[Parathyroid hormone: perinatal aspects].

Authors have measured parathormone levels in 28 mothers at delivery and their respective newborns at 40-50 hours after birth. They also quantified these levels in umbilical vessels. Control groups was formed by 48 healthy non pregnant women. PTH levels in pregnant women were similar to controls. In umbilical artery these values were higher than in umbilical vein and in both, lower than other groups. Newborns at 40-50 hours after birth showed higher PTH levels than controls and pregnant women at delivery. A positive correlation between the PTH levels of the mother and those of the umbilical vessels is found. PTH levels of the neonate at 40-50 hours after birth correlated with levels of umbilical artery and umbilical vein. They do not observe a hyperparathyroidism at the delivery, although there is a slight decrease in Ca levels. Newborns at 40-50 hours after birth did show an increase in their PTH levels in parallel with hypocalcemia correlation found between PTH levels of the mother and those of the umbilical vessels could suggest a placental transfer for this hormone.

[Tobacco and pregnancy. Weight of the newborn infant and weight gain of the mother].

865 patients delivered at women's clinic "La Paz" have been examined, to prove the hypothesis, that the weight of the newborn may be influenced by smoking indirectly via nutrition. 276 of the 865 women did smoke. In this group the weight of the newborns was lower significantly. If we select both groups of smoking and nonsmoking patients appropriate to the weight increase during pregnancy above or below the average value, we can state that those fetuses had a intrauterine growth retardation whose mothers did smoke and had a lower weight increase than the average value of all pregnant women.

Complications of the Simcoe flexible loop phacoprosthesis in the anterior chamber.

A preliminary study of the use of a Simcoe 101-J implant in the anterior chamber after pars plana lensectomy by ultrasonic fragmentation showed initially good results. Further observations of these and additional eyes showed 20/20-20/40 vision in 86.9%, retinal detachment in 3.5%, retinal hole in 1.5%, clinical cystoid maculopathy in 13%, and so-called UGH syndrome in 5%. Pars plana lensectomy alone in the last 136 eyes resulted in vision of 20/20-20/40 in 98.5%, a retinal detachment rate of 0.73%, retinal hole rate of 0.73%, no UGH syndrome, and no clinical cystoid maculopathy. Pars plana lensectomy alone appears to be a far safer procedure than pars plana lensectomy plus a Simcoe implant in the anterior chamber. Implantation of the Simcoe 101-J manufactured by IOLAB Corp. in the anterior chamber is not recommended.

Short-term follow-up of the nutritional status of children undergoing autologous peripheral blood stem cell transplantation.

A prospective longitudinal study was conducted to analyze the evolution of the nutritional status of 34 children (12 girls and 22 boys), aged 1.5-15.8 years (median age 9.06), undergoing autologous peripheral blood stem cell transplantation (PBSCT). The nutritional status was evaluated at baseline, days +1 and +7, discharge, and day +30 by dietary or parenteral intake, anthropometric and laboratory measurements, and nitrogen balance. At baseline, changes in anthropometric (53%) and biochemical measurements (83%) are frequent but mild. The mean caloric intake was normal. Children with normal values for the anthropometric parameters all had an intake > 80% (p < .01). No correlation was found between the anthropometric and biochemical parameters. During transplantation, significant changes (p < .001) were found for energy intake, albumin, transferrin, and nitrogen balance. Fibronectin, prealbumin, and retinol-binding protein showed only a few changes. All but prealbumin recovered on day +30. No correlation was found between the nutritional status and toxicity or infection in children undergoing autologus PBSCT. The changes in the nutritional status observed at the start of transplantation correlated with the nutrional intake. Anthropometric and biochemical changes are complementary. The results may be ascribable to the fact that the patients in this series had mild malnutrition.

[The Fuenlabrada's study: tobacco as cardiovascular risk factor in children and adolescents].

Smoking habits were studied in 1,274 healthy adolescents of both sexes, aged 10 to 18 years, of Fuenlabrada, Madrid. It was known by personal interview. 30% of the population were smokers. Smoking habits increased with age up to 18 years when 63% of males and 53% of females were smokers. Smoking prevalence was significantly higher among males than in females. Males and females smokers had significantly lower levels of C-HDL and higher levels of C-LDL/C-HDL ratio than non smokers. No differences were observed for anthropometric variables, body mass index, blood pressure, skin-fold thickness, total cholesterol, glucemia and uric acid. The implications of these findings are of concern in light of the apparently protective role of C-HDL in coronary heart disease. These findings provide valuable information to the promotion of antismoking campaigns in adolescent age.

[The Fuenlabrada study: lipids and lipoproteins in children and adolescents].

Lipids and lipoproteins were studied in 2,153 healthy children, of both sexes, aged 0 to 18 years old, of Fuenlabrada, Madrid, population. We determined: Total Cholesterol (TC) and Triglycerides (TG) by automatized enzymatic techniques. C-HDL by precipitation method. C-LDL and C-VLDL were calculated by Friedewald-Fredrickson equation. Our laboratory have previously standardized the techniques of obtention of TC, TG and C-HDL using serums of reference. Mean values were: TC = 166 +/- 36 mg/dl, TG = 63 +/- 39 mg/dl, C-HDL = 53 +/- 13 mg/dl, C-LDL = 102 +/- 34 mg/dl, C-VLDL = 13 +/- 6 mg/dl. In our population 14% had TC greater than or equal to 200 mg/dl, 2.6% TG greater than or equal to 140 mg/dl and 15% C-HDL less than or equal to 40 mg/dl. During adolescence a significant change in the lipid and lipoprotein pattern in both sexes was observed. At the end of the adolescence, males had C-HDL lower and TG, C-LDL and C-VLDL higher than females. Correlations and associations of lipids and lipoproteins were similar in our population than in adult population. The mean finding of this study is the actual high level of TC in our children population.

Neutrophil dynamics in abdominal cavity of peritonitic rats treated with antiseptics.

Peritonitis was induced in Wistar rats by intraperitoneal inoculation of pure Escherichia coli. Rats in which 2 ml of 1% povidone iodine had been injected intraperitoneally 5 min after the bacterial challenge, showed a decrease in the neutrophil percentage of the peritoneal cell population during the first hours of peritonitis compared to the control group. When the same experiment was performed with 2 ml of 0.05% chlorhexidine the percentage of neutrophils was superior to the control group in the first hours after bacterial challenge. These results concur with a previous study in which the deleterious effect of povidone iodine and the beneficial effect of chlorhexidine were demonstrated.
