[Breathing sleep disturbances and occupational medicine: study of 20 clinical cases].

During 2004, in the Center for Sleep Disorders, a questionnaire including Epworth sleepiness scale (ES) was administered to 120 subjects; 20 male subjects of this group with elevated score (ES >14) were selected and submitted to polysomnography. Subjects, all in working age, were represented by 3 (15%) shift-workers, 9 (45%) drivers, 17 (85%) industrial workers (among those 5 building workers) and 3 (15%) employers. By polysomnography, moderatelsevere OSAHS was diagnosed in all subjects (40% moderate, 60% severe). CPAP (Continuous Positive Airway Pressure) therapy led to an improvement of clinical symptoms since the first month. Counselling of Occupational Medicine Physician with the Center for Sleep Disorders, was useful to direct the action of Competent Doctor, especially for jobs requiring high vigilance (drivers or shift-worker). The pass certificate for jobs with an high risk (alone, in high places, heavy means drivers) cannot avoid to evaluate this pathology, that is often associated to other related risk factors (obesity, hypertension, diabetes), because it compromises both the specific suitability and the protection of common health and safety.

Isolation of the human glyceraldehyde-3-phosphate dehydrogenase gene by hybridization with synthetic oligonucleotides.

By screening a human genomic library from human lymphocyte DNA cloned in EMBL 3 vector with synthetic oligonucleotides homologous to the human GAPD cDNA 3'-non-coding region as probes, a unique lambda recombinant clone (EMBL-G5) was isolated at the Tm value. Preliminary restriction analysis and sequence data proved that this recombinant clone is the human structural gene for the glyceraldehyde- 3-phosphate dehydrogenase.

[Blood gas analysis and evaluation of pulmonary functional damage in bronchiolitis].

212 Sucklings affected with bronchiolitis were evaluated monitoring values of haematic gases and acid-base balance. The patients were differentiated in three groups: Hypoxemia--Hypercapnia (3.7%). Hypoxemia--Normocapnia (38.2%). Normoxemia--Hypocapnia (58.1%). Metabolic acidosis was detected in 55.1% of cases. The different value of haematic gases was correlated with entity of the pulmonary functional damage.

[Biochemical monitoring of the insufficient systemic perfusion in bronchiolitis].

Biochemical monitoring was performed on 113 sucklings affected with bronchiolitis in order to evaluate early onset of insufficient systemic perfusion. Values of haematic lactate, GPT and CPK levels in the serum, partial tension of haematic gases were evaluated on first day of hospitalization. Haemogasanalytic monitoring made it possible to differentiate two groups of infants: 21 patients with hypoxemia and 92 patients with normoxemia. Monitoring of lactatemia, CPK and GPT activity evidenced following values: hyperlactemia (66.6%), increased CPK levels (47.6%), increased GPT levels (14.2%) in the group of infants with hypoxemia. Hyperlactemia (55.4%), increased CPK levels (62%), increased GPT levels (19.5%) in the group of patients with normoxemia. These findings show no significant difference between the two groups and frequent occurrence of tissular hypoxia attributable to insufficient systemic perfusion. Moreover the results of such biochemical monitoring permit precise indication about appropriate treatment.

[Cardiac involvement in systemic lupus erythematosus: anatomo-pathological study].

To study the cardiac involvement in systemic lupus erythematosus, the clinical and necroscopy records of 29 patients were analyzed. Of these, 28 were female and 1 was male with a mean age of 28.6 +/- 10.4 years. The mean duration of the disease was 23.8 +/- 20.1 months. Twenty patients had the diagnosis of renal failure, 11 of arterial hypertension and 8 of congestive heart failure. No valvular or coronary arterial disease was diagnosed in any patient. The most frequent cause of death was sepsis, followed by renal failure. Only one patient died of causes directly related to heart disease. At necroscopy, 12 patients had pericarditis and 10 had cardiomegaly. Nonspecific valvular abnormalities were observed in 2 patients, Libman-Sacks' endocarditis in 1 and bacterial endocarditis in 2. Cardiovascular abnormalities were absent in only 8 (7.5%) patients. It was concluded that although frequent and contributing to mortality in some patients, cardiovascular involvement in systemic lupus erythematosus is multifactorial and, usually, nonspecific.
