The 'dry' non-eczematous skin associated with atopic eczema.

The non-eczematous but 'dry' skin of twelve patients with eczema has been studied and contrasted with skin from similar sites in control subjects of similar age and sex and with patients with autosomal dominant ichthyosis. The areas investigated were examined histologically and by techniques for assessment of the stratum corneum structure and function. The 'dry' skin sites demonstrated increased intracorneal cohesion, increased total epidermal thickness, patchy parakeratosis and in places slight hypergranulosis. It is concluded that the dry skin condition of patients with atopic eczema is not autosomal dominant ichthyosis and is probably eczematous in nature.

HLA antigens in geographic tongue.

HLA-A,B,C phenotyping was performed on 95 patients with geographic tongue to determine whether there is an increased frequency of any particular allele in this condition. An increased frequency of B15 was found in the patients when compared to normal controls. When the patients were divided into atopic and non-atopic groups there was an increased frequency of B15 and a decreased frequency of B40 in the atopic group compared to the controls. B40 was as decreased in the atopic group when compared to the non-atopic group. When the type I correction factor was applied to the probability values the differences in antigen frequencies in all cases became insignificant.

Transepidermal elimination: studies with an animal model.

Experiments designed to identify the stimulus to transepidermal elimination (TE) are reported. An animal model has been used in which a charcoal suspension is injected intracutaneously and which mimics the process seen in human skin disease. Studied with antihistamines and indomethacin indicate that neither histamine nor prostaglandins are the initiating stimulus, although the latter substances may be responsible for the associated hyperplasia. Experiments using explants of previously injected skin suggest that physical disturbance of the skin may be implicated in the development of the TE response.

The efficacy of histamine antagonists as antipruritics in experimentally induced pruritus.

1. The antipruritic ability of histamine (H1 and H2) antagonists alone and in combination in experimentally induced pruritus has been investigated in 12 normal human volunteers. 2. A combination of cimetidine (H2 anatagonist) and chlorpheniramine (H1 antagonist) proved effective in suppressing itch that was artificially induced by the application of papain and histamine. 3. The combination was more effective than chlorpheniramine or cimetidine alone or placebo. 4. The results suggest that both H1 and H2 receptors are involved in the mediation of pruritus.

Atopic disease and dermatitis herpetiformis.

Patients with dermatitis herpetiformis were found to have an increased incidence of atopic disorders compared to an age and sex matched control group. The increase in incidence was statistically significant for atopic eczema and any atopic disorder. Serum immunoglobulins (including IgE) were estimated in forty-five dermatitis herpetiformis patients and no significant abnormalities were found.

The change in properties of the stratum corneum as a function of depth.

The constancy of stratum corneum thickness suggests an ordered decrease in intracorneal cohesion binding forces. In this study, intracorneal cohesion was measured in the vertical dimension by cohesography and the number of cells released after applying a standardized stimulus was determined before and after repeated stripping of the same sites with adhesive tape. In addition, surface replicas and the corneocytes from different levels were examined by scanning and transmission electron microscopy and differential phase interference microscopy. The results indicate a gradual loss of cohesion within the stratum corneum towards the surface and that anatomical changes take place in the corneocytes as they progress towards the surface.

A syndrome of ichthyosis, hepatosplenomegaly and cerebellar degeneration.

Two brothers are described with ichthyosis, hepatosplenomegaly and signs of cerebellar degeneration. The ichthyosis was similar to autosomal dominant ichthyosis both clinically and biochemically. No metabolic abnormality has yet been identified to account for this previously undescribed triad of abnormalities.

Geographic tongue--a manifestation of atopy.

One hundred patients with geographic tongue have been studied for evidence of an association with atopy. There was a significantly high frequency of personal and/or family history of asthma, eczema and hay fever amongst the patients when compared to a control population. A raised total serum IgE( greater than or equal to 200 u/ml) was significantly more frequent in patients compared to a control population. These findings confirm the impression that in some patients geographic tongue is a sign of atopy.

A history questionnaire for dermatology out-patients.

A questionnaire has been devised to be completed by new patients at home before attending the dermatology clinic. We report our experience with this questionnaire and discuss the potential advantages of this system; these include increased detail and accuracy of history, standardization of information and easier availability of information at patients' follow-up. Previously used questionnaires and other similar methods of obtaining a self administered history are discussed.

Familial Kaposi's sarcoma.

Kaposi's sarcoma occuring in a mother and son is described, and previous reports of familial occurrence of this disease are reviewed.

Is the stratum corneum of uninvolved psoriatic skin abnormal?

A variety of abnormalities of the uninvolved skin have been reported in psoriasis, but there are few studies in which abnormalities of the stratum corneum (SC) have been investigated. In this study we have examined the intracorneal cohesion and structural detail of corneocytes of the SC from involved and uninvolved sites in 24 patients with psoriasis and 10 controls. We have found that intracorneal cohesion is increased in the involved and uninvolved skin of psoriatic patients compared to controls and that there are abnormalities of stratum corneum and corneocyte structure as determined by scanning electron microscopy. The changes in the uninvolved sites may well be due to the increased rate of epidermal cell production in these areas.

Procollagen synthesized by newborn rat skin in culture.

Newborn rat skin explants in culture have been found to synthesize and secrete into the medium a considerable amount of collagen precursors (procollagens). Gel-electrophoresis analysis of the material extracted from the medium indicates that it is constituted mainly by procollagen Type I with a small percentage of procollagen Type III. Antibodies have been raised to the extracted procollagen and, although no attempt has been made to render these antibodies specific to one procollagen type by appropriate immunoadsorption, the immunofluorescence patterns that have been obtained are in good agreement with those reported on procollagen Type I by other groups. When unfixed cultured fibroblasts are examined by indirect immunofluorescence in the presence of the antiprocollagen antibody, a fluorescent network of interwoven fibres on the cell surface is observed which is similar to the network formed by fibronectin (LETS) on the fibroblasts' cell membranes.
