[Human anisakiasis: 5 cases in northern France].

We report 5 cases of human anisakiasis revealed respectively by an acute intestinal obstruction due to multiple small bowel stenosis, an acute inflammatory ileitis simulating appendicitis, ulcer type epigastralgia, purulent peritonitis and a latent small bowel tumor. Diagnosis was established in 4 cases on microscopic analysis of bowel specimens (eosinophilic granulomas and/or parasitic fragments) and in 1 case at gastroscopy. Serodiagnosis was positive in 2 of 4 cases and the consumption undercooked fishes was found in 4. In small samples we estimated the infestation (1 to 50 parasites) by anisakis larvae of 3 fish species: 80 p. 100 for herrings, 63 p. 100 for mackerels and 100 p. 100 for whitings. This work emphasizes the role of anisakiasis as a source of digestive symptoms and intestinal eosinophilic granuloma.

Diffuse follicular lymphoid hyperplasia of the small intestine without primary immunoglobulin deficiency.

Three cases of follicular lymphoid hyperplasia extending to the whole length of small intestine are reported in three young adult patients of low economic status. The disease was revealed by chronic diarrhea with malabsorption and/or protein-losing enteropathy. In one patient, all transitional patterns were found between the hyperplastic follicles and a small intestinal multicentric centrocytic-centroblastic lymphoma. No abnormalities in humoral or cellular immunity were found in the three patients. In particular, serum immunoglobulins (except IgG in one case) and plasma cell populations of small intestinal lamina propria were normal. Diffuse follicular lymphoid hyperplasia of the small intestine in the absence of primary immunoglobulin deficiency appears to be a rare condition associated with (or leading to) intestinal malignant lymphoma in most cases. Patients usually belong to the same populations as those suffering from alpha-chain disease.

A cooperative study on the detection of colorectal cancer and polyps in France.

Systematic investigations testing for the presence of polyps and cancers of the large bowel were conducted in a population of 1,369 inpatients and outpatients, aged 45-70 years, in eight university departments of gastroenterology or abdominal surgery (Toulouse, Dijon, Paris, Marseille, and Strasbourg). Double-contrast barium enema and proctosigmoidoscopy examinations were carried out in all cases, whereas total coloscopy was performed only in the case of detected tumors. A questionnaire including 233 parameters (age, sex, family, and personal history and symptoms) was completed for each patient. A total of 414 lesions were detected in 252 patients, including 245 adenomatous or villous polyps, 8 transformed polyps, and 30 carcinomas; 3 of 4 lesions were located in the rectum or sigmoid. Cancer or adenomatous or villous polyps were found in 13% of the patients. The prevalence of these lesions in the population studied was increased in patients with rectorrhagia (19%) or with a personal history of surgery for colorectal cancer or polyp (23%). In the patients without rectorrhagia or a history of intestinal tumor, the incidence was 9.7%. It was significantly increased in males and patients more than 50 year old. The efficacy of proctosigmoidoscopy and double-contrast barium enema was compared in 909 patients. Sensitivity and specificity were, respectively, 35% and 99% for endoscopy, 96% and 94% for radiology.

Extramedullary plasmacytoma with gastric and lymph node involvement: an immunological study.

A case of gastric and lymph node extramedullary plasmacytoma is reported in a young Algerian female. Treatment with cyclophosphamide was ineffective, but rubidazone, followed by a maintenance therapy with prednisone, procarbazine, and vinblastine induced a complete remission. Immunofluorescence studies and biosynthesis experiments in vitro showed that the tumor cells produced and secreted IgA-kappa molecules, whereas no monoclonal immunoglobulin could be found in the serum and gastric juice.

Villous tumors of the colon and rectum: double-contrast study of 47 cases.

The radiological features of 47 villous tumors of the large bowel found in 39 patients by the double-contrast barium enema were studied. An attempt was made to determine the radiological indicators of malignancy and two features proved to be suspicious: large tumor size and indentation of tumoral base. This retrospective study emphasizes the advantages of the double-contrast technique in the detection of colonic villous tumors.

[Hepatitis due to ajmaline. Report of cases and review of the literature].

The authors report the cases of 4 patients with jaundice following the administration of ajmaline. The disease had a pseudo- angiocholitic onset with fever, chills and pruritus in the 4 patients and abdominal pains in 2 patients. Serum transaminase activity and serum alkaline phosphatase activity were increased in the 4 patients. Blood eosinophilia was found in 3 patients. Liver lesions included predominantly centrilobular cholestasis, mild hepatocytic lesions, and portal inflammation. After the interruption of the drug administration, recovery occurred in the 4 patients. Two patients resumed the intake of ajmaline; transient hepatitis recurred in these 2 patients.

Fibrosing alveolitis and hepatitis B surface antigen-associated chronic active hepatitis in a patient with immunoglobulin A deficiency.

Fibrosing alveolitis is described in a 22 year old woman with immunoglobulin A (IgA) deficiency and hepatitis B surface antigen (HBsAg)-associated chronic active hepatitis. At lung biopsy HBsAg was detected by indirect immunofluorescence in the alveolar space but not in the septal fibrosis. We discuss the possible relationships between IgA deficiency on the one hand, and HBsAg-associated lung and liver diseases on the other hand.

Villous adenoma of the duodenum. Endoscopic diagnosis and resection.

The authors report a new case of villous adenoma of the duodenum diagnosed by X-ray examination and duodenoscopy. Endoscopic removal allows an accurate histologic diagnosis. This endoscopic resection relieves symptoms and spares the patient a laparotomy. It seems to be an acceptable treatment of benign duodenal tumors.
