

Effect of gangliosides on the transmembrane transport of methionine in crushed sciatic nerves was studied in alloxan diabetic and control rats until the 4th day after nerve crushing. Methionine uptake rate decreases while time passes after injury in both control and diabetic rats, but the decrement is greater in diabetic rats than in controls. Ganglioside treatment causes an enhancement on the transmembrane transport in both diabetic and control animals, but the effect in time was greater in diabetic rats than in controls.

Pelvic fractures crash vehicle indicators.

Fracture of the pelvis often results in major hemorrhage and even death in victims of motor vehicle trauma. In the hospital emergency department, diagnosing a fracture of the pelvis is difficult because the patient may be unconscious or disoriented, have multisystem injury, may not demonstrate bruising or other physical findings, and because the attendant bleeding is often retroperitoneal. This paper develops information from a diagnostic source that is currently almost unused: the vehicle. Accident statistics files from the National Accident Sampling System are used to relate pelvic fractures and dislocations with vehicle information. Collision type, occupant seating position, intrusion, entrapment, restraint use, age, and sex are analyzed as they bear upon the likelihood of pelvic fracture. Linear discriminant analysis is used to form simple equations that predict whether the patient will have a fracture or dislocation of the pelvis.

Treatment of multiple myeloma with nasal spray calcitonin: a histomorphometric and biochemical study.

To evaluate the effect of calcitonin on the bone lesions of multiple myeloma, we studied 11 patients treated for 3 months with salmon calcitonin in nasal spray (200 IU) and 500 mg of elemental calcium/day. Pre- and post-treatment biochemical and histomorphometric parameters were compared to those of 12 patients treated for the same time with 500 mg elemental calcium alone. Both groups received the same hematological treatment. In the group treated with calcitonin there was a significant increase (P less than 0.01) in trabecular bone volume, cortical thickness, osteoid volume and osteoid seam thickness index and the osteoclast resorption surface fell significantly (P less than 0.01). There was also a decline (P less than 0.001) in corrected serum calcium and OHP/Cr, which accounts for the diminished bone resorption. The group not treated with calcitonin showed only significant changes in OHP/Cr which increase (P less than 0.05). Calcitonin was perfectly tolerated by all patients and our results show it to be useful in the treatment of bone lesions of multiple myeloma.

Injury control: a primer for physicians.

Injuries are a disease process that costs billions of dollars every year in direct and indirect costs. Despite recent emphasis on prehospital systems and trauma care, prevention of injuries remains an area underutilized by physicians. As a disease process, injuries are amenable to evaluation with epidemiologic tools and subject to prevention by modifying the transmission of energy to human beings. This may occur by either active or passive means of intervention. Physician leadership in the prevention of injuries is important, yet few physicians have training in injury control. The concepts of injury control are presented by defining the components that create injury and by examining the factors that affect each component. Strategies for identifying countermeasures are described and the merits of each of the applications of countermeasures are discussed. The physician must understand these basic concepts of injury control to an effective leader in developing strategies that mitigate injury in the community.

The dexamethasone suppression test in Alzheimer's disease and major depression: relationship to dementia severity, depression, and CSF monoamines.

Patients with Alzheimer's disease (AD) have been reported to have a rate of nonsuppression on the dexamethasone suppression test (DST) comparable to that of patients with major depression. With symptoms of depression being increasingly recognized in patients with AD, studying their DST response may provide clues to the etiology of the abnormal response in both diagnostic groups. A correlation between dementia severity and post-dexamethasone cortisol was found within the group of male, but not female AD patients. Within the group of elderly depressives, a correlation between post-dexamethasone cortisol and ratings of depression was found. Serum dexamethasone levels were not significantly lower in the nonsuppressors as compared with suppressors in either diagnostic group. Within the AD group, dexamethasone levels themselves correlated significantly with ratings of dementia severity and with the Wechsler Memory Scale score. Cerebrospinal fluid (CSF) 3-methoxy-4-hydroxyphenylglycol (MHPG) correlated positively with 4:00 pm post-dexamethasone cortisol level and with ratings of dementia severity in the AD patients. Findings are discussed in light of the known clinical and other biological similarities between AD and major depression, followed by a review of theories regarding the etiology of the hypothalamic-pituitary-adrenal abnormalities in these two illnesses.

Molecular weight dependence of the pharmacokinetic of hydroxypropyl methylcellulose in the vitreous.

The influence of molecular weight on the elimination kinetics of the potential vitreous substitute hydroxypropyl methylcellulose (HPMC) was studied using solutions of equal viscosity (15,000 cps) of HPMC of molecular weight 86,000 daltons (HPMC 86,000) and HPMC of molecular weight 120,000 daltons (HPMC 120,000). Three days after gas vitrectomy of the right eyes of 64 rabbits, the gas was replaced by 0.5 ml of HPMC solution (2.5% HPMC 86,000 in 39 and 2% HPMC 120,000 solution in 25); HPMC concentration was determined immediately afterwards and 7, 14, 30, 60 and 90 days later by the diphenylamine method, the left eyes being used as blanks. HPMC elimination exhibited order one kinetics, with a half life of 10.4 days for HPMC 86,000 and 38.4 days for HPMC 120,000. These results suggest that by choosing HPMC of suitable molecular weight it may be possible to control the residence time of the polymer in the vitreous cavity.

Immunohistochemical study of natural killer cells in tumor-infiltrating lymphocytes of primary intracranial germinomas.

A monoclonal antibody against the surface marker IOT-10 of natural killer (NK) cells was used to investigate the presence and distribution of these cells in a series of nine primary intracranial germinomas. In all of these tumors, IOT-10-positive NK cells were found in small numbers, mainly distributed among the tumor cells. The data obtained in the present study suggest that the presence of NK cells in primary intracranial germinomas can be influenced by factors other than the mere quantity of tumor-infiltrating lymphocytes.

Alcoholism and society.

The problems of alcohol abuse are rampant in society and permeate the Emergency Department experience. The cost to society as measured by social, medical, occupational, and legal effects is exceedingly high and probably underestimated. Emergency physicians have the opportunity to intervene in this vicious cycle and must learn their role in the process.

The role of serum CEA as a prognostic indicator in stage II and III breast cancer patients treated with adjuvant chemotherapy.

Carcinoembryonic antigen (CEA) values in 529 patients treated in two consecutive adjuvant chemotherapy protocols were analyzed to determine if CEA values correlated with disease-free status or prognostic utility. CEA values were evaluated preoperatively, before chemotherapy, at the conclusion of chemotherapy, and during postchemotherapy followup. The sensitivity of CEA for predicting disease recurrence was low; however, any abnormal CEA at the conclusion of chemotherapy and during followup significantly correlated with reduced disease-free and overall survival. A CEA value greater than or equal to 20 ng/ml at the end of chemotherapy or during followup was highly specific and a strongly positive predictor for the presence of metastases. Abnormal CEA values before chemotherapy that became normal at the conclusion of chemotherapy were associated with a significantly reduced recurrence rate. An abnormal CEA value obtained before or after adjuvant chemotherapy is clinically useful and can provide prognostic information.

Sphingosine inhibits phorbol 12-myristate 13-acetate-, but not serum-induced, activation of Na+/H+ exchange in mammalian cells.

Addition of serum to quiescent mammalian cells in culture initiates a series of events which culminates in DNA replication and cell division. One of the earliest events in this sequence of events is activation of Na+/H+ exchange, which can result in an increase in intracellular pH (pHin). The regulation of this change in activity is not known. Since treatment of 3T3 cells with activators of protein kinase C (kinase C) can result in an increased pHin, it has been hypothesized that serum stimulation of kinase C is responsible for activation of Na+/H+ exchange. Recently, sphingolipids have been discovered to inhibit kinase C both in vitro and in vivo. Therefore, we undertook the present study to ask whether or not inhibition of kinase C using sphingolipids prevents mitogen-induced alkalinization in 3T3 cells. Our results indicate that activators of kinase C stimulate Na+/H+ exchange in normal human fibroblasts (BoGi), but not in mouse embryo (3T3) cells. Addition of serum to BoGi cells, on top of saturating doses of phorbol 12-myristate 13-acetate (PMA), results in a further cytoplasmic alkalinization. Furthermore, sphingosine prevents the PMA-induced increase in pHin in BoGi cells, and phosphorylation of an 80 kDa protein in 3T3 cells, but not the serum-induced alkalinization in either BoGi or 3T3 cells. These data indicate that activation of kinase C does not participate in the physiological activation of Na+/H+ exchange in human fibroblasts or mouse embryo cells by serum.

Dimethylsulfoxide modifies the sensitivity of BALB/c-3T3 cells to the activation of Na+/H+ exchange by phorbol esters.

In a companion paper (Gillies et al.: J. Cell. Physiol. 139:124-129, 1989) we show that phorbol esters (PEs) are unable to stimulate Na+/H+ exchange in BALB/c-3T3 cells under a wide variety of conditions. The Na+/H+ exchangers of a number other cell types are also not responsive to PEs yet have been rendered responsive by treatment with agents such as dimethylsulfoxide (DMSO). We undertook the present study to evaluate whether or not the treatment of BALB/c-3T3 cells with DMSO will induce modifications in the sensitivity of these cells to activation by PEs. The present study indicates that a 3-5 day exposure of BALB/c-3T3 cells to 1.25% DMSO leads to changes in the sensitivity of these cells to the activation of Na+/H+ exchanger by PEs. These changes in sensitivity were apparent at day 3 and maximal at day 5. Non-tumor-promoting analogues of PEs do not activate Na+/H+ exchange, suggesting that the effect is mediated through kinase C. Sphingosine prevents PE-, but not serum-induced alkalinization. However, the half-time of the intracellular pH (pHin) response to serum was increased by sphingosine, suggesting that kinase C participates in, but is not required for, the serum induced activation. Since DMSO does not induce any apparent morphological change, the change in sensitivity of Na+/H+ exchange to PEs is not likely to be related to differentiation, but may be associated with structural changes in the Na+/H+ exchanger and/or changes in isoforms of kinase C which recognize the exchanger as a substrate.

Mirror feedback and judgments of body size.

Body size perception was measured in 44 obese and normal weight subjects using a video TV methodology. Subjects performed a task whereby they manipulated the size of their body image on a TV screen. Subjects viewed their images in the presence and absence of a full length mirror. Subjects were more accurate judging body size with the mirror present. Subjects also responded more slowly and made more adjustments of their body image when the mirror was present. No differences between obese and normal weight subjects were found. Ramifications of the findings for individuals who diet are discussed.
