ArrayExpress--a public database of microarray experiments and gene expression profiles.

UNLABELLED: ArrayExpress is a public database for high throughput functional genomics data. ArrayExpress consists of two parts--the ArrayExpress Repository, which is a MIAME supportive public archive of microarray data, and the ArrayExpress Data Warehouse, which is a database of gene expression profiles selected from the repository and consistently re-annotated. Archived experiments can be queried by experiment attributes, such as keywords, species, array platform, authors, journals or accession numbers. Gene expression profiles can be queried by gene names and properties, such as Gene Ontology terms and gene expression profiles can be visualized. ArrayExpress is a rapidly growing database, currently it contains data from >50,000 hybridizations and >1,500,000 individual expression profiles. ArrayExpress supports community standards, including MIAME, MAGE-ML and more recently the proposal for a spreadsheet based data exchange format: MAGE-TAB. AVAILABILITY: www.ebi.ac.uk/arrayexpress.

Membrane-bound dimer structure of a beta-hairpin antimicrobial peptide from rotational-echo double-resonance solid-state NMR.

The intermolecular packing of a beta-hairpin antimicrobial peptide, PG-1, in lipid bilayers is determined using solid-state NMR distance measurements. Previous spin counting experiments showed that PG-1 associates as dimers in POPC bilayers; however, the detailed dimer structure was unknown. We have now measured several intermolecular 13C-19F, 1H-13C, and 15N-13C distances in site-specifically labeled PG-1 to constrain the structure of the intermolecular interface. The distances are measured using the rotational-echo double-resonance (REDOR) technique under magic-angle spinning. The results indicate that two PG-1 molecules align in a parallel fashion with the C-terminal strand of the hairpin forming the dimer interface. Six hydrogen bonds stabilize this interface, and the Phe12 side chain adopts the g- conformation in the membrane as in solution. The parallel packing of the peptide in the lipid bilayer differs from the antiparallel dimer found in DPC micelles and may be stabilized by its strong amphipathic character, which should facilitate its insertion into the amphipathic lipid bilayer. This study demonstrates the utility of the REDOR NMR technique for the elucidation of the oligomeric structure of membrane proteins.

[NK/T-Cell lymphoma of nasopharynx].

OBJECTIVE: Discussion of the clinical and paraclinical features of a rare tumor in the nasopharynx, the NK/T-cell lymphoma, with an emphasis on the implications of Epstein Barr Virus (EBV) in its pathogenesis and prognosis. CASES REPORT: We report the cases of a woman and a man aged 34 and 48, respectively. Clinical presentation was non-specific and diagnosis was established with deep biopsies under general anesthesia. Immunochemistry and in situ hybridization were positive for Epstein Barr virus in tumor cells in both cases. The patients were first treated with chemotherapy (CHOP regimen), then underwent radiotherapy. The first patient achieved complete response at twenty months follow-up. The second patient presented two recurrences which were treated by chemotherapy. He died from acute renal failure during treatment of the second recurrence. CONCLUSION: Diagnosis of NK/T-cell lymphoma may be delayed in the event of non-specific symptoms. Deep biopsies outside necrotic tissue must be obtained. Search for EBV is important as its presence is a factor of poor prognosis.

[Mycobacterial cervical lymphadenitis: role of surgery].

OBJECTIVE: To discuss the place of surgery in the management of mycobacterial cervical lymphadenitis. PATIENTS AND METHODS: It's a retrospective study (1982-2002) about 246 patients treated in the ENT department of Farhat Hached Sousse for cervical lymph node tuberculosis. Resolution was considered when neither symptoms nor lymphadenopathy in examination were noted. RESULTS: The mean age of our patients was 28 years. A light female prevalence was noted. In 5 patients diagnosis was established by lymph node punction. A medical treatment of first intention was then managed with a failure in 3 cases, after deadlines from 3 to 4 months. All other patients were operated: cellulolymphadenectomy (47%), adenectomy (47%) or drainage of a cervical abscess (4%). Recurrence and antituberculosis treatment resistance were noted respectively in 6% and 3% of cases. DISCUSSION: Two questions are still discussed in the management of mycobacterial cervical lymphadenitis: 1- Is surgery necessary for the diagnosis? 2- When is surgery indicated directly? Histological specimen established the diagnosis in all cases and excluded a mestastatic lymph node. In some situations surgery must be indicated at first: cold abscess, lymph node fistulation. CONCLUSION: Surgery still has an important place in the treatment of tuberculosis lymphadenopathy.

Which treatment for genital tract Chlamydia trachomatis infection?

A national opportunistic chlamydia screening programme, mainly targeting young sexually active women, is gradually being introduced across the UK and in future will predominantly occur in primary care sites. The relative efficacy of recommended antibiotic treatments for chlamydia has been poorly studied and especially that of single dose azithromycin. In Portsmouth, 1536 patients treated for chlamydia, with four different antibiotic regimens, during the Department of Health pilot study, were asked to return for test of cure. No difference in treatment outcome was found using doxycycline, oxytetracycline, erythromycin or azithromycin. Directly observed therapy with azithromycin may be especially helpful in treating young chlamydia-positive patients.

Intermittent pneumatic compression for treating venous leg ulcers.

BACKGROUND: Intermittent pneumatic compression (IPC) is a mechanical method of delivering compression to swollen limbs. This technique has been used to treat venous leg ulcers and limb swelling due to lymphoedema. The effectiveness of IPC, and the appropriate duration and frequency of IPC therapy are unknown as are the differences between various types of IPC. This review analyses the evidence for the effectiveness of IPC as a treatment for venous leg ulcers. OBJECTIVES: To determine whether IPC increases the healing of venous leg ulcers. To determine the effects of IPC on health related quality of life of venous leg ulcer patients. SEARCH STRATEGY: The Cochrane Wound Group Trials Register was searched for RCTs of intermittent pneumatic compression in February 2001. Journals and relevant conference proceedings were searched by hand. Companies were also contacted for relevant unpublished data or ongoing studies. SELECTION CRITERIA: Randomised controlled studies either comparing IPC with control (sham IPC or no IPC) or comparisons between IPC treatment regimens, in venous ulcer management were included. DATA COLLECTION AND ANALYSIS: Data extraction and assessment of study quality were undertaken by two reviewers independently. MAIN RESULTS: Four randomised controlled trials were identified. One small trial (45 people) found increased ulcer healing with IPC plus compression than with compression alone (relative risk for healing 11.4, 95% Confidence Interval 1.6 to 82). Two small trials with a total of 75 people found no evidence of a benefit for IPC plus compression compared with compression alone. One small trial (16 people) found no difference between IPC (without additional compression) and compression bandages alone. REVIEWER'S CONCLUSIONS: Further trials are required to determine whether IPC increases the healing of venous leg ulcers.

Randomized controlled study of ultrasound therapy in the management of acute lateral ligament sprains of the ankle joint.

OBJECTIVE: To determine the efficacy of a fixed dose of ultrasound energy to treat acute lateral ligament sprains of the ankle joint. STUDY DESIGN: Double-blind randomised controlled trial. SETTING: Accident and Emergency department of University Teaching Hospital. SUBJECTS: Patients presenting at Accident and Emergency with ankle injuries. INTERVENTION: Ultrasound or placebo, and Tubigrip. OUTCOME MEASURES: Pain measured with visual analogue scales, swelling using a tape measure, range of movement using a fluid-filled goniometer, and weight bearing using two scales simultaneously. RESULTS: Patients in both groups improved symptomatically. There were no statistically significant differences between groups in any outcome measure. Within groups, statistically significant differences were detected in pain perceived, and range of movement (dorsiflexion). CONCLUSION: At the dose and duration used, ultrasound therapy is no better than placebo in the management of lateral ligament injuries.

A study of low clinic attendance of leprosy patients: reasons and solutions.

In this study, the effect of health education on a sample of 325 absentee leprosy patients was assessed in a leprosy endemic area. About 46% of the absentees from the study group returned to the clinic following health education (HE). HE was more effective amongst those who defaulted in the later part of their treatment sessions. Monthly attendance rate increased from 70%-74% to 72%-91% following HE. Amongst the absentees, 58% were absent due to personal reasons and 8% due to health service related reasons. Personal reasons was the commonest cause for absenting at second pulse. At third pulse it was due to socio-economic reasons. At fourth pulse it was due to service-related reasons. At fifth pulse the commonest reason was disease-related. Health education had proved to be a definite solution to solve the absentee problem.

Duplex ultrasound scanning for diagnosing lower limb deep vein thrombosis.

BACKGROUND: Traditionally, venography is used to investigate deep vein thrombosis (DVT), though duplex ultrasound offers a challenging, noninvasive alternative method and previous reports have described the use of duplex ultrasound for detecting proximal lesions. OBJECTIVE: To compare duplex ultrasound imaging with venography for both proximal and distal lesions using the probe compression technique and a different protocol. METHODS: Duplex scans were done by the same operator prior and blind to venography, which was performed using a standard protocol with the films being reviewed blindly by a second radiologist at a later date. RESULTS: In 74 patients, 21 of 22 above-knee lesions present on venography were detected by duplex scanning and there was complete agreement in the 52 of 52 venogram negatives. For lesions below the knee, duplex detected 26 of 27 lesions also present on venography and agreed on 37 of 43 venogram negatives. These figures give duplex ultrasound overall accuracies of 99% and 90% (Kappa 98% and 89%) for above- and below-knee lesions, respectively. CONCLUSION: These figures suggest duplex ultrasound technique compares favorably with venography for diagnosing both proximal and distal lesions.

Abnormal microvascular responses in a lateral epicondylitis.

The laser Doppler flowmeter, an instrument highly sensitive to changes in blood flow velocity in the dermal microvascular bed, was used to examine a sympathetic vasomotor response (SVR) in 40 patients with lateral epicondylitis. There was a significant association between the absence of a normal SVR in the skin overlying the affected enthesis compared with the unaffected contralateral epicondyle (P < 0.01). These results suggest that local dysfunction of the sympathetic nervous system may be associated with the pathogenesis of anatomically discrete pain in the enthesopathy of epicondylitis.

Postabortal sepsis.

A review of postabortal sepsis following a 2-year study undertaken at the Department of Obstetrics and Gynaecology, SSG Hospital, Baroda is presented here with a view to know the incidence of postabortal sepsis and septic induced abortions and to re-evaluate the effectiveness of the MTP services in reducing its impact on maternal morbidity and mortality, since the implementation of the MTP Act in 1972. It has been observed that postabortal sepsis is a major cause of maternal mortality even now and MCH service is to be improved to reduce the same.

Assessment of tennis elbow using the Marcy Wedge-Pro.

The Marcy Wedge-Pro (MWP), a device used in training by tennis players, was employed in the assessment of tennis elbow. The MWP was used to measure the ability of patients to perform wrist extension exercises, since pain resulting from this specific activity is a prominent symptom of the condition. The MWP results were compared with clinical measures and found to identify accurately patients who responded to treatment (P < 0.05). This study illustrates the potential of the MWP to assess tennis elbow quantitatively.

Multiple sclerosis as a cause of atrial fibrillation and electrocardiographic changes.

We present the case of a young woman with an acute exacerbation of multiple sclerosis associated with paroxysmal atrial fibrillation and electrocardiographic changes characteristic of central nervous system effects on cardiac conduction. The patient presented with ataxia, vertigo, and left facial paresthesia, and was found to have a large demyelinating lesion involving the left middle and inferior cerebellar peduncles with extension rostrally into the brain stem. Profound pulmonary edema, out of proportion with the cardiac abnormalities, also developed. This confirms that demyelinating disease, like other central nervous system abnormalities, can lead to alterations in cardiac conduction, and may induce neurogenic pulmonary edema.

Venous haemodynamics--a consideration of macro- and microvascular effects.

In health, valves in the deep veins function unidirectionally permitting blood flow towards the heart. This function is often degraded following deep vein thrombosis leading to venous hypertension and disturbed haemodynamics in the legs of the patient. Hypertension, in turn, promotes venous ulceration through a series of degenerative pathological steps. Traditional methods of treatment include leg elevation and compression using elasticated garments or bandaging. These techniques manage the problem with varying success but do not cure it. Recent advances in surgical research suggest that novel techniques are being developed for treating valve incompetence. In these circumstances, vascular scientists should be examining methods of addressing both the macro- and the microcirculation so that changes in these consequent to venous hypertension may be measured. This review examines existing methods of measuring the macrocirculation as well as the potential techniques to study effects of venous haemodynamics in the microcirculation.
