[Spectral power density and coherence in sleep EEG in patients with acquired immunodeficiency syndrome].

15 male AIDS-patients from 26 to 55 years (mean 41.8 +/- 8.5) with various cerebral manifestations had a whole-night-sleep-EEG registration. As control the recordings of 15 age-matched volunteers (26-55 years, mean 41.8 +/- 9.8) were examined. Spectral characteristics of elementary EEG-epochs of 40 s length were computed, and sleep staging was performed visually for these intervals. The spectral power density of eight EEG-derivations (left and right frontopolar, frontal, central and occipital electrodes, reference montage to the ipsilateral Cb) were measured (sampling rate 64(-1) s, spectral resolution .25 Hz, frequency range from .25 to 24 Hz). Interhemispherical coherences of the frontal and occipital derivation pairs, and intrahemispherical fronto-occipital coherences of the left and right hemisphere, were computed. In the patients the frontal power density of NREM sleep showed lower values in the frequency range of 10 to 14 Hz. In central and in occipital derivations the power density between 12.5 and 15 Hz was lower in the patients, but the difference was less accentuated. The spectral power density of REM sleep showed similar characteristics in both groups. The interhemispherical frontal coherence of the whole frequency range below 13 Hz was markedly lower in the patient group. This was true for the NREM sleep, and, slightly less, for the REM sleep, too. The interoccipital spectral coherence was generally slightly lower in the patient group; the difference was most clearly in the 12.5 to 15 Hz range of NREM sleep.

Purification of cytochrome P-450 from n-hexadecane-grown Acinetobacter calcoaceticus.

A convenient procedure for the purification of cytochrome P-450 from an n-alkane-assimilating strain of the bacterial species Acinetobacter calcoaceticus has been elaborated. The cytochrome P-450 of n-hexadecane-grown cells was found to be distributed in cell-free extracts among particulate and soluble subcellular fractions. For isolation, cytochrome P-450 was extracted from particulate fractions by addition of Triton X-100 to the buffer. Subsequently, purification to an apparently homogeneous state was achieved by chromatography on DEAE-cellulose, Sepharose CL-6B and hydroxylapatite cellulose. A Mr of 52,000, an isoelectric point of 4.7 and the amino acid composition were determined. The Soret bands of absolute absorption spectra showed maxima at 417 nm for the oxidized form, at 408 nm for the reduced form and at 448 nm for the carbon monoxide compound of the reduced cytochrome. Difference spectra with octylamine showed maxima at 432 nm and minima at 410 nm indicating the predominance of the low-spin state. Conversion to the high-spin state could not be obtained. The isolated cytochrome P-450 was stable in the presence of Triton X-100 under neutral pH conditions. Removal of the detergent or change of the pH to values higher than 8.0 or lower than 6.0 resulted in the destruction of the cytochrome P-450.

Transfer of metaclazepam and its metabolites into breast milk.

Ten young women took part in this study a few days after delivery (day 3 and day 5 post partum). Lactation had developed normally but the newborn infants were not breast-fed. The study was intended to investigate whether metaclazepam (Talis), a new 1,4-benzodiazepine, and some of its metabolites were present in breast milk. Levels were measured in the plasma and milk. The levels in the milk showed that metaclazepam, N-desmethylmetaclazepam and two of its metabolites with a lactam structure could be found in small amounts. Differences in metaclazepam and N-desmethylmetaclazepam concentrations in the breast milk on days 3 and 5 post partum are discussed.

[Prophylactic irradiation of blood components with conventional radiotherapy devices].

The irradiation of blood components is supposed to be the safest way of the graft versus host reaction prophylaxis. The radiotherapy facilities and compact irradiators for blood preparations and biological materials are used for prophylactic irradiations, practically. The radiotherapy machines allow a homogeneous dose application, but need a high organizing effort. The use of compact irradiators is less complicated but the large dose differences can be found in the target volume. There is no agreement about the extent of the prophylactic dose. For radiation biology reasons the application of prophylactic doses between 15 and 50 Gy seems to be justified. A restriction of this range in favor of higher doses can be expected, when newer experimental findings are proven by clinical observations.

[The rectal-digital fetal stimulation test--an alternative to fetal blood analysis?].

The rectal-digital fetal stimulation test (FST) is outlined. The indications of FST are the same as for the fetal scalp, blood sampling (FBA). Our clinical experiences suggested, that the fetal scalp blood sampling rate may be reduced by 50% using FST. The main advantage of the FST is that it can be repeated easily without any increased infection risk.

[Reaction and effectiveness of prednisolone in zinc oxide containing dermatologic agents].

The ions of prednisolone and zinc are reacting in a mole proportion of 2:1, and are forming a defined compound according to the formula ZnL2 (L = prednisolone anion). This was proven by potentiometrical equilibrium investigations. Therefore the application of prednisolone within zinc oxide containing dermatics is less indicated. The limited usability and reduced effectiveness of prednisolone containing zinc oxide-dermatics forbid a central production and storage for a longer time.

[Odontoid fractures in childhood].

Using a case report as an example, we report on the special features involved in fracture of the odontoid process in childhood. Although fractures of the cervical spine are very rare in children, this is the most common form. The fracture line will follow in subdental synchondrosis, so that it is possible to obtain complete recovery in nearly all cases. Conservative therapy is indicated if the diagnosis is correct. There is a detailed discussion on the embryonal and radiological peculiarities of this age group.

[Virostatic therapy of chronic hepatitis].

Interferons are glycoproteins which exert a potent inhibitory effect on the replication of hepatitis B virus (HBV). Controlled studies of interferon treatment in patients with chronic HBV infection have shown seroconversion rates, from HBeAg positive to anti-HBe positive, 2 to 3 times higher than without treatment. No enhancement of this effect was observed using combinations of interferon with other antiviral substances. The reason for poor therapeutic response to interferon in the majority of patients with chronic hepatitis B may result from its lost virostatic activity against HBV-DNA located in the cell nucleus.

[Pregnancies following kidney transplantation--clinical problems and genetic studies].

Successful transplantations of the kidney by means of the medical rehabilitation also improve the reproductive rehabilitation of the young patients with distinct wish for children. In these cases the increased infectious, genetic, rejection and tumour risks as well as a limited transplant survival time require particular consideration. On 15 pregnancies among 151 patients in the Regional Dispensary for Patients who underwent kidney transplantations in Halle the clinical and genetic problems are discussed. A patient with preexisting risk factors under interdisciplinary intensive care bore a healthy child who is now 10 years old. The mother is still alive 12 years after the transplantation. Studies on 6 pregnancies in 3 patients who underwent a transplantation revealed 1 normal delivery and 5 interruptions for somatic and genetic indications. 2 female patients are alive 5 and 12 years after transplantation, 1 female patient again undergoes dialysis. From 9 pregnancies by 8 fathers who underwent transplantation 8 children were born (4-10 years) and 1 interruption had to be made. 4 patients live in the 8th to 11th year with the first transplant and 1 patient lives with the second transplant for 8 years. 4 patients again underwent dialysis, one patient died. Human-genetic problems are existing in the autosomal-recessively hereditary Alport-syndrome by the daughter as gene-carrier. 15 analyses of chromosomes on 396 investigated mitoses resulted in chromosomal break rates of 2, 4.7 and 5% in three children at the end of the second year of age, in the mother of 7% in the 4th year and in 2 fathers of 4 and 22% in the 4th and 5th year after transplantation.(ABSTRACT TRUNCATED AT 250 WORDS)

Deposition of terminal C5b-9 complement complexes on erythrocytes and leukocytes during cardiopulmonary bypass.

Hemolysis, leukopenia, a hemostatic deficit, and nonspecific systemic reactions collectively known as the postperfusion syndrome develop in patients who undergo cardiopulmonary bypass. We now report that terminal C5b-9 complement complexes are deposited on erythrocytes and polymorphonuclear neutrophilic leukocytes during cardiopulmonary bypass. Plasma samples taken from 48 unselected patients during and at the end of cardiopulmonary bypass contained raised levels of fluid-phase SC5b-9 complement complexes, indicating that the complement sequence had been activated to completion. Various degrees of overt intravascular hemolysis were observed in all the patients, and lysed erythrocyte membranes were recovered from the blood samples. Immunoassays performed with use of antibodies to C5b-9 neoantigens demonstrated the presence of C5b-9 on red-cell ghosts but not on intact erythrocytes. The appearance of ghosts carrying C5b-9 always coincided with hemolysis. Furthermore, granulocytes isolated from 20 patients during bypass were all found to carry C5b-9 complexes, whereas cells isolated before or 24 hours after surgery carried no C5b-9. The neoantigen-positive material present in detergent extracts of granulocytes sedimented in a broad peak (25 to 40 sedimentation coefficient [S]) in sucrose-density gradients, exactly as did pore-forming C5b-9 complexes. Deposition of C5b-9 on blood cells during cardiopulmonary bypass may be partly responsible for the hemolysis and may augment granulocyte activation by the stimulation of arachidonate metabolism in those cells.

Shaker encodes a family of putative potassium channel proteins in the nervous system of Drosophila.

The Shaker locus of Drosophila contains a very large transcription unit. It is expressed predominantly in the nervous system by multiple, differential as well as alternative, splicing mechanisms into different, but functionally related proteins. The structure of the Shaker transcription unit and the properties of the encoded Shaker protein family provide a molecular basis for A channel diversity in excitable cells.
