[Osteocalcin or bone GLA-protein].

Osteocalcin or GLA bone protein, the most abundant of non-collagen proteins in bone, is produced only by osteoblasts. Synthesis is enhanced by the active form of vitamin D. Most of the osteocalcin produced combines with hydroxyapatite crystals, but a small proportion is circulated in the bloodstream and eliminated by the kidneys. Because bone resorption and bone formation are closely linked, serum osteocalcin is a specific and reliable marker of bone tissue activity.

[Specificity and indications of bone scan in non-neoplastic bone and joint disease in children. A study of seventy cases].

We studied 70 bone scans in pediatric patients to define the indications of this procedure in non-neoplastic disease. Sensitivity of bone scans in infections proved outstanding except in children under one year of age. Eighty-nine per cent of children with pain and fever and a positive bone scan had either an infection or osteochondritis. The same symptoms with a negative bone scan indicated either transient synovitis of the hip or the absence of bone lesions in 83% of cases. Children with pain as the only symptoms and a negative bone scan consistently had either transient synovitis of the hip, or normal bones and joints (100% of cases). Eighty-two per cent of children with pain and a positive bone scan had an infection or osteochondritis. Indications of bone scanning vary according to clinical features and include all children with functional impairment, pain and fever, and isolated limps with no obvious cause.

[Physiologic variations in plasma osteocalcin in the child and the adolescent].

Osteocalcin, the most abundant of non-collagen proteins in bone, is produced by osteoblasts and incorporated into the bone matrix. A small amount is circulated in the bloodstream where it can be detected by radioimmunologic assay. Serum assays in 128 children and adolescents showed that serum osteocalcin levels accurately reflect the various stages of statural growth: peak levels were found at three different ages, i.e. infants (21.8 +/- 4.9 ng/ml), children over seven (19.4 +/- 5.2 ng/ml), and adolescents undergoing the early stages of puberty (22.5 +/- 4.2 ng/ml). Although significant differences were found between each age group and the preceding or following group, these results can be applied to individual patients only for longitudinal studies.

[Blood pressure in 6- to 45-month-old children. Apropos of a study of 264 children from nurseries].

The systolic, diastolic and mean blood pressure and pulse rate were measured among 264 children aged 6 to 45 months, during routine check-up in 4 day-nurseries. We took the mean value of 3 successive measurements taken at one minute intervals on the upper arm of calm, asymptomatic children, with an automatic oscillometric monitor (Dinamap). When the mean systolic blood pressure was greater than 110 mmHg, the measurement was repeated. The results were related to weight, height and age and to personal and/or family history. The systolic pressure was steady with a 97th percentile at 110 mmHg. The diastolic pressure presented two levels: 97th percentile at 81 mmHg before 24 months and 73 mmHg afterwards. Nine children born to toxemic mothers, and 14 having required neonatal hospital care, had lower diastolic pressure. The other family or personal data were not linked to particular pressure groups.

[Atypical mycobacterial pulmonary infections. Apropos of a case in a newborn infant].

The authors report the case of an 8 month-old infant presenting with mycobacterium avium-intracellulare pulmonary infection. The etiological diagnosis relies upon isolation of the organism. The cutaneous reactions to avian tuberculin are only a guiding factor. Evolution was favorable in spite of the resistance of the bacteria to antituberculous chemotherapy. The epidemiological research carried out in a neighbouring chicken-run was negative.

[Alloimmune neonatal neutropenia in homozygotic female twins].

Female homozygous twins presented with alloimmune neonatal neutropenia due to a maternal neutrophil-specific anti-NA2 antibody. The disease is usually benign, but when accompanied by severe infection it may require exchange transfusion and transfusion of granulocytes free of the antigen involved in the immunological conflict. Owing to the risk of relapse in subsequent pregnancies, the haemogram of the following infants must be supervised.

[Spondylodiscitis in children following lumbar puncture. Apropos of a case].

We described a case of spondylodiscitis L3 L4, in a 3 year old child that appeared 3 weeks after a lumbar puncture. The infectious agent responsible wasn't found. The child recovered without sequelae with an antistaphylococcis treatment. The mecanism by inoculation has been described after surgery of the inter-vertebral disc but also after spinal anesthesis. The bone complications of the lumbar puncture are exceptionally rare but elementary aseptic precautions must not be overlooked in order minimise the infection.

[Registry of cancers in children in the regions of Provence, the Alps, Côte d'Azur and Corsica. Setting up and evaluation of the year 1984].

Epidemiology is a promising clinical way for research on cancer. A population based pediatric cancer registry was opened since January 1984 in 2 regions of south-eastern France. Its purpose is to analyse childhood cancer incidence and possible variations. A case-control study on risk factors was set up at the same time. Registration procedures and 1984 data are reported. The observed incidence is 11.86 new cancer cases/10(5) children/year.

[Congenital tracheal stenosis associated with aberrant left pulmonary artery. Apropos of a severe form with neonatal disclosure].

The case report is presented of a newborn infant afflicted with extensive congenital tracheal stenosis associated with anomalous left pulmonary artery, proving rapidly fatal. The possibility of tracheal anomaly must be envisaged in the face of any neonatal respiratory distress unexplained by pulmonary parenchymatous pathology. Diagnosis of this complex malformation relies on endoscopy and oesophagogram and tracheo-bronchogram contrasts.

[Supraventricular tachycardia and fetal edema. Apropos of 2 cases].

Two cases of supraventricular tachycardia responsible for hydrops fetalis emphasize the interest of an antenatal echocardiography. The systematic use of this technique will increase the frequency of diagnosis of this etiology in other cases where the prognosis is less favorable. The in utero treatment of this rhythmic disorder consists in administering Digoxin to the mother and the newborn child must receive excellent care from the moment of the birth.

[Longterm pH measurement in the diagnosis of gastroesophageal reflux in children. Apropos of 20 cases].

Long duration continuous recording of esophageal pH was performed in 20 children suspected of having gastroesophageal reflux. Six pH monitoring parameters were analysed and their values compared with normal standards found in the literature. The results of pH monitoring were compared with those of upper gastrointestinal series and with oesophagoscopy, performed in all patients but one. The diagnostic utility of long duration pH monitoring is discussed, according to data in the literature.
